AMENDMENT NO. 3 TO THE AGREEMENT BETWEEN
MARICOPA COUNTY,
BY AND THROUGH THE MARICOPA COUNTY ATTORNEY’S OFFICE
AND SAGE COUNSELING, INC.
Amend Section 1.1 – Extend Term of Agreement by TWELVE MONTHS
1.1 Unless extended pursuant to Section 1.2, the Agreement shall terminate on September
30, 2019 at 12:00 midnight, Phoenix local time.
All other terms of the Contract shall remain in full effect for the term of the agreement, unless
otherwise noticed.
IN WITNESS WHEREOF, the Parties have duly executed and delivered this Amendment #3 to
the Contract.
