Narrative: A narrative of no more than two (2) pages describing the following
The proposed program or project, and whether it is a new or continuing project:
Since 1997, Mission of Mercy’s (MOM) mission has been to restore dignity to the sick, working poor, under-
insured, and uninsured by providing free healthcare and “healing through Love.” In the simplest of terms, we
provide a no-cost medical home for Maricopa County’s uninsured and underinsured families and individuals for as
long as they need it – no questions asked. Our highly efficient, volunteer-driven mobile healthcare delivery model,
appropriately called “Access to Care,” is 100% privately funded and uses no public or tax dollars. For 20 years,
MOM has been committed to restoring dignity, empowering patients, and strengthening communities.
MOM currently operates seven free mobile clinics out of churches, schools, and community centers in Avondale,
Central Phoenix, Maryvale, Mesa, Chandler, South Phoenix, and South Central Phoenix. Our direct care model
utilizes over 250 medical and non-medical volunteers managed by two full-time staff physicians and three part-time
staff nurses. To date, MOM’s professional staff and compassionate volunteers have provided more than 336,000 free
prescription medications and 220,000 individual patient visits to vulnerable Maricopa County families.
Each of our free clinics are located in neighborhoods of severe unmet need – those that rank highest on Dignity
Health’s Community Need Index (CNI). The CNI measures barriers related to income, transportation, access to
culturally appropriate health and community services, cultural and ethnic identity, socioeconomic and education
levels, and language barriers. MOM clinics subscribe to a holistic “medical home” model for these vulnerable
individuals, with more than 65% of our patients designating MOM as their ongoing primary care medical home. The
U.S. Department of Health and Human Services defines a medical home not simply as a physical location but as a
model of primary care that provides four core functions (Accessible Services, Comprehensive Care, Patient-Centered
and Coordinated Care). Since our inception, MOM has steadfastly embodied an innovative model of community-
based healthcare using the talents of gifted volunteers and the leverage of local investment partners like Ft.
McDowell Yavapai Nation.
Through MOM's "Access to Care" program, two fully equipped mobile medical units bring the doctor directly to
patients in neighborhoods of documented unmet need throughout Maricopa County. MOM's established clinics
currently serve 3,500 annually. With the ultimate goal of improving health outcomes for patients, MOM treats
patients' medical conditions, monitors their progress and provides ongoing free prescription medications and health
education at clinics operating 48 weeks per year.
Our mission is to restore dignity to the sick, working poor and medically underserved by providing health care and
"healing through Love." This mission has proven the test of time with decades-long partnerships and more than 250
volunteer doctors, nurse practitioners, RNs, bilingual medical interpreters, registration attendants and drivers, who
make our work sustainable and highly impactful. For 20 years, Sonora Quest Laboratories has donated all patient lab
testing, and Dignity Health has been a referral partner, specialty care provider and funder. Additional partners
include Banner Health hospitals, Esperanca and Mobile On-site Mammography.
The purpose of the program or project and how it meets one or more of Fort McDowell’s funding priorities (i.e.,
education, public safety, health, environment, and economic and community development):
Thousands of Arizonans lack access to quality, consistent health care. About 1-in-5 Arizonans is uninsured, and 1-
in-4 is underinsured. Since the expansion of Medicaid in Arizona, many low-cost and free clinics have closed.
Moreover, Arizona's children rank at the very bottom of the nation for being insured. These vulnerable children and
families often forego routine wellness checks and cut back their use of prescription medications to manage chronic
health conditions such as diabetes, asthma and hypertension. This results in undiagnosed and untreated chronic
illness; unnecessary, costly emergency care for illnesses that have gone untreated and have become acute; lost
wages; lost jobs; stress on fragile family units; and, in many cases, preventable premature death.

Through our "Access to Care" program, MOM brings the doctor directly to patients in neighborhoods in critical
need throughout Maricopa County, serving 3,500 under- or un-insured. MOM treats patients' medical conditions,
monitors their progress and provides ongoing free prescription medications and health education at clinics operating
48 weeks per year; a model program helping us reach our ultimate goal of improving health outcomes for all
patients.
The need that the program or project aims to fulfill, the population targeted, and the estimated number of
people who will be served by the grant:
This project will support all of our sites, including our newest site which opened at the Chandler First Church of the
Nazarene on March 3, 2018. The downtown Gilbert clinic is slated to open in the fourth quarter of 2018, in a new
family service center that will combine medical care, children's dental and immunizations, patient education, food
access, financial assistance and behavioral health services. Mission of Mercy was invited by the Town of Gilbert
and Dignity Health's East Valley hospitals to be the anchor medical partner in this innovative family resource
center.
This project will also help to expand our Community Connections program to address social determinants that
impact our patients' long-term health outcomes. Our patients are the uninsured and the underinsured. More than
56% are women and children; more than 67% are Hispanic; and, all face far more than a lack of health care-they
wrestle with challenges such as poverty, homelessness, poor nutrition and job insecurity. Since 2011, our
Community Connections program has provided additional on-site health education programs, patient navigation to
government services, ancillary health services and community resources. This grant would provide additional
financial resources to strengthen the program with new services and service partnerships. As we have successfully
proven for the last 20 years, we are confident that our strategic health care and partnership model will have positive
community impact for generations to come.
At a remarkable cost of just $85 per patient visit, Ft. McDowell’s investment of $25,000 will pay for 294 unique
patient visits, at no cost to the clients. Patients, particularly those with chronic illnesses, such as diabetes or heart
disease, needing multiple visits, rely and often get visibly emotional when visiting the “medical home” MOM has
created in their very own neighborhood.
The partners in the program or project, and other entities providing funding and support (please indicate
whether the organization will be submitting applications to any other tribes):
Mission of Mercy has established many decades-long collaborations with community partners. Since launching our
first clinic in 1997, Sonora Quest Laboratories (SQL) has donated free, unlimited lab services to all MOM patients.
SQL is further committed to continuing their pro bono partnership as we expand to new clinic sites throughout the
Valley. Dignity Health is a funder, specialty medical care provider and patient referral partner; MOM takes patients
who are referred from Dignity and provides them with a medical home, keeping them out of the hospital and costly
emergency rooms with ongoing primary medical care. We also partner with churches and community centers who
donate free clinic space, volunteers and program funding. Additional partners include Estrella Mountain Community
College, who provides certified bilingual medical interpreters for our Spanish-speaking patients, and Esperanca,
whose community health educators provide on-site, culturally sensitive health information, demonstrations and
resources to families.
The timetable for implementation of the program or project.
Our medical program is ongoing, however the time frame for this grant would assist Mission of Mercy in its FY 19,
from July 1, 2018 – June 30, 2019. As an organization that cares deeply for the health and well- being of our
neighborhood residents, Mission of Mercy would be proud to partner with Ft. McDowell Yavapai Apache Nation on
MOM’s “Access to Care: Bridging the Healthcare Gap for Children and Families.”

Attach a detailed budget for the program or project, a statement addressing the need for long-term funding
(if applicable), and any other relevant attachments that will give the Grant Committee a fuller understanding
of the program or project. The budget is required; however, other attachments are optional.

Budget Narrative: Your support is critical to help offset the cost of patient care, including Dr. visits, Rx
medications and patient education, which are not covered by other funders. We utilize a blended revenue
strategy and seek funding from individuals, foundations, corporations and fundraising events. Partnerships
with other non-profit service organizations, buoyed by over 250 volunteers, provide critical resources, in-
kind donations and support services to significantly offset program costs. Your support will be leveraged
with other community resources to provide underserved children and their families with the healthcare they
need to lead full, vibrant lives.

Our pastor knew of our situation and told us about Mission of Mercy. I asked about medications. When you can’t
put food on the table, you use what money you have to buy cheese and crackers, not diabetic strips and insulin.
He told us the medications are free.
So many times we had been told, “We can’t help you.” That happened over and over. It is demeaning. And

Nearly 31 million Americans—most covered by an

often skip needed health care. Too many families are
faced with choosing between medications and paying bills.

EMPOWERED PATIENTS
Our little girl, Jenny, has asthma. She was in the hospital three times over the summer. One time she stopped
breathing and nearly died.
When our daughter’s asthma gets so bad that she cannot breathe the only place we can go is the emergency room.
I work installing insulation. I make too much money for AHCCCS and not enough money to pay for health
insurance. A social worker told us about Mission of Mercy.
When we walked into Mission of Mercy, we were very apprehensive.
But they didn’t ask for our papers. And the most important thing, they
didn’t ask for money.
Total Patient
I couldn’t believe there was someone there who spoke Spanish. Through an
interpreter, my wife told Dr. Smith about my daughter’s asthma. He was Visits Since
able to teach us how to use the nebulizer, so we wouldn’t need to go to the ER.
Inception:
When we were done, Dr. Smith got up from the table and walked with us to make
an appointment for our next visit. I have never had a doctor do that. Thank you 204,129

—Barry and Gerri Knight, former patients
Nearly 31 million Americans—most covered by an
employer or private health plans—are underinsured and
often skip needed health care. Too many families are
faced with choosing between medications and paying bills.

Hospital Emergency Rooms
are the most expensive form
of health care.
We aim to conserve resources by
being an ongoing medical home
for people who otherwise would
have gone to the ER.
Your impact and support
