Effective upon signature, it is mutually agreed that the Contract referenced is amended as follows:
1. Pursuant to Terms and Conditions, Provision Six (6), Contract Changes, Item (6.1) Amendments, Purchase
Orders and Change Orders, The Scope of Services: the Contract is amended as follows:
1.1 Provision Four (4), Tasks, Item 4.2 is amended to read as follows:
4. TASKS
4.2 Maricopa County Department of Public Health (MCDPH) will provide Arizona Department of Health
Services (ADHS) annual reports of the number of enteric disease or other infectious disease cases
investigated with completed key variables.
1.2 Provision Five (5), Deliverables 1, Item 5.1.3 is added to read as follows:
5.1 DELIVERABLE 1: DISEASE INVESTIGATIONS
5.1.3 Provide a Healthcare Associated Infections/Antibotic Resistance (HAI/AR) annual summary
including, but not limited to, activities that involve the coordination of approval, submission, and
test result distribution of isolate/specimens to Arizona State Public Health Laboratory (ASPHL)
and Antibotic Resistance Lab Network (ARLN).
