Denial Gila River Indian Community Grant Application Grant Cycle 2018 Narrative Please structure your proposal to provide the following information in the order indicated. Provide the narrative in paragraph form in the text field provided. Please be thorough but strive for brevity. 1. Briefly describe the organization’s history, mission and goals. First Place® AZ is a nonprofit charitable organization established in 2012 with the mission to develop communities that integrate adults with autism and other special abilities into the fabric of society through housing, healthcare, education, employment, supportive systems and culture. Our goal is to ensure that housing and community options are as bountiful for individuals with autism and other disabilities as they are for everyone else. Led by market principles, First Place is creating replicable residential and post-secondary education models that promote collaboration among the private, public, nonprofit and philanthropic sectors to serve as catalysts for advancing public policy focused on housing solutions for special populations. Such advances support local economic development and create opportunities for long-term employment and independent/interdependent living for members of this population. Our work captured the attention of PBS NewsHour through two nationally broadcast segments, featuring Greater Phoenix as “the world’s most autism-friendly city.” First Place AZ is leveraging the investment of 20-plus years in community development by the Southwest Autism Research & Resource Center (SARRC), our sister nonprofit, by addressing the needs of an ever-increasing population of children with autism transitioning to adulthood in record numbers—approximately 60,000 per year in the U.S. Set in the heart of the urban region, our first property, First Place–Phoenix, broke ground in December 2016 and is set for completion early this summer. This unique development offers three primary components: 1) The First Place Apartments provide access to various services and amenities—with all the benefits of community- connected, independent living—by a broad range of differently abled adults; 2) The First Place Transition Academy offers courses and life skills-based training, including career development, to help residents find employment and live more independently; and 3) The First Place Global Leadership Institute serves as an international training center for professionals, direct support service providers and medical personnel, and a robust site for research and public-policy advancements. Gila River Indian Community Grant Application Grant Cycle 2018 First Place underscores safety as the top priority for all residents and staff, as well as the family members and friends who come and go, as evidenced by our guiding principles: • Home: Ensure the property provides the comforts, security and safety of home for all residents. • Stepping Stones: Ease the transition for adults and their families through continuous communication, involvement and support. • Person-Centered Culture: Celebrate individuality and neuro-diversity supported by innovations in design and technology that promote safety, security, choice and greater independence for residents. • Inclusivity: Support diversity within the property and outside in the community at large by connecting residents to opportunities and to people with shared and complementary interests. The Global Leadership Institute also focuses on multidisciplinary study and research for educators and medical professionals, enabling the creation of more residential options that support greater independence. The Institute is focused on establishing and implementing promising best practices for this population, including security and safety at First Place–Phoenix and beyond. 2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona municipalities including past and present support, whether through partnerships, collaborations, in-kind contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If the applying organization is a municipality governmental sub-division please enter the text “Not applicable.” First Place has received the following government funding: Maricopa County Industrial Development Authority (IDA): $335,000 community investment grant for the First Place Leadership Institute’s Center for Training, Education and Employment Phoenix IDA: $250,000 community investment grant for the First Place Transition Academy expansion 3. Describe the proposed project and planned goals and activities. First Place–Phoenix will be a first “home away from home” for many adults with autism and other special needs. These individuals face limited-to-nonexistent options for more independent living in their communities without access to jobs, people, places and activities. Their parents and families have been their lifelong lifelines of constant support. Results from various focus groups conducted with current and potential Gila River Indian Community Grant Application Grant Cycle 2018 residents with autism and their families indicate that safety and security are the number-one concerns for members of this population. For many people with autism and other special abilities, associated core deficits in key areas linked with greater independence—executive function, communication and social connectedness—are a challenge and often an obstacle to independent living. Safety and security promote confidence and self-sufficiency, critical components of independent living. First Place is seeking a grant from the Gila River Indian Community to fund security and safety equipment for the First Place–Phoenix property. This includes interior and exterior security cameras with accompanying equipment (video recorder, monitors, keyboards), plus cabling; and iGuardStove automatic shut-off devices for every stove/oven on the property—including the First Place common-area teaching kitchen—as well as remote monitoring equipment. Major Goals: • Providing a secure and safe residential and learning environment that paves the way to the acquisition of life skills and more independent living for these adults and peace of mind for their families • Offering a comprehensive and effective program for anticipating and addressing possible security and fire- safety issues • Serving as a respected model primed for research and evaluation • Sharing of best practices and expansion throughout Arizona and across North America The role of the First Place Global Leadership Institute will also be integral to the creation of new and the fine- tuning of existing programs that focus on healthier, safer lifestyles—including behavioral health—to ensure individual growth, independence, employment and long-term success. This includes direct involvement in life- skills training and education for residents and the training and monitoring of direct support service providers. 4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River Indian Community. The First Place®–Phoenix Residential Security and Safety Program satisfies the “Public Safety” priority funding area identified by the Gila River Indian Community. Gila River Indian Community Grant Application Grant Cycle 2018 Products, services and programs that reduce risk and improve outcomes in the areas of security and safety are critical to the robust community life that First Place–Phoenix will provide for its residents. This project will not only allow First Place to establish sound preventive security and safety measures, but it will connect residents to the broader community, as well as help to build positive relationships with first responders—police and fire— who are so critical to maintaining public safety. It is also the goal of First Place to help educate first responders about the unique behaviors and needs of adults with autism and other special abilities—a population that is too often misunderstood—in order to foster lasting, trusting relationships that enhance overall safety and security. 5. Identify the needs/problems to be addressed, target population and number of people to be served by the project. Once the decision is made to take that daunting next step to First Place or another place away from the family home, parents themselves face a whole new set of challenges: how to ensure their adult children will thrive in a safe, secure and supportive environment—and how their grown children will learn to practice their own personal safety within that environment. First Place recognizes the need for innovative solutions when considering current data and these lifelong challenges: • Autism is the largest disability group among adults in the U.S. • The incidence of autism today is 1 in 68 compared with 1 in 2,500 20 years ago. • The unemployment rate among adults with autism is 86%. • 60,000 children with austism are entering adulthood annually—a number that’s on the rise. • The lifetime incremental cost per individual in this population: $2.4 million Parents shoulder enormous and often overwhelming responsibilities as they guide their adult children with autism toward various goals and milestones, including the opportunity for that first home away from home. What kinds of assurances can we give those parents whose lives are dedicated to supporting their special needs children? With safety being a parent’s top priority for their children, First Place is actively addressing their concerns with the implementation of effective security and safety tools like those included in this grant request. According to the National Fire Protection Association, over 80 percent of deaths in the home are caused by fires. And the overwhelming majority of those fires are caused by unattended or forgotten items cooking in the kitchen. Requested grant dollars will make possible the purchase of a safety system that not only shuts off oven Gila River Indian Community Grant Application Grant Cycle 2018 and stove appliances when not in use, but also collects data enabling First Place staff to help educate and train residents to advance their life skills toward greater independent living. Features of iGuardFire’s devices include motion detectors, automatic shut-off timers, manual timers, panic buttons, audible and visual reminders so important for this population, nighttime activity tracking, smartphone notifications and remote monitoring, among others. Not only will these devices be installed in each apartment, but they will also be incorporated into the First Place teaching kitchen, a common area where residents gather as a group for cooking and safety instruction. The target population served by First Place is adults ages 18 and older with autism and other special abilities. First Place–Phoenix will serve approximately 80 residents, including First Place Transition Academy participants and First Place fellows, along with 50-plus First Place and SARRC staff members combined. Direct support service providers and other professionals (medical, educational) will number approximately 60. 6. Define the project as a new or continuing program. The First Place®–Phoenix Residential Security and Safety Program is a new program. 7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible. • Purchase of equipment: by March/April 2018 • Installation of equipment: by May/June 2018 • Testing of equipment: by June 2018 • Regular use and maintenance of equipment: starting July 2018 This is a specific timeline, in light of completion of construction and occupation of First Place–Phoenix scheduled for early summer 2018 and the importance of project completion by resident move-in. 8. Identify other organizations, partners or funders participating in the project and their roles. • iGuardFire, the company providing First Place with the equipment and training • Extreme Integration, the security camera company • First Place staff to train and monitor residents in the proper use of appliances • First Place maintenance staff to monitor effective operation of cameras to ensure 24/7 security Gila River Indian Community Grant Application Grant Cycle 2018 • Phoenix first responders who will be participating in the implementation of First Place–Phoenix’s ongoing safety plan • First Place staff and SARRC staff, as well as operators of the First Place Transition Academy, who will evaluate data and develop protocols for education and training 9. Would you be able to implement the proposed plan if your organization received partial funding for this project? Yes. With partial funding, First Place would move forward with implementing the Residential Security and Safety Program and continue seeking funding from other public, private, nonprofit and philanthropic sources to meet the goal of establishing a safe and secure property for residents, staff and visitors at First Place–Phoenix. 10. Describe your plan for project financial sustainability beyond the grant period. If this is a program/project previously funded by the Gila River Indian Community describe efforts made towards the previously described sustainability plan. Project sustainability beyond the grant period is accomplished in several ways. Once the initial purchase and installation of all equipment is completed in the first year of funding, the most important focus will be ongoing training and the monitoring of residential outcomes. This includes ensuring that equipment is regularly functional and that residents are honing their life skills through responsible operation of the appliances in their apartments. Regular, correct operation becomes a habit, leading to safer outcomes for all who reside and work at First Place. 11. Describe your plan to document progress and results. Evaluation of program progress and success will be measured through ongoing safety assessments conducted at First Place–Phoenix, including training of residents and staff in the correct operation of iGuardStove devices and maintenance staff in the correct operation of security cameras. This includes monitoring each individual’s level of functionality and ability in their apartments; the completion and practice of the First Place Safety Plan protocols; the facilitation of fire drills; the completion by each resident of a Resident Emergency Profile, or REP, listing current medications, medical history, emergency contacts, etc.; and the ongoing engagement of Phoenix first responders. Gila River Indian Community Grant Application Grant Cycle 2018 The safeguards inherent in the iGuardStove products go beyond simply automatically turning a stove/oven off or on. There is also the ability to capture data from each device from a central location for training purposes and the constant improvement of policies and procedures for ensuring safety at all times. First Place staff will be able to track stove/oven activity in each apartment, determining which residents may need more education and training in the safe operation of their appliances. This ability provides valuable “teaching moments” so that residents are able to make necessary adjustments in their learning patterns and apply them to more independent living. 12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state shared revenues for this and any other project for the past five (5) years. If this information is included in a separate attachment, please indicate that here. First Place–AZ received a 12% gaming grant from the Salt River Pima-Maricopa Indian Community in the amount of $120,000 in 2016 and $51,000 in 2017 to fund the purchase of appliances (refrigerators, stoves/ovens, microwaves, washers, dryers) for the First Place Apartments and for the First Place teaching kitchen. This investment will support hundreds of people with different abilities to build their life skills and provide opportunities for safer, greater independence.
