Attn.: Prop 202 Grant Application
March 28, 2018
Dear Ms. Enos:
On behalf of Duet: Partners In Health & Aging, we are submitting a request for Prop 202 funds in support of Expanded
Outreach for Services to Seniors in the amount of $10,000. Expanded outreach efforts will support vitally needed, free of
charge services to Maricopa County’s rapidly growing aging population, in the areas of homebound adults, kinship caregivers,
and congregational health programs. Funding will enable us to serve more people through community outreach efforts. We
are grateful for your consideration.

Duet promotes health and well-being through vitally needed community-based services to homebound adults, adult
caregivers, faith communities, and grandfamilies.
Narrative
Proposed program or project: Duet requests funding for increased outreach efforts. Duet is vigorously working to provide
culturally competent outreach in response to the disproportionate growth in the population we serve.
Methodology -
• Hire of an Outreach Coordinator in 2017 to build awareness.
• Outreach to more Hispanic older adults and family caregivers.
• Presentations on our services for local congregations, fairs, clubs, coffees.
• Acceleration of the start-up of congregational health programs through robust outreach to nurses and congregations.
Sixty-eight congregations of our 140 partner congregations currently have active health programs.
• Engagement of volunteer “ambassadors” who carry out community outreach on Duet’s behalf.
• Increased media releases, paid radio ads, Facebook Likes, Twitter Followers, Website Hits, newsletter recipients.
• Modernized field-outreach technology.
• Expanded utilization/placement of existing video series, traveling exhibition, and informational flashdrives.
• Signage outside our brand new facilities location to aid significantly in visibility.
• Training for volunteers, ambassadors, nurses.
• Increased grassroots techniques such as word of mouth.

• Increased visibility through vehicle signage and Tshirts.
New or continuing project: Expanded project. Increased outreach is an integral component of Duet’s long range plan 2015-20,
with a mandate to increase service across programs by 5-15% annually.
Purpose of the project: Increased outreach assures that those in need know of Duet’s services –
- Volunteer services for homebound adults perform essential needs such as transport to food and medical care, and
relieve isolation and depression common among homebound elders – healthcare experts are declaring loneliness to
be a public health epidemic.
- Courses that provide registered nurses the training to serve as faith community nurses.
- Support and guidance for grandparents raising grandparents, to reduce harmful stress.
- Resources for caregivers caring for aging loved ones, whose own health is put at risk due to the strain of caregiving.
Project meets one or more of Fort McDowell’s funding priorities: Health.
Need that the program or project fulfills: Growth projections as a result of the wave of baby-boom generation retirement -
research shows that those born between 1946 and 1964 are the largest elderly population in human history. Duet’s strategic,
managed growth in outreach responds to this data. In Arizona persons 65 years and over represent 15.4% of the population.
In Maricopa County, where Duet conducts its services, more than 650,000 adults are over age 60 with the fastest-growing
segment being over age 80. Outreach is essential so that more families know that they may receive help through Duet.
Outreach leads to more personalized care and guidance. A grant makes it possible for Duet to achieve growth in the total
number of people served. Our newly reinforced outreach has already proven to work – we seek to build on the momentum.
Funding for critical outreach activities is difficult to find – Ft. McDowell Yavapai Nation funding would fill an important gap.
Population targeted: Homebound elders, compassionate volunteers, grandparents raising grandchildren, family caregivers,
registered nurses. Currently utilizing Duet’s services - Age groups: under 60 7%,  60-70 30%,  71-80 22%, 81-90 20%,  91-
95 20%, Over 95 1%. Gender: Female 80%, Male 20%. Ethnicity: African American 7%,  Asian/Pacific Islander 1%,
Caucasian 72%, Hispanic 16%,  Native American 1%, Other 3%. Income: At or below federal poverty guidelines +20%. There
are many more seniors needing our services than know about Duet.
Estimated number of people who will be served by the grant: Duet served 30,000 people in 2017. Your grant will help to
extend that by 5%, or an additional 300 people.
Partners in the program or project: The very name “Duet: Partners In Health & Aging” demonstrates our commitment to
collaboration. Other partners include KJZZ and other media outlets providing reduced and in-kind publicity, and program
partners including Foundation for Senior Living; the Area Agency on Aging, Region One; ASU; Muhammad Ali Parkinson
Center; Adult Protective Services; the Alzheimer’s Association; Jewish Family and Children’s Services; Benevilla, and many
others.
Other entities providing funding and support: In 2017 Duet was the recipient of a prestigious Nina Mason
Pulliam Charitable Trust grant in the amount of $110,000 over the period of two years, to increase direct services and
outreach to grandparents raising grandchildren. The Virginia G. Piper Charitable Trust funded a proposal from Duet in
2016 for $460,000 over three years to enhance and expand Duet’s caregiver services. Cook Native American Ministries
provided funding in 2018 for outreach to Congregational Health. The Kemper & Ethel Marley Foundation generously funded
Duet in 2017 in the amount of $75,000, a portion of which is for outreach.
Whether the organization will be submitting applications to any other tribes: Duet has received $7,500 from Salt River
Maricopa Pima Indian Community in general support of all of our programs for the past two years, and has been notified of a
$10,000 award for our general services this year. The funding is bolstering direct services to family caregivers, grandparents
and the children they are raising, and homebound adults. We received a first-time grant in 2016 from the Tohono O’odham

Nation in support of our services to grandparents and the grandchildren they are raising. We did not receive a grant in
response to our application in 2017, and we will reapply to them again this year for grandfamily services. From Gila River
Indian Community we have received $35,000 each year for the past three years for volunteer transportation for homebound
adults. In 2013 Duet received $43,000 from Gila River Indian Community. We currently have a proposal submitted to Gila
River Indian Community for a $70,000 grant over two years for volunteer transportation for homebound adults.
Timetable for implementation of the program or project:
Duet’s services are consistent throughout the year. Implementation of increased outreach can take place as soon as the
funding is in place.
