AMENDMENT No. 3 TO LEASE AGREEMENT
Between
HTA-THUNDERBIRD MEDICAL, LLC, LESSOR
And
MARICOPA COUNTY, LESSEE
RECITALS
A. Lessor and Lessee are parties to that certain Lease Agreement No. L-7357 approved by the
Maricopa County Board of Supervisors January 19, 2005 and subsequently amended by
Amendment No. 1 and Amendment No. 2 (collectively, the “Lease Agreement”). The
Lease Agreement is for approximately 4,292 square feet of office and clinic space at 5422
W. Thunderbird Road, Glendale, AZ 85306.
B. As per Section 21 of the Lease Agreement, on August 9, 2017, Lessee delivered a written
Revised Notice of Lease Termination (“Notice of Termination”) to Lessor in order to
terminate the Lease Agreement as of September 30, 2018 due to non-appropriation of
funds.
C. Lessee now desires to rescind the Notice of Termination
D. Lessee and Lessor now mutually desire to continue with the terms and conditions of the
Lease Agreement.
AGREEMENT
NOW THEREFORE, in consideration of the foregoing recitals and other good and
valuable consideration, receipt and sufficiency of which is hereby acknowledged, Lessor
and Lessee agree as follows:
1. The Recitals, by this reference, are hereby incorporated into this Amendment No. 3.
2. Lessee does hereby rescind its Notice of Termination and Lessor agrees to this.
3. This Lease Agreement is subject to termination pursuant to the provisions of A.R.S. § 38-
511, the provisions of which are incorporated herein by this reference.
4. The effective date of this amendment is the date of execution by the Maricopa County
Board of Supervisors.

5. The foregoing paragraphs contain all the changes made by this Amendment No. 3. All
other terms and conditions of the original agreement and as modified by subsequent lease
amendments remain the same and in full force and effect.
6. This Amendment No. 3 may be executed in multiple counterparts, each of which is an
original, but all of which, taken together, constitute a single document.
