Contract No. C-22-18-007-3-01 Amendment No. 1 AMENDMENT NO. 1 TO THE CONTRACT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND AZCEND Maricopa County (“County”) by and through the Maricopa County Human Services Department and AZCEND (referred to as “Subrecipient”) entered into a Contract on or about July 1, 2017, for the provision of rapid rehousing services for homeless persons in Maricopa County. The County provided the Subrecipient with $37,000 PY2017 Emergency Solutions Grant (ESG) funds, $18,000 PY2017 Community Development Block Grant (CDBG) funds and $100,000 in Community Services Block Grant (CSDG) funds for these services. The County and the Subrecipient may be referred to individually as “Party” or collectively as “Parties.” The Parties now wish to amend the Agreement as follows: I. Extend the contract termination date from June 30, 2018 to June 30, 2019. II. The County shall provide Subrecipient with $47,387 in PY2018 CDBG funds, $37,000 in PY2018 ESG funds and $50,000 CSBG funds. Amendment No. 1 total funds shall not exceed $134,387 for continued rapid rehousing services for homeless persons in Maricopa County. III. Period of funding availability shall begin on July 1, 2018 through June 30, 2019. IV. The County shall reimburse the Subrecipient on a Net 10 payment standard. V. This Agreement is subject to A.R.S. §38-511. The foregoing paragraphs, contain all the changes made by this Amendment. All other terms and conditions of the original agreement remain the same and in full force and effect as approved and amended.
