AMENDMENT No. 2 to FACILITIES USE/REVOCABLE LICENSE AGREEMENT
Between
PHOENIX DAY (“Contractor”)
And
MARICOPA COUNTY (“County”)
RECITALS
A. County and Contractor (the “Parties”) are Parties to that certain Facilities Use/Revocable
License Agreement (the “Agreement”) approved by the Maricopa County Board of
Supervisors on October 15, 2015 and subsequently amended September 7, 2016. The
Agreement is for use of County leased premises at 19401 N. Cave Creek Rd., #8, Phoenix,
AZ 85024 and 5422 W. Thunderbird Rd., #6, Glendale, AZ 85306.
B. The term of the Agreement expires September 30, 2018.
C. County and Contractor now mutually desire to amend the Agreement to extend the term.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing and other good and valuable
consideration, the receipt and sufficiency of which are hereby acknowledged, the Parties
hereto agree as follows:
1. The term of the Agreement is hereby extended, to commence October 1, 2018 and continue
until terminated as provided for in the Agreement. The Agreement may terminated by
either Party giving the other Party a 30-day written notice.
2. Exhibit “A” of the Agreement is deleted in its entirety and replaced by the attached Exhibit
“A”, amended to delete one site, attached hereto and made a part hereof.
3. The effective date of this amendment is the date of execution by the Maricopa County
Board of Supervisors
4. The foregoing paragraphs contain all the changes made by this amendment. All other terms
and conditions of the original Agreement remain the and in full force and effect.
