AMENDMENT TO RFP CONTRACT 860-15-08B
Between MARICOPA COUNTY By and Through its
DEPARTMENT OF PUBLIC HEALTH and
BIO-REFERENCE LABORATORIES
I. The above named contract is hereby amended as specified below:
A. Cover Page, under 1.0 Contract Term, 1.2., the County extends the contract on a
month-to-month basis for a maximum of three (3) months after expiration,
beginning on July 8, 2018, and ending October 7, 2018.
B. The Contract extension amount is not to exceed $16,600 per month for each of
the 3 months for a not to exceed total of $49,800.
II. All other terms and conditions of the original contract and other amendments will
remain in full force and effect.
