Attn: Prop 201 Grant Application
Dear Ms. Enos,
On behalf of Phoenix Children’s Hospital, I want to extend heartfelt appreciation for our partnership with the Fort McDowell
Yavapai Nation. We are grateful for the opportunity to present an application for a 12% Tribal Gaming Grant request below.
Project Description. Phoenix Children's is committed to providing children with the best treatment under a family-centered
model of care. This comprehensive approach to medical care calls clinicians and staff at the Hospital to address a number of
needs beyond a child’s medical treatment. While most hospitals focus only on the patient’s medical care, the family-centered
philosophy here considers family as integral members of care teams. As the primary caregivers in a child's life, they are best
equipped to act as advocates. Parents and siblings, too, are deeply affected by a child's change in health, and adjustments
must be made to the entire family's lives. The holistic approach to healing and recovery involves not only treating the medical
needs of a child, but addressing psychosocial needs of both the patient and family for optimum healing.
Because the Hospital can be a stressful place for children as well as families, efforts are made to make the experience a little
easier by providing some essential comfort items to families during extended periods away from home. In 2012 the Family
Care Cart was introduced, and has been an essential component of family-centered care ever since. This highly anticipated
cart circulates throughout the Hospital twice each day, thanks to cheerful Hospital volunteers. Family members are offered
hygiene products and toiletries, hair products and accessories, blankets, socks and activity kits. Parents and families,
sometimes finding themselves unexpectedly in the Hospital with their children, are grateful for the Family Care Cart. The
items delivered to their room ensure caregivers can remain at
the bedside during their child’s healing journey for as long as
possible.
Phoenix Children’s Hospital respectfully requests a grant award
of $25,792.00 to name the Fort McDowell Yavapai Nation
Family Care Cart. Funds will be utilized to stock the Family Care
Cart for an entire year, providing families with these comforting
essentials during long hospital stays.
Purpose of the program or project and how it meets Fort
McDowell’s funding priorities. The impact of parental and family
support during a child’s journey of healing cannot be
understated. Children suffering from illness and injury
experience better outcomes when supported by caregivers and family members. The vision for family-centered care is to
ensure that all families are in the best position to focus on healing and recovery by relieving some of the small stressors that
may impact them during their stay. The Family Care Cart at Phoenix Children's closely aligns with Fort McDowell Yavapai
Nation’s focus on health service programs. We too believe in the “Never give up, and always give back” motto of Fort
McDowell Yavapai Nation, as that spirit is woven throughout the family-centered care services we provide all families in their
healing journey. Because Phoenix Children's will never turn a child away based on ability to pay, we must continue to provide
wrap-around services, to ensure all families have access to the best pediatric care in Arizona. The Hospital’s family-centered
care model provides safety nets for families and eases some of their burdens, so they are in the best position to support their
children and focus on recovery.
The need that the program or project aims to fulfill, the population targeted, and the estimated number of people who will
be served by the grant. Of the 148,000 children who will be treated at Phoenix Children's Hospital annually, about 33 percent

(nearly 49,000) will have a serious illness or injury that requires ongoing Hospital stays, surgeries and medical appointments.
As the complexities of our patient base continue to grow, the need for family-centered care services also increases. Children
and families often come to the Hospital during the most vulnerable, stressful and fearful times in their lives. In addressing a
child’s medical needs, the family’s immediate basic needs must also be met. Time and again, care teams have witnessed that
providing a hygiene kit to a parent or an activity kit to a sibling can revitalize spirits and ease stress and anxiety. The circulation
of the Family Care Cart is a small part of the ancillary services Phoenix Children's delivers in alignment with our mission to
provide hope, healing and the best health care. These services, combined with top-notch medical treatment collectively
impact the patient and family experience, which directly correlates with better outcomes for our patients. More than 3,500
families will welcome The Fort McDowell Yavapai Nation Family Care Cart during their Hospital stay this year. The Fort
McDowell Yavapai Nation Family Care Cart will transport more than 5,000 items during its twice daily circulations of the
Hospital, serving as a constant reminder of your commitment to the healing and recovery of Arizona’s children. The Hospital
will continue to operate under a comprehensive care model because it has proven successful. Family-centered care has
evolved as the needs of our patients and families have increased. This constant commitment to solution-based strategies will
continue to help the children and families served here during their greatest time of need.
The partners in the program or project, and other entities providing funding and support (please indicate whether the
organization will be submitting applications to any other tribes). Most of the Family-Centered Care staff and operations,
including the Family Care Cart, are completely funded through philanthropy. At this time the Family Care Cart is stocked
through various corporate product drives and donations. The Volunteer Services department at the Hospital ensures there are
volunteers to manage the cart and circulate throughout the Hospital each day. Phoenix Children's Hospital Foundation intends
to continue to seek donations from organizations and individual donors expressing interest in patient and family services.
Since 2011, Phoenix Children’s Hospital has partnered with the Salt River Pima Maricopa Indian Community, the Tohono
O’odham Nation, the Gila River Indian Community and the Fort McDowell Yavapai Nation. This year, the Hospital anticipates
submissions to Tohono O’odham Nation, Gila River Indian Community and Salt River Pima Maricopa Indian Community. The
planned applications to the other tribes include requests for programs and projects unrelated to the Family Care Cart.
The timetable for implementation of the program or project. By the end of the first quarter 2019, The Fort McDowell Yavapai
Nation Family Care Cart will be identified with color signage on the cart. Products for the cart will be purchased and stocked as
needed throughout the year. It is anticipated that items will be distributed to an average of 14 families per day. Interim and
final reports will indicated service numbers for both number of families served and number of items distributed.
Please join us in continuing to serve children and families with the most essential and basic needs. Family-centered care
services are critical to ensuring children and families successfully navigate the path to recovery.
Thank you for your consideration. Should you have any questions regarding this proposal, please contact me.
Sincerely,
Steve Schnall
