I. The above named IGA is hereby amended as specified below:
A. Page 1, Item 3: Agreement amount is changed to an aggregate, not-to-exceed
amount from $776,680 to $860,970.76 (increased by $84,290.76 for the budget
period November 1,2018 through March 31, 2019);
B. Year 5 Budget is replaced with the revised year 5 budget (Exhibit A) and budget
narrative (Exhibit B), The five (5) month budget is for a total of $84,290.76.
C. Section IV, Compensation, 1. Compensation, D. replace the first sentence with:
The Agreement amount is not-to-exceed $84,290.76 for five (5) months of year
five (5).

guidelines.
Dr. Dean Coonrod will provide 10 hours as the Program Director for the MIHS Healthy
Start project. Rate is reflective of the 2018 Federal salary cap for physicians at the
annual rate of $189,600.
