AMENDMENT NO. 2 TO THE
CONTRACT
BETWEEN MARICOPA COUNTY
ADMINISTERED BY ITS HUMAN SERVICES
DEPARTMENT
AND
LUTHERAN SOCIAL SERVICES
Maricopa County (“County”) by and through the Maricopa County Human Services Department
and Lutheran Social Services (referred to as “Subrecipient”) entered into a Contract on or about
July 1, 2017, for the provision of emergency shelter services for homeless persons in Maricopa
County. The County provided the Subrecipient with $31,803 PY2017 Community Development
Block Grant (CDBG) funds for these services. The Parties executed Amendment No. 1 which
extended the term of the Agreement to June 30, 2019 and provided $31,803 for the period of
July 1, 2018 through June 30, 2019. The County and the Subrecipient may be referred to
individually as “Party” or collectively as “Parties.”
The Parties now wish to amend the Agreement as follows (Amendment No. 2):
I. The County shall provide Subrecipient with an additional $18,275 in PY2017 Emergency
Solutions Grant (ESG) funds for continued emergency shelter services for homeless
persons in Maricopa County.
II. Period of funding availability shall begin on October 1, 2018 through June 30, 2019.
III. This Agreement is subject to A.R.S. §38-511. The foregoing paragraphs, contain all the
changes made by this Amendment. All other terms and conditions of the original
agreement remain the same and in full force and effect as approved and amended.
