SALT RIVER PIMA-MARICOPA INDIAN COMMUNITY 10005 E. Osborn Road, Scottsdale, Arizona 85256 / Phone 480-362-7520 / 12% Gaming Contributions Application Date of Application: July 11, 2018 Organization Name: Duet: Partners In Health & Aging Contact Person: Elizabeth Banta, Executive Director Email Address: banta@duetaz.org Phone: 602-274-5022 x 111 Organization Mailing Address: 10000 N 31st Avenue, Phoenix, AZ 85051 Project Name: Improving Quality of Life for Elders, Grandparents and the Children they are Raising, and Family Caregivers Grant Award Request: $10,000 Please provide a brief description of the project being submitted to the 12% SRPMIC Grant program. We are grateful for the 12% grant funds from Salt River Pima Maricopa Indian Community given in 2017, which are bringing compassion, dignity, and hope to thousands of people who otherwise would have felt alone in their struggles. In 2018-19, through a vital investment from the 12% SRPMIC Grant program, Duet will help 880 children stay out of foster care by nurturing and guiding the grandparents who raise them; bring dignity and independence to 700 homebound adults, through volunteer help such as grocery shopping, visits, paperwork/computer assistance, and rides to doctors; bring hope and strength to 650 caregivers through support groups, workshops, mentoring, guided assistance, a video discussion series, and information resources, so that they will care for themselves while caring for their aging family members; and train and support more than 70 faith community nurses in congregational health programs that will offer over 50,000 health interventions. Research shows that the Baby Boom generation is transforming into the largest elderly population in human history, with 10,000 a day turning 65. By 2030, the number of seniors (65+) in the U.S. will nearly double to 71.5 million. Requests for our crucial, free-of-charge services are constantly increasing. Beginning Date of Project: July 1 2018 End Date of Project: June 30, 2019 Geographic Area to be served: Maricopa County. Duet will help family caregivers of older adults and grandfamilies throughout the county, as well as congregational health programs. Duet’s services for homebound adults are provided in Scottsdale, Fountain Hills, Phoenix, Glendale, and Peoria. Please attach budget on how funds will be allocated. The attached budget shows the distribution of income and expenses for the range of Duet’s free-of-charge services for the fiscal year. Total Amount for Project: $1,627,220. Amount of 12% Request: $10,000 for the period of one year (No Multi-Year Request will be accepted) If the amount of funding received is less than the amount needed for the project, do you have other funds to complete the project? (Please Explain): Duet's funding sources include individuals, corporations, foundations, government sources, and partner congregations. Our 2016-2020 strategic plan incorporates a variety of initiatives designed to identify and grow individual giving. In 2017 Duet's board of directors voted to adopt the Benevon method of fundraising which has strengthened the fundraising capacity of non-profits throughout the nation for decades. In 2017 we initiated Discover Duet gatherings, which serve as a behind-the-scenes view of Duet’s work and the people we serve. In this manner we cultivate the relationships that inspire people to give. The model includes the opportunity for individuals to make five year pledges, thus providing a predictable source of revenue. This process has enabled Duet to successfully engage over 250 new community members. We have instituted Duet's Vision Society - a five year pledge giving society for supporters who commit to give a minimum of $1k per year for five years. Twenty-four people joined in our first year. Our goal is to add at least twenty new members to the Vision Society per year. We participate in Arizona Gives Day as well as Giving Tuesday, and Duet’s annual Poinsettia Tea has been a favorite fundraising event for many years. For 36 years, Duet operated from office space generously provided by Church of the Beatitudes. The building Duet occupied was demolished in 2018 as it required extensive renovations. Relocating was a perfect opportunity to expand reach and enhance service coordination. The new 8,000 square foot office space, near Dunlap and I17, accommodates 22 staff, two interns, three volunteers, has growth for five additional staff, and space for support groups, volunteer orientations, Discover Duet sessions, and workshops. It requires $75,000 more per year than previously paid through – as rent had largely been provided as an in-kind contribution. Although Duet launched a brief campaign to assist with moving expenses, this does not cover the increased rent. Raising sufficient funds to continue services is a constant challenge and your support this year remains critically needed. We strive to have strong and diverse funding from Salt River Pima-Maricopa Indian Community along with a broad range of community partners in service.
