SALT RIVER PIMA-MARICOPA INDIAN COMMUNITY 10005 E. Osborn Road, Scottsdale, Arizona 85256 / Phone 480-362-7520 / 12% Gaming Contributions Application Date of Application: Organization Name: Opportunity4Kids Contact Person: Alexandra Munter Email Address: Alexandra@Opportunity4Kids.org Phone: 480-340-7017 Organization Mailing Address: 4295 N 75th Street Scottsdale, AZ 85251 Project Name: Leveling the Playing Field in Arizona! Grant Award Request: $25,000 Please provide a brief description of the project being submitted to the 12% SRPMIC Grant program. Our primary initiative is to increase out-of-school time opportunities available to children who would otherwise not be able to afford to engage in after school programs, summer camps, sports and extracurriculars. We facilitate these opportunities by providing Youth Scholarships to pay for registration fees required to participate and/or by providing equipment and uniforms as designated by the out-of-school time activity. It is through word-of-mouth within communities of need, or by case workers and coaches, that Opportunity4Kids will identify underprivileged youth who would benefit from out-of-school time Scholarships. Parent/Guardian/Case Workers of all qualified children will be required to complete an online application vetted by the Opportunity4Kids Director of Operations and then reviewed and processed by a designated Scholarship Approval Committee. Youth Scholarships are awarded on a case-by-case timeline throughout the year. The application process is directly utilized to identify the true need of applicants by requesting financial and health information to determine that Scholarships are awarded to those most disadvantaged. To determine the success of Scholarships provided, Opportunity4Kids asks the Parent/Guardian/Case Worker to complete a post survey to analyze individual progress and development. Through the Scholarship process, Opportunity4Kids’ Director of Operations will utilize a tracking system to record information on children and their families applying for Scholarship. Information gathered will include- school enrollment, age of participating children, participation in government financial assistance programs, family financial situation, disabilities and individual stories. The expectation is that 98% of children provided Scholarships will fall below or within 1.5% of the federal poverty line and at least 75% also receive additional government assistance. Furthermore, at the conclusion of Scholarship activity, 90% will show increase in self-esteem and social skills due to participation in programming. Opportunity4Kids has established a supportive network of partners throughout Arizona by building relationships with nonprofits and recreational organizations that provide influential out- of-school time activities such as: Boys and Girls Clubs of Metropolitan Phoenix, Skate After School, ATA Martial Arts, POPPKiDZ, Scottsdale Soccer Club, Phoenix Center for the Arts, San Marcos Music Academy, ProCamps, Ballet Arizona, The Hubbard Family Swim School, The Little Gym and many more. These organizations support Opportunity4Kids’ goals of providing financial resources by discounting or reducing registration fees required to participate thus facilitating the Scholarship of more children for a longer duration. Opportunity4Kids’ Director of Operations is the only paid personnel with the oversight of a volunteer Board of Directors. Funding provided would go directly to program Scholarships, increasing impact in Arizona by at least 82 children. Beginning Date of Project: January 1, 2019 End Date of Project: December 31, 2019 Geographic Area to be served: Opportunity4Kids focuses on serving youth and their families living throughout Maricopa County.
