CONTRACT NO: C-86-15-086-3-01 AMENDMENT NO. 1 AMENDMENT TO RFP CONTRACT 860-15-086 Between MARICOPA COUNTY By and Through its DEPARTMENT OF PUBLIC HEALTH and BIO-REFERENCE LABORATORIES I. The above named contract is hereby amended as specified below: A. Cover Page, under 1.0 Contract Term, 1.2., the County extends the contract on a month-to-month basis for a maximum of three (3) months after expiration, beginning on April 8, 2018, and ending July 7, 2018. County may extend the Contract for an additional 3 months. B. The Contract extension amount is not to exceed $16,600 per month for each of the 3 months for a not to exceed total of $49,800. C. 6.10 Unconditional Termination for Convenience: County may terminate the Contract by providing 30 calendar day advance notice to Contractor. All other terms and conditions of the original contract and other amendmprits will remain in full force and effect.
