Contract No: C-22-18-003-3-03 Amendment No. 3 AMENDMENT NO. 3 TO THE CONTRACT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND THE LODESTAR DAY RESOURCE CENTER Maricopa County (“County”) by and through the Maricopa County Human Services Department and The Lodestar Day Resource Center (referred to as Contractor”) entered into a Contract on or about September 5, 2017 for the provision of baggage storage services at the Human Services Campus. Amendment No. 1 was approved on November 29, 2017 to extend the Contract term to March 31, 2018 and to increase the funding amount. Amendment No. 2 was approved on February 28, 2018 to extend the contract term to June 30, 2018 and to increase the funding amount. The County and the Contractor may be referred to individually as “Party” or collectively as “Parties.” The Parties now wish to amend the Agreement as follows: I. Extend the contract expiration date in paragraph 3.1 from June 30, 2018 through September 30, 2018. II. Increase contract amount in paragraph 7.1 by $16,500 for baggage storage services. Reimbursement requests will be distributed monthly to be billed through September 30, 2018. III. Parties agree that the statutory language of A.R.S. §38-511 are applicable to this Amendment and incorporate the same by reference. IV. The County shall reimburse the Contractor on a Net 10 payment standard. V. The foregoing paragraphs contain all the changes to the Agreement as amended. All other terms of the original Agreement and Amendments not amended hereby, remain in full force and effect as approved and amended.
