This project will also 21. Geographic area served serve the surrounding communities within the automatic/mutual aid system throughout Maricopa and Yavapai County. Gila River Indian Community Grant Application Grant Cycle 2018 Narrative Please structure your proposal to provide the following information in the order indicated. Provide the narrative in paragraph form in the text field provided. Please be thorough but strive for brevity. 1. Briefly describe the organization’s history, mission and goals. Daisy Mountain Fire & Medical (DMFM) was created in 1988 to provide fire and medical services to the growing Desert Hills and New River communities. In 2004 we annexed the new master planned Anthem community into our district and most recently in 2017 have consolidated with the Black Canyon City Fire District. Currently DMFM is a 5 station fire department that serves approximately 55,000 citizens with multiple public education programs that benefit the communities we serve. Our mission is to be “ Honorable, Compassionate and Exceptional” with goals of “Seeking innovative ways to protect the community we serve”. With these goals as a growing organization, we strive to being able to improve and increase the services we provide while still being fiscally responsible of the taxpayer’s dollar. 2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona municipalities including past and present support, whether through partnerships, collaborations, in-kind contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If the applying organization is a municipality governmental sub-division please enter the text “Not applicable.” Daisy Mountain Fire & Medical (DMFM) is a special district tax-based entity that is funded through Maricopa County property taxes. Maricopa County is our pass through entity for grants funded by proposition 202. DMFM contributes our emergency services to multiple Maricopa County Regional Parks, State Forestry/BLM recreational use areas and mutual aid partner communities. We have the support from our neighboring municipalities for this project such as Phoenix Fire, Scottsdale Fire and Peoria Fire-Medical (see attachements). The support from these departments comes from decades of their own Technical Rescue Teams (TRT) responding to the variety of technical rescues within our district. DMFM TRT will be a valuable resource for all 26 departments in the Phoenix Automatic Aid Consortium (PAAC) in Maricopa County region to assist in technical rescues. With these partnerships, our Technical Rescue Personel will be trained in the Phoenix Fire Departments 200 hour TRT course. 3. Describe the proposed project and planned goals and activities. Gila River Indian Community Grant Application Grant Cycle 2018 The proposed project will directly benefit the Daisy Mountain Fire & Medical (DMFM) citizens, automatic/mutual aid recipients and Maricopa County recreational enthusiasts. The first objective is to send 15 DMFD members through special operiations training at Phoenix Fire Departments 5 week, 200 hour Technical Rescue course that focuses on specialized training including trench rescues, swift water rescues, technical high angle rope rescues, confined space, industrial emergencies, structural collapse and mountain/trail rescues. The second objective for this project will be purchasing the necessary individual load out gear (see attachment) and the required technical rescue equipment (see attachment). The last objective of this project is for DMFM to build a $60,000 support vehicle (as an in-kind expense, see project budget) needed for this Technical Rescue Team. The planned goals/activities for the first years allocated grant money will be utilized to train 3 DMFM captains as subject matter experts for establishing the team and purchasing the necessary equipment. The second years allocated grant funds will be utilized for training 12 additional DMFM members required to constant staff the Technical Rescue Team. Once the DMFM members are trained, specialized equipment is received, and support vehicle is equipped, we will be able to add this vital resource into the PAAC 911 automatic/mutual aid system. We will attend the valleywide trainings with the PAAC Technical Rescue Teams to establish continuity with crews, continued education and real scenario based drills. 4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River Indian Community. Daisy Mountain Fire District is a committed fire department that provides fire and medical emergency services for the publics safety. The proposed project will enhance our efforts by combating the increasing technical rescues in our urban interface communities. 5. Identify the needs/problems to be addressed, target population and number of people to be served by the project. Our fire district is now approximately 200 square miles of growing coverage area located in northern Maricopa County, with a majority of our 55,000 residences living in rural areas encompassing the communities of New River, Desert Hills, Anthem and Black Canyon City. Due to the rural setting and increasing population, we have seen a rise in flood water and mountain rescues. Currently our members have minimal training to support life safety rescues in slow moving Gila River Indian Community Grant Application Grant Cycle 2018 flood water conditions and no certified training in mountain rescues. This has put our members in extremely dangerous situations with near misses that could have been avoided. With these funds we will be able to properly train and equip DMFM members to avoid these scenarios. Our district responds to a variety of high traffic recreational areas including; Maricopa County Regional Parks, Tonto National Forest, Bureau of Land Management and Arizona State Land. The Technical Rescue Team will not only improve firefighter/public life safety in our fire district, but other heavily populated surrounding communities as well. Implementing this resource in our fire district will reduce the response times for these emergencies. Currently we have extended response times for technical rescues, due to the locations of the nearest Technical Rescue Teams. Our recent consolidation with Black Canyon City Fire, we have inherited a 35 mile stretch of the I-17 corridor through remote mountainous terrain. With this expansion, the Technical Rescue Team is now a critical resource for DMFM. 6. Define the project as a new or continuing program. This project is new to Daisy Mountain Fire & Medical. DMFM has been striving for the last 10+ years to establish a Technical Rescue Team without success due to the districts budget being severely constrained by Arizona’s property tax limiting legislation, Prop 117. We have also seeked out federal Homeland Defense Grants to assist in funding this project without success due to the grant funding guidline limitations. With these limitations and financial hardships we are seeking other funding sources to support this need. 7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible. Once we are awarded the first year funding from this grant in October 2018, our timeline will be to train the first 3 DMFM captains in the Spring of 2019 TRT course. Following their completion of the course we will order the necessary equipment (see attachment). When the second year funding is received in October 2019, we will enroll the next 12 DMFM members in the Spring of 2020 TRT course. During this time DMFM will be establishing a support vehicle. Following their completion of the course we will be able to add this resource into the regional 911 automatic/mutual aid system. The flexibility of this projects timeline is limited due to the life safety impact to the public/members and the limitations of the 5 week, 200 hour TRT course availability. We attempted to extend this project into a 3 year funded grant. Due to the specialized training and continued education needed, it was determined that extending this program over 3 years would jeopardize the Gila River Indian Community Grant Application Grant Cycle 2018 retention of the specialized training needed to perform these tasks. The finish dates for this project will be summer of 2020. 8. Identify other organizations, partners or funders participating in the project and their roles. Daisy Mountain Fire & Medical is a funding partner for this project by contributing a total of $92,142.84. This is a combination of the 10% match or $32,142.84 and the in kind contribution of $60,000.00. Salt River Pima-Maricopa Indian Communities partially funded DMFM UTV project (see line 12), totaling $28,312.56. 9. Would you be able to implement the proposed plan if your organization received partial funding for this project? The Daisy Mountain Fire & Medical will not be able to fund this project beyond our 10% match and in kind contribution of $60,000 equaling $92,142.84. Due to the backfill expense for the 15 DMFM members to attend the 5 week training course and initial equipment costs, DMFM will not be able to fund the start up costs needed for this vital TRT resource. 10. Describe your plan for project financial sustainability beyond the grant period. If this is a program/project previously funded by the Gila River Indian Community describe efforts made towards the previously described sustainability plan. Daisy Mountain Fire & Medical will be able to budget the equipment, training and apparatus upkeep that is required to sustain this project once implemented. There is a standard for continued education in the valley with multiagency weekly TRT training that we will be participents in. We have researched other fire departments TRT sustainment budgets and forecasted our ability to sustain this project. 11. Describe your plan to document progress and results. Our Plan is to write multiple follow up reports on the progress of this project. In the first years funding, there will be a period of time before the first 3 DMFM captains will be trained and equipment delivered. During this time we will send an interim report to follow up on the status of the project. Once the second year funding is allocated, we will send another interim report to announce our accomplishments, the plan for the remaining 12 DMFM members to be trained and the status of the Gila River Indian Community Grant Application Grant Cycle 2018 support apparatus. When the final group has completed there training, we will conclude our final report for 2020. 12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state shared revenues for this and any other project for the past five (5) years. If this information is included in a separate attachment, please indicate that here. Daisy Mountain Fire & Medical has most recently received 90% partial funds of $25,738.69 to purchase a Polaris 6x6 with a Fire/Rescue skid unit and enclosed trailer from the Salt River Pima- Maricopa Indian Community from their 2016 grant process. This apparatus was chosen for its wildland capabilities, special public event medical response, and ability to access remote locations for medical emergencies, which is a valuable piece of equipment to support our future technical rescue team.
