AZCEND, formerly Chandler Christian Community Center, a non-sectarian nonprofit
organization established fifty years ago, is pleased to present this request for qualifications for
Page Park Center Non-Profit Operator in Gilbert, Arizona. Based on fifty years of serving the
community, including over 5 years delivering Community Action and Senior Program services in
Gilbert, AZCEND is well-positioned to further expand its services in Gilbert, AZ. This location is
specifically beneficial to the community due to its proximity to an underserved area according
to the 2014 Town of Gilbert Needs Assessment.
Community partnerships are a significant component of AZCEND's programming and would
substantially enhance the range and depth of services available to families visiting the Page
Park Center. Stakeholders have confirmed their interest and commitment to this location and a
partnership, and plans for the expansion of services could move forward smoothly if AZCEND
was selected as the operator.
6.1 Project Understanding and Approach
AZCEND has an extensive history serving the needs of the East Valley's most vulnerable
families. AZCEND is proposing to provide direct services through the CAP office and Family
Resource Center as well as managing the partnerships and joint operating agreements with
compatible third party service providers.
The Gilbert CAP, administered currently by AZCEND, already provides financial assistance to
prevent homelessness and utility disconnect and provides case management, financial coaching
and referrals to other community resources for low-income Gilbert residents. Typical households
seeking financial assistance through the Gilbert CAP have very limited or no cash resources to
serve as personal safety nets. Gilbert CAP participant demographics paint a clear picture; 45% of
applicants had no post-secondary education; 67% had at least one child in the household; 54%
were receiving food stamps; and 94% had total household income at or below 150% of the federal
poverty line (less than $36,375 annually for a family of 4). These families often just need small
amounts of financial assistance, such as a one month rent payment, to maintain a stable living
environment. CAP also serves many chronically poor elderly and/or disabled individuals living on
fixed incomes. AZCEND's CAP service delivery model is unique in Gilbert as our expertise and
access to funding to conduct intake, referral, direct assistance and case management is
unparalleled in this community.
AZCEND has been selected as the provider of CAP services in Gilbert, Chandler and Queen Creek by
Maricopa County Human Services Department, the federally designated Community Action
Agency in the region. AZCEND CAP staff members have been providing CAP services in Gilbert for
many years. The funding streams for this program are based upon the CAP's long term
partnerships with Maricopa County Human Services, Valley of the Sun United Way, Town of
Gilbert and private donations. Each of these sources provide operating funds in addition to the
funds for direct assistance provided to eligible CAP clients.
4
In FY 16/17, the Gilbert CAP served 1,730 unduplicated individuals and provided a total of 870
direct financial assistance payments, totaling $341,440 to Gilbert residents utilizing all of our
available funding sources. The Gilbert CAP also provided 1,629 referrals for food boxes, clothing,
job training, budgeting, home repairs and many other community resources.
AZCEND also intends to expand our Family Resource Center (FRC) programs into Gilbert, subject to
securing additional funding. The Gilbert FRC will serve as a community hub for connecting
families with children birth to age five to information, resources, and services; parenting
workshops; serve families with children birth to age five in intensive case management using
family navigator services; and assist families with children birth through age five with affordable
health insurance enrollment, retention, and renewal. The expected results are improved
parenting skills and social supports for Gilbert families, increased knowledge of child
development, and support for children's school readiness.
As an existing FRC in Chandler, AZCEND has developed the necessary capacity and
infrastructure to offer responsive family-centered services in the Town of Gilbert. Since
launching the Chandler FRC in 2010, AZCEND has engaged a number of organizations and
formed numerous partnerships that supplement services not offered by AZCEND for families in
need. Many of these partnerships will be extended to the Gilbert FRC model.
The Town of Gilbert's recent Human Services Needs Assessment (The Williams Institute, 2014),
refers to conditions of "prosperity for many, challenges at the margins." Specifically, "For the
less visible part of the population ... (more than one-third of the Town population) experiencing
struggles and grave challenges, the needs are palpable." The Town of Gilbert's most-in-need
populations were found to be reluctant to voice their needs "out of concern for what they
perceive as a social stigma that accompanies not being able to measure up," (The Williams
Institute, 2014). This report found:
O 14,000 Gilbert residents living below the federal poverty level
• More than 26,000 residents qualify as low income
O Over 10,000 students qualify for free/reduced lunch program
e 13,000 residents with chronic special needs
• Approximately 12 calls to the Police Department each week for suicides or
attempted/threatened suicides and another 4 calls for emergency mental health issues,
28 domestic violence calls per week, and significant numbers of emergency room visits
for alcohol and substance abuse issues.
The report rank orders the criticality of need for added services by population groups, as
identified by human service recipients and providers participating in the study. Families and
individuals in crisis ranked highest as having the greatest need for immediate assistance, while
low/moderate income individuals and families ranked third highest in need. Gaps in services
for families and individuals in crisis, the population group ranked highest for greatest need was
also identified. Additionally, the need for centralized resources was identified as a strong need
5
for Gilbert. In response, AZCEND will work with other stakeholders to bring services to meet
the needs and goals of the community and this project.
Potential partners for this project include Dignity Health, Mission of Mercy, St. Vincent de Paul,
WIC, Eve's Place, Goodwill and Southwest Behavioral Health. These partners will provide
nutrition services, domestic violence support, employment services, basic and preventive
medical and dental and behavioral health to vulnerable Gilbert families and individuals. These
partners are already identified and either committed to this project or partnering with AZCEND
at our Chandler location. Additionally, we are open to exploring partnerships with other
organizations providing needed services for Gilbert families and individuals.
As the Non-profit Operator, AZCEND will perform Facility Operator and Administrative Services
for the Town of Gilbert in addition to providing CAP and FRC services. AZCEND will assume
responsibility for coordinating and operating Page Park Center as a location for programs that
are CDBG eligible activities, meet Town guidelines, and meet the identified needs of Gilbert.
AZCEND does not have any conflicts of interest in providing services for the Town and is
currently already providing Senior Nutrition and Community Action Program services for the
Town.
AZCEND will collaborate with community partners and develop partnership agreements. These
agreements will designate reporting requirements to ensure the numbers and percentages of
those served qualify as low income in order to meet CDBG requirements. The agreements will
also specify expectations regarding service provision to ensure the services meet those
identified in the needs assessment. Should a partner choose to leave this location, AZCEND will
work with other community partners to identify a new partner that will fill the missing need
and ensure the building is being operated to capacity.
The needs of the community will be reviewed every 4 years in partnership with the Town and
other Stakeholders to ensure the programs and services being offered are still relevant.
Additionally, AZCEND will solicit feedback from clients through the use of surveys and an
Advocacy Council, a group of community members who work collaboratively with AZCEND to
work on community issues. AZCEND can meet the expectation to provide presentations to
Gilbert Town Council and key stakeholders, as will be identified in the final contract.
Additionally, all documents will be provided in electronic format compatible with Town of
Gilbert software.

6.2 Experience:and Capability far Work
AZCEND was organized in 1966 to provide for the emergency hunger needs of Chandler's most
vulnerable residents through the Chandler Food Bank. Today AZCEND operates from its original
downtown Chandler neighborhood in the heart of the city's low and moderate income areas,
and in additional sites in Chandler and Gilbert.
AZCEND operates one of the nation's oldest food banks and is Chandler's largest basic needs
service provider. Our mission is to change lives by nourishing minds and bodies to create a
connected, thriving community. Our vision is to have a healthy and productive community built
on a foundation of dignity and hope. Primary goals include reducing hunger and empowering
individuals and families to develop their capacity for greater self-sufficiency, by helping people
as they move from crisis to stability and ultimately prosperity.
8
AZCEND has grown dramatically in size and scope since 1966. Last year, we celebrated 50 years
of service to our community. Over the years we expanded the food bank facility to
accommodate a larger food warehouse and indoor and outdoor program space, offices,
classrooms, and additional parking. Programmatically, we enhanced food resources by adding
specialty items for infants, toddlers and seniors. In 2010 we began to deliver additional
programs under a comprehensive Family Resource Center (FRC) model. FRC programs address
early childhood literacy, family education, and health/wellness. In 2012 AZCEND assumed
ownership and financial responsibility for Chandler/Gilbert Community Action (CAP) and Senior
Nutrition Programs as these programs were closed by the original administering agency.
AZCEND absorbed 22 employees and responsibility for an additional $1.5 million in program
expenses and over 14,000 individuals served through the CAP and Senior programs (many of
whom also access Chandler Food Bank services). In FY2012/13 we launched the Interfaith
Homeless Emergency Lodging Program (I-HELP) to provide emergency overnight lodging for
homeless adults.
AZCEND provides for the basic and emergency needs of individuals and families through a
number of programs. At AZCEND residents are able to access emergency and supplemental
food through the Chandler Food Bank; emergency rent and utility assistance, enrollment of
public benefits, job search resources, financial literacy training and VITA tax assistance, and case
management through the Community Action Program; and home delivered and congregate
meals through the Senior Nutrition Program. Our I-HELP Program provides homeless individuals
with safe overnight lodging, meals, case management and connections to community services
through partnerships with faith and community organizations. The Family Resource Center
offers educational programs (early childhood literacy, parenting and ESL classes, and school
supplies), health/wellness services (wellness checks, HIV/STD testing, access to children's
fluoride treatments), family play groups and holiday toy drives. AZCEND offers the convenience
of a one-stop environment to address a myriad of individual and family basic needs.
Our FY2017 accomplishments are many. We collected 1,012,290 pounds of donated food for
distribution in 16,537 emergency and supplemental food boxes with enough food to provide
892,083 meals benefiting 12,143 unique individuals. We recruited and coordinated the work of
1,346 community volunteers who donated 32,397 volunteer hours. Our Family Resource Center
provided almost 5,000 hours of instruction through comprehensive programming. I-HELP
served 428 individuals with 8,523 bed nights. Our CAP office provided 2,606 direct financial
assistance payments for rent and utilities totaling $980,750. Our Senior Centers provided
68,361 meals to 1,213 unduplicated individuals and 1,393 activities. Almost 780 children
participated in Operation Santa in 2016. In July 2016, we partnered with For Our City-Chandler
and others to assist over 3,000 children with backpacks, school supplies and clothing through a
collective effort.
AZCEND management is familiar with CDBG guidelines and has received CDBG funding
previously for our Senior Nutrition program, our Tenant Based Rental Assistance Case
Management Program and 2 building expansion projects.
9
isca ResponSibility
AZCEND's extensive experience with grant and project management provides the Town of
Gilbert with a high degree of confidence over our ability to provide appropriate management of
this project. AZCEND has a record of reliability, fiscal responsibility and extensive experience
with projects such as this one.
Our internal controls (including those listed below) are designed to help AZCEND meet financial
goals as well as ensure reliable financial reporting, accurate recording of transactions,
compliance with laws and regulations, and prevent a loss of resources. Fiscal and accounting
controls include but are not limited to:
• An identified grant budget and a method for monitoring actual costs against the budget.
• A financial management and accounting system.
• Internal policies and systems ensure effective management of funds and compliance with
grant requirements.
• Independent auditors report on compliance for each major program and report on
internal control over compliance required by OMB Circular A-133.
• Applicable OMB cost principles and the terms and conditions of the grant award are
followed in determining the reasonableness, allowability and allocability of costs.
• A CPA firm performs monthly accounting and payroll preparation services for the
organization.
• Accounting records are maintained on a current basis and balanced monthly. Bank
accounts are reconciled monthly by someone other than the person who signs the
checks.
• Source documentation is maintained in financial records.
AZCEND has partnerships with Area Agency on Aging, Maricopa County Human Services Division,
the City of Chandler and many others (see attachments for letters of reference). Additionally,
AZCEND has partnered with the Town of Gilbert for Community Action and Senior Center
Programs. AZCEND also has strong community partnerships with Dignity Health, the Boys and
Girls Club, ICAN, Eve's Place, Goodwill, and Southwest Behavioral Health.
