11. PURPOSE:
To provide additional FY 2018 funding to the
Maricopa County Department of Emergency
Management, Cooperative Agreement Number
L18AC00046, Supplement 0001, to Line Item 00020
(see attached Project Plan for a detailed project
description for this Supplement).
This will also incorporate the Application for
Federal Financial Assistance Package, SF 424 and
the SF424A Budget Information dated September 19,
2018, which are incorporated herein, by reference
only.
As stated in the Base Award, Block 12, Period of
Performance is for three (3) years, 08/26/2018
through 08/25/2021.

This supplement corrects the following provision,
"3. TERM OF AGREEMENT" (refer to page 4), in the

Attachment 1 - Revised", to reflect:
"A. The term, or period of performance, of this
agreement shall become effective as of the date
shown on the signed award cover page and may
remain in effect for a maximum of three (3)
years. The BLM will consider continued support of
the project upon; (a) the recipient showing
progress satisfactory to the BLM toward program
goals and the determination by the BLM that
continuation of the program would be in the best
interests of the Government, and/or (b) the
availability of funds."
All other terms and conditions remain in full
force and effect. Ensure SF 425's and Progress
reports are submitted timely as per agreement
terms and conditions.
