The proposed program or project, and whether it is a new or continuing project: This is a new project, but it is based upon a previous experience of the project manager’s. The Glendale Office of Disability Resources and Services (DRS) will hold the first annual, Glendale "Game of Life" 2019 Disability Resource Services (DRS) Transition Fair. Two students who attend will be eligible to win four-hundred-dollar scholarships to use for tuition and books. Vendors, employers, and other associated services will be there for students to meet and ask questions. Students will participate in a game, similar to the board game The Game of Life, to give them a sense of the challenges and rewards they will find on their pathways to independence. 2. The purpose and how it meets one or more of Fort McDowell’s funding priorities: The purpose of the Fair is to address directly the unique challenges families of teenagers who have disabilities will encounter when transitioning out of high school. These challenges involve health status and establishing the necessary supports to live independently and to enter higher education and/or the workforce successfully. This event aligns with three Fort McDowell funding priorities: health, community development, and education. 3. The need or aim, the population targeted, and the estimated number of people served: All parents want their children to be as prepared as possible before launching into the world. Parents of children with disabilities face even greater challenges. The Fair will fulfill the needs families have to prepare for their children’s transition towards levels of independence, higher education, and or the workforce. The Fair will raise awareness of many unforeseen challenges and connect families to appropriate support services. Following the Fair, providers of those services will follow up with families and guardians, as appropriate. The anticipated outcome is that approximately 200 teen-aged high school students who have disabilities and their families will gain wider exposure to supports that will increase their success in achieving their personal future goals. The Project Manager of the first annual Glendale "Game of Life" 2019 Disability Resource Services (DRS) Transition Fair is Dr. Amy Rabideau. Dr. Rabideau has great experience in bringing these types of fairs to the community and is highly experienced in Life Transitions for the disabled. She recently received the 2018 Northbridge Visionary Partner Award for her years of student-centered services helping individuals with disabilities succeed in college and life. Dr. Rabideau manages the Glendale Office of Disability Resources and Services (DRS) and is uniquely qualified to design and produce successful community support events. DRS and Glendale Community College (donating the venue space) have a long and successful history collaborating with local school districts and community partners. Similar annual events include the Health and Wellness Fairs, the Mental Health Expo, Stand Up for Veterans, Career Fairs, The Native American Parent Conference, and United Way events. DRS anticipates this first annual half-day event to serve approximately 350 people (200 disabled teenagers plus their parents/guardians), and is requesting grant support in the amount of $9,350 from the Fort McDowell Yavapai Nation 2018 Arizona Benefits Fund. 4.
