SALT RIVER PIMA-MARICOPA INDIAN COMMUNITY 10005 E. Osborn Road, Scottsdale, Arizona 85256 / Phone 480-362-7520 / 12% Gaming Contributions Application Date of Application: July '19, 2018 Organization Name: Midwestern University Contact Person: Staci Glass Email Address: sglassPmidwestern.edu Phone: 623-572-3286 Organization Mailing Address: 19555 N. 59th Avenue, Glendale, AZ 85308 Project Name: Midwestern University Community Health Outreach Grant Award Request: $125,000 Please provide a brief description of the project being submitted to the 12% SRPMIC Grant program. Midwestern University strives to remove barriers and improve access to high-quality healthcare for Arizona's homeless and underserved populations while educating students about the health and social issues facing poverty-stricken communities. As a result, Midwestern University's Eye and Dental Institutes have established partnerships with UMOM New Day Centers, Arizona Safety Net (a consortium of clinics), NOAH, Native Health, Native American Connections, and other agencies assisting economically disadvantaged individuals. Through these partnerships, faculty and students work together to detect and address vision and dental problems commonly found in homeless and low-income populations. Among those served through our partnerships, Native Americans experience the most severe disparities of any marginalized group. A grant of $120,000, divided between the two clinics, would allow the University to provide free, comprehensive vision and dental care for up to 350 people in one year who are struggling to overcome joblessness, health crises, or homelessness. In addition to a comprehensive examination, additional testing for glaucoma, infection, unexplained vision loss, macular degeneration, and other conditions common in this target population will be provided when deemed necessary. Patients identified with a need for eyeglasses will be fitted with free frames and lenses. Similarly, a comprehensive dental examination would include x-rays and cleaning. Additional services such as tooth extractions, implants, fillings, and the like, will be assessed and administered on a case-by-case basis. An additional $5,000 is requested to cover bus, taxi, or shared ride fares for individuals who are not able to seek care due to lack of transportation. The clinics will track the number of individuals who schedule visits, repeat visits, services provided, and patient demographic information. Using grant funds, Midwestern University intends to provide free, comprehensive care to as many people as possible referred to us by our community partner organizations. Research shows that medically indigent individuals who receive vision and dental care experience an improvement in their overall health and appearance, which, in turn, leads to increased self- esteem, employment, and enhanced function in society. The feedback we have received from the Clinic Directors at UMOM regarding our partnership to date is both rewarding and heartwarming. Their clients have expressed how much it means to them to have access to our services. They have commented on how thorough our examinations are and how the care they receive significantly impacts their lives. Many of our patients from UMOM were previously prevented from obtaining job interviews, receiving training, and the like, because of the serious issues related to their vision and oral health. Similarly, our optometry and dental students, faculty, and staff have commented on how engaging with families from UMOM is professionally fulfilling, thus realizing how their services are changing the trajectory of people's lives for the better. Beginning Date of Project: January 2019 End Date of Project: December 2019 Geographic Area to be served: Midwestern University's Eye and Dental Institutes will provide services to patients referred to us by UMOM and multiple other agencies serving medically indigent populations in Arizona. Please attach budget on how funds will be allocated. Please see attachment for budget and budget justification.
