Project Background This change order accounts for the following scope items: I. Extending the main Gilbert Road Bridge from 1,445 feet to 3,685 feet. 2. Adding a new 450 foot bridge "Northern Reliever Bridge". 3. Preparing new 60% Plans, Specifications, and Cost Estimate. 4. Additional effort to complete 95%, 100%, and Sealed submittals with new project definition. 5. Providing Post Design services. Project Purpose The purpose of the change order is as follows: 1. After completing additional hydraulic analysis, utilizing 2-D modeling (Change Order #3), 11DR developed numerous alternatives which focused on key objectives of not worsening flooding of properties outside of the ROW, including along Roma Ave during a 100-year storm event and keeping all proposed improvement within ROW. After considering all of the information, MCDOT and project stakeholders selected an alternative (Alternative 3A) which extended the main Gilbert Road Bridge over the Salt River from 1,445 feet to 3,685 feet. This extension results in the northern abutment moving further to the north. The southern abutment will remain in the same location as previously identified and the horizontal alignment will remain the same. 1 As part of Alternative 3A, it was determined that a new bridge structure is needed approximately 2,000 feet north of the main Gilbert Road Bridge. This new bridge "Northern Reliever Bridge" allows for the conveyance of various flood water that is "pushed" up towards Roma Avenue during a 100-year storm event to drain underneath Gilbert Road rather than perpetuate flooding of the areas along Roma Avenue. Based on the analysis, a proposed bridge (or equivalent hydraulic structure) with an opening length of 450 feet is needed to provide adequate relief drainage. 3. With the changes to the main bridge, the addition of a completely new bridge structure, and the associated changes with the roadway profile to accommodate these changes, a new 60% design plan submittal is required. The critical path for this project is getting the Conditional Letter of Map Revision (CLOMR) submitted and approved. The scope changes associated with Alternative 3A directly impact the evaluation current design package and the CLOMR thus requiring a completely new 60% submittal. 4. With the additional length of the main Gilbert Road Bridge and the new Northern Reliever Bridge, additional efforts will be required in developing the 95%, 100%, and Sealed project submittals as a function of the increase infrastructure and projects over design complexity. Time has been included to participate in additional agency and stakeholder coordination meeting to share the findings and recommendations of this project. Additional time has been added to support public outreach and public meetings to obtain community approval for the recommended alternatives. In conjunction with mcDoT, HDR will hold approximately six (6) additional monthly progress meetings where all stakeholders will be invited and project progress, issues and direction will be discussed and documented. Time is also included for a new 60% comment resolution meeting. 300 Revised 60% Design Plans, Specifications, and Cost Estimate Due to the significant level of change to the scope of the project, primarily more than doubling of the main Gilbert Road Bridge and the addition of the new Northern Reliever Bridge, located slightly south of Roma Avenue, a new 60% design plan submittal is required. While certain elements of the previous 60% submittal (submitted July 19, 2017) can be resubmitted without modification, the majority of plans require re-work. A description of the modifications for each discipline are detail below. 300.1 Civil This task consists of civil design efforts associated with updating the roadway layout, performing additional design associated with the change in the bridge layout, performing additional design associated with the addition of the northern reliever bridge, revising the drawings that were included in the original 60% submittal, and resubmitting the 60% submittal. Unless noted herein, the scope provisions for the civil final design and development of plans included in the original contract still apply. This includes design specifications, design guidelines, construction specifications, and submittal requirements. The roadway design will follow the current MCDOT Roadway Design Manual, standards and guidelines and utilize current MAG and MCDOT standard details as much as practical. Roadway cross sections will be prepared at appropriate locations to provide accurate earthwork calculations at regular intervals along the alignment. The design will rely on the topographic surveys, mapping and digital terrain models (DTM) provided by MCDOT. The roadway will be designed using InRoads software. Earthwork will be calculated using the average end area method. The following is assumed for the new civil layout: (1) the roadway profile grade will be changed, resulting in modifications to geometry; (2) roadway design coordination with other technical disciplines is required to account for the extend bridge and the new reliever bridge; (3) the location of the south abutment of the main Gilbert Road Bridge will remain unchanged; (4) the east maintenance road will be combined with the drainage ditch/channel to reduce impacts to existing right-of-way; (5) maintenance of traffic and construction sequence plans will need to be revised to account for the extended bridge and the new reliever bridge. HDR Drainage Lead shall participate in public meeting and stakeholder outreach in support of communications with MCDOT for proactive messaging of project findings and recommendations of potential FEMA outcomes. 300.2.4 Drainage Construction Plans (Revised 60%) HDR shall facilitate the coordination and development of the construction plans, details, and specification for the design package of the Revised 60% review of the project. The HDR drainage lead shall be involved in ongoing discussions and review of plan components to support the early identification of potential conflicts or challenges. 300.2.5 Conditional Letter of Map Revision Based on the new project configuration, the Conditional Letter of Map Revision (CLOMR) will require significantly more effort and coordination with Flood Control District of Maricopa County (FCDMC), FEMA, and MCDOT than was originally scoped for the project. In order to assist with the FEMA review and approval process, an in-person meeting with the assigned FEMA reviewer is proposed. The purpose of this meeting is to provide a detailed explanation of the project, the special conditions associated with the CLOMR, and to answer questions early to prevent a project review stall out. The location of this meeting will not be known until FEMA has assigned the case to a specific review. It is anticipated that the meeting Project No.: TT0339 Contract No.: 2015-039 Revised: April 2018 Page 10 of 22 Scope of Services Project Specific will require travel out of state. This scope provides for travel, including airfare, with per diem. For two I-LDR staff and one representative from subconsultant J2. Additional representation from MCDOT and FCDMC will be required as well. This scope and budget does not include recovery costs for mcDorr and FCDMC, it is understood those services will be funded separately by others. 300.3 Structural 300.3.1 Bridge Final Design and Plans The layout of the Main River Bridge will be revised from nine spans to an assumed layout of twenty-three spans. This task consists of bridge design efforts associated with updating the bridge layout, performing additional design associated with the change in the bridge layout, revising the drawings that were included in the original 60% submittal, generating new drawings required for the new bridge layout, and resubmitting the 60% submittal. Unless noted herein, the scope provisions for the bridge final design and development of plans included in the original contract still apply. This includes design specifications, design guidelines, construction specifications, and submittal requirements. Contrary to the scope of work in the original contract, but as shown in the original 60% submittal, the bridge will consist of a precast concrete Bulb Tee girder superstructure supported on 2-column piers founded on drilled shafts. Due to the possibility that the hydraulic design conditions (i.e. flow, velocity, scour, etc.) for the north half of the proposed bridge may not be as severe, and that designing to meet these conditions will reduce cost, it is assumed that a new/separate pier design is included. The following is assumed for the new twenty-three span bridge layout: (1) An updated Bridge Selection Report will not be required; (2) The bridge horizontal alignment and skew of the piers and abutments will match the original 60% submittal; (3) The span lengths will match the original 60% submittal; (4) The profile grade will be changed, resulting in modifications to geometry; (5) The size and spacing of columns at all piers will not change and will match the original 60% submittal; (6) The size and depth of the pier drilled shafts for the spans included with the original 60% submittal will not change; (7) A new drilled shaft size and depth will be required for the piers within the extended limits of the bridge; (8) The size and spacing of the drilled shafts at Abutment 1(south end) will remain unchanged; (9) The size and spacing of the drilled shafts at the new Abutment 2 (north end) will match Abutment 1; (10) The wing walls at both abutments will be similar in length as shown on the original 60% plans, thus wing wall redesign will not be required. However, during the original 60% design, it was determined that retaining walls will be necessary beyond each abutment wing wall (not feasible to extend wing walls past the slope catch point). The layout/design of these retaining walls (1 sheet) is included with this change order since it was not included in the original scope of work and fee proposal; Project No.: TT0339 Contract No.: 2015-039 Revised: April 2018 Page 11 of 22 Scope of Services Project Specific (11) The new twenty-three span layout will consist of 7 three-span frames and 1 two- span frame, thus requiring consideration of design and details for a two-span frame (the original 60% submittal only consisted of three-span frames); (12) A deck drainage system will not be required. The following list of bridge drawings identifies the existing drawings included as part of the original scope of work (for the bridge length included in the original 60% submittal) and new drawings required for the new bridge layout. For the existing drawings, the list notes which drawings are assumed to required revisions and which drawings will not require revisions. The remainder of the effort associated with coordination with external stakeholders is included in the original contract. Project No.: Tr0339 Contract No.: 2015-039 Revised: April 2018 Page 13 of 22 Scope of Services Project Specific 300.4 Geotechnical With the length of the main bridge expanding from 1,445 feet to 3,685 feet, a significant amount of geotechnical investigation will be required to account for the increased number of bridge foundations. The following descriptions accounts for geotechnical investigation of both the main bridge, the new northern reliever bridge, and the roadway embankment segment between the two bridges. 300.4.1 Exploration & Access Plan Preparation Terracon, accompanied by MR, will perform a site visit to evaluate the prospective boring locations, site and access constraints, traffic control requirements, and other logistics of the field investigation which require consideration. HDR will prepare a draft Geotechnical Investigation and Boring Plan which will depict the boring locations and depths, proposed access routes, and estimated areas of ground surface disturbance, and include a description of the drilling equipment and methods, and will submit the draft Plan to MCDOT (including to MCDOT Materials Group), and the HDR Design Team. The Plan will be a modification of the boring plan developed for the investigation of the original (60 Percent Design Phase) nine-span main river bridge layout. HDR will revise the draft Plan in response to review comments from MCDOT Materials Group and the team, and submit a final Plan to MCDOT and the design team. 300.4.2 Environmental & Encroachment/Access Permits Coordination Environmental Review & Clearances: The Geotechnical Investigation and Boring Plan prepared by HDR will be submitted to MCDOT Engineering Division, Environmental Program Branch planners for their use in obtaining environmental permits for geotechnical field work, including for the MCDOT environmental clearance and applying for the SRPMIC Environmental Review and clearance. Communications and coordination with MCDOT Environmental Program Branch will be required during the permitting process for the geotechnical field investigation. Further, HDR will coordinate with MCDOT and the SRPMIC to determine if SRPMIC representative(s) need to be present during the geotechnical field work. It is assumed that advance notification of SPRMIC by HDR will be required prior to initiation of the field work. An estimate of the area of disturbance due to the field investigation will be prepared by Terracon and submitted as part of the environmental clearance package. The area of disturbance estimate will include the boring locations themselves, access route to each boring location (travel off of existing roadways), and each boring work zone including drill spoils. All project field personnel will have completed the required cultural sensitivity training course administered by SRPMIC. Encroachment Permits: Terracon will apply for and secure an encroachment permit from MCDOT for work within the Gilbert Road right-of-way, and an access permit from SRPMIC for work within the Community's right-of-way, including preparing and submitting a traffic control plan for proposed borings adjacent to the existing roadway. Traffic lane closures are not anticipated to be needed for this field investigation. Borings along the existing Gilbert Road will be located on or adjacent to the existing roadway shoulder and include shoulder closures and "shoulder work ahead" signage. No borings are anticipated to be located within existing MCDOT or SRPMIC paved roadways. ADWR Permits & Boring Backfill/Abandonment: For the 60 Percent Design field investigation, it was determined by Terracon that ADWR permits for borings which encounter groundwater were not needed, as ADWR indicated they have no permit jurisdiction on SRPMIC lands. Regardless, the top 20 feet of all bridge borings will be backfilled with cement-bentonite Project No.: TT0339 Contract No.: 2015-039 Revised: April 2018 Page 14 of 22 Scope of Services Project Specific grout in accordance with ADWR abandonment of groundwater pavement will will be be backfilled with drill cuttings, requirements. and borings Borings which do not reach backfilled with 1/2-sack CLSM. located within 20 feet of the edge (provided Utility Clearances: by 1VICDOT) Borings will be located in the field by Terracon may be adjusted depending and upon surface features.
