Dear Tenant:
Effective as of December 28, 2015, landlord has contracted with ACM, LLC (Arizona Commercial
Management) to manage the property. The specific ACM employees who will be responsible for the daily
operations of the property are myself, as the Property Manager and Ruben Osorio and Angel "'aro as the
Roving Building Technicians. However, please direct all inquiries as well as after-hour emergency calls to
ACM's main telephone number, 602.586.2880.
As of format), 1, 2016, monthly rental payments (along with notices and correspondence) should be
addressed and sent to the address below:

To ensure that communications are directed to the correct individuals within your company, enclosed is a
tenant information sheet; thus at your earliest convenience I would appreciate you supplying the information
requested and returning to my office via fax (602.586.2881) or emailing to achavezRaz-c-m.com . Monthly
rent statements will be emailed to you so please be sure to include your accounting email address.
I have also attached a tenant survey form that would be helpful for us if you would complete and return in
order that we better understand the operations of the property thus far, as well as your satisfaction with the
services provided - please return to my attention via fax (602.586.2881) or email to achavez@az-c-m.com .
Lastly, attached are also the insurance requirements which should be emailed to achavezPaz-c-m.com as
soon as possible to ensure the correct documentation is on file regarding your operations in and about your
leased premises.
All of us at ACM will use our best efforts to minimize any inconvenience to you and your staff during this
transition. I welcome any comments, suggestions or concerns you have regarding your tenancy at the
property.
