1. Proposed Program
Sun Health established its Memory Care Navigator program to provide a roadmap on how best to
live life with dementia in a way that honors the patient as well as supports the caregiver. Our
goal is to encourage and support West Valley families caring for family members with memory
loss and offer professional guidance, training and the resources necessary to provide safe and
compassionate care year-round. Since this program started we have assisted more than 1,000
families across the West Valley. Now in its fifth year, Sun Health is committed to responding to
the demand for this service and therefore added a second navigator to expand its service
capacity.
Our Memory Care Navigator program is the point of entry for all Sun Health memory services,
which are all provided at no cost to the community. This comprehensive approach is a way to
provide caregivers and families with the tools and knowledge to care for their loved one at home
as long as possible. It is an effective approach to addressing a significant and persistent gap in
services and is based on best practices in geriatric care and includes:
 Personalized Support: one-on-one guidance from a trusted advisor for caregivers resulting in
individual care plans for families
 Educational Classes: classes including Brain Fitness, Meditation and Brain Health,
Supportive Communication, Managing Caregiver Stress and more
 Caregiver Support Groups: a safe place to discuss experiences, share ideas and receive
emotional support for caregivers
 Community Connection: links clients and family members to services, resources, financial
assistance, education, support groups, and clinical trials in the West Valley
 Medication and Emergency Management Tools: Sun Health Medication Profile keeps track
of medication schedules and Sun Health Vial of Life / File of Life provides quick access to
emergency medical information
Our Memory Care Navigator was established in 2013 and it has remained as a continuing
program.
Sun Health champions healthy living, research and superior health care.

2. Purpose
We believe our program meets Fort McDowell’s funding priorities in the areas of health and
education. Our Memory Care Navigator works directly with families to map out individualized
care plans, identifies supportive services in the community and links families to existing Sun
Health Community Wellness programs such as memory care education, support groups and
medication management tools. The program empowers families by providing the resources and
training necessary to provide loved ones with Alzheimer’s and other forms of dementia, with
safe and compassionate care year-round.
3. Need, Target Population, Estimated Impact
Nearly 1 in 9 seniors 65 and older live with Alzheimer’s disease, leaving families in the West
Valley cities where the median age is 70+, searching for help. Families often find themselves at a
loss as to how to best care and provide a safe environment for a loved one with dementia.
Memory and language loss can complicate relationships. Changes in personality and behavior
make it difficult to interact. Routine activities, such as cooking a meal, become a struggle.
Driving and wandering issues can pose serious safety concerns. We find most are earnest in their
desire to provide care, but simply do not have the skills, emotional bandwidth, time, resources or
caregiver knowledge. Our Memory Care Navigator program is provided at no cost thanks to the
support of grants and community donations, however these services would otherwise not be
available as they are not provided by insurance companies or health providers.
Sun Health is the primary non-profit agency providing memory care services in the West Valley
where we target residents with memory loss issues (ages 55+), and their families in need of our
program services. In fiscal year 2016-2017, our navigator met one-on-one with 230 caregivers,
facilitated support groups for 284 families and welcomed 760 area residents to our education
classes. Specifically, a $25,000 contribution from Fort McDowell Yavapai Nation would support
our Memory Care Navigator program expansion and serve an additional 120 caregivers, offer
Dementia Caregiver Support Group twice per month for 200 caregivers, and have 20 classes
regarding memory issues and brain health for approximately 1,000 area residents.
4. Program Partners
It is imperative for Sun Health to have partnerships with local nonprofit agencies in the West
Valley to help promote the accessibility of our Memory Care Navigator free service.
Organizations such as Benevilla and Northwest Valley Connect are important partners.
Additionally, medical professionals, medical facilities and physician’s offices are also referral
sources. The program is funded through individual donations, special event fundraising, and
grants such as the AZ Republic Season for Sharing. Other funders approached for support
include USAA, Sundt, Phoenix Suns Charities, and John F. Long Foundation. For this specific
program, we will not be approaching any other tribe, however, we have submitted an application
to Gila River Indian Community to support our Care Transitions program.
5. Implementation Timetable
Sun Health’s Memory Care Navigator program is available year-round, however, fiscal budget
runs from July 1 through June 30. No percentage of Fort McDowell Yavapai Nation’s grant
proposed for the program’s 2018-2019 budget would be allocated to overhead resulting in 100%
of the funds going directly to the program.

No overhead expenses are included in the Sun Health Memory Care Navigator budget as administrative
services are leveraged through Sun Health fee based programs. We started with one memory care navigator in
2013 and in July 2017, we added a second navigator in response to the demand in the community. Our hope is
that with the continued support of the philanthropic community, we will be able to double the number of
people we help going forward. Every donation, regardless of the size, is imperative as we work tirelessly to pull
together numerous donations from individuals, foundation grants, and special events to meet funding
requirements and keep our program available at no cost, to all struggling to care for loved ones with dementia.
