Provide the narrative in paragraph form in the text field provided. Please be thorough but strive for brevity. 1. Briefly describe the organization’s history, mission and goals. Founded in 1947, Gompers is an organization dedicated to empowering individuals with disabilities. Our programs include our K-12 special education school which provides students ages 5 to 22 with individualized academic, behavioral, therapeutic and vocational programs to maximize skills and increase independence. Our Day Training for Adults program is built around community integration and self-determination where individuals maximize their abilities and independence. Finally, our Employment Services program allows individuals opportunity to develop important skills to support their transition to community employment. 2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona municipalities including past and present support, whether through partnerships, collaborations, in-kind contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If the applying organization is a municipality governmental sub-division please enter the text “Not applicable.” Gompers is a contractor for services for individuals with disabilities with the Arizona Department of Developmental Disabilities and has been for over 50 years. For this grant, Maricopa County will work with Gompers should Gila River Indian Community choose Gompers for the 12 % revenue sharing grant. 3. Describe the proposed project and planned goals and activities. According to the Association of People Supporting Employment First, an organization focused on integrated employment for individuals with disabilities, supported employment and job coaching is cost-efficient with a $1.22 return on every $1 spent. In Arizona, 10.7% of the population are capable working-age people with disabilities using data from the 2015 American Community Survey (ACS). Yet, only 22% of those who can work, do. The community can support the inclusion of these individuals with disabilities with help from a job coach. One of the hurdles in helping individuals with disabilities gain employment Gompers found is the need for a job coach who will work one on one with the members as they're beginning their work in Gila River Indian Community Grant Application Grant Cycle 2018 the job force. This coach will help the individual work through any issues they're having as they begin their training and work at the company. Most significantly, this alleviates the business’ financial burden that comes with an employer hiring a person with a disability. With a position like this, Gompers will be able to use specialized knowledge of employment and disabilities to help the employee, thus making them a stronger employee to the businesses who may be skeptical. Individuals placed in Glendale businesses will help the economic grow. The job coach will do a variety of duties in the course of assisting someone both on and off the job site. These duties and phases of work include: gathering assessment data and assisting the person with a disability to develop a list of interests and potential skills, gathering employment information by doing job analyses at business sites in order to match a person with a position, providing one to one training on a job site, and providing job retention services and ongoing support to the employer and person with disability. The funds requested from GRIC will start the Individual Employment Program and will be providing the salary and benefits of the Job Coach for one year. Funding to continue the program once started can be provided by the Division of Developmental Disabilities with the Arizona Department of Economic Security. The overriding philosophy of the Gompers Individual Employment Program is the belief that every person with a disability in capable of working competitively in the community if the right kind of job and work environment can be found. Rather than trying to sculpt participants into becoming “perfect workers’ through extensive prevocational assessments and training, participants are offered help finding and keeping jobs that capitalize on their personal strengths and motivation. Thus, the primary goal of the Job Coach is not to change participants, but to find a natural “fit” between participants' strengths and experiences and jobs in the west valley community. 4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River Indian Community. This program will help individuals with disabilities gain employment, thus helping the communites in West Phoenix, Glendale and Peoria in the area of Economic Development. Gila River Indian Community Grant Application Grant Cycle 2018 5. Identify the needs/problems to be addressed, target population and number of people to be served by the project. Did you know that there is an unemployment rate of 78% for people with developmental disabilities? These are individuals of the age and ability to work. There are several reasons why this statistic is so high. 1. Individual education – Individuals and their support system don’t understand the financial and health benefits of work. 2. Employer education – there are concerns about employment procedures for individuals with limited abilities, like how do I train and manage them, how do I address issues that arise, and financially how can I afford to hire these individuals. 3. It takes a specialized program to improve the employment opportunities for individuals with disabilities. The Job Coach will be based in an office at our Glendale campus at 53rd/Bethany Home and working with at least 10 individuals with disabilities and businesses in the West Valley. 6. Define the project as a new or continuing program. The component of individual employment is new for Gompers, though with knowledgeable supervisors and a skilled support team, Gompers Employment Services (ES) program excels at offering members opportunities to explore interests and gain experience with a goal of independent, competitive employment. The ES program offers: Center, Group, and Community-Based Employment programs, Disability Related Employment Services (DRES), Ticket to Work, Transition to Employment (TTE) and Vocational Rehabilitation (RSA). 7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible. The Individual Employment Program will beginning once funding for the job coach is secured and is on target to begin in 2019. The typical timeline for an individual consists of phases and is defined by each individual's needs and supports they desire. The ideal situation in best practices is to gradually phase out the Job Coaching as the individual progresses and feels confident and comfortable in their new role in employment. This time period can vary from 3-4 months to one-year to indefinite for some people based on their challenges or limitations due to their disabilities. The phases the individual undergoes and the intensity of the support provided Gila River Indian Community Grant Application Grant Cycle 2018 depends on his or her situation. Not everyone requires each phase of the process. If it is clear what type of job is wanted and that such a job is available, the early phases can be skipped and the focus can be on training and placement. 8. Identify other organizations, partners or funders participating in the project and their roles. Gompers works with Division of Development Disabilities with the State of Arizona to provide the best quality service for individuals with disabilities. Gompers is proud to partner with HBI International, Healthy Solutions Inc, Kevinsoffroad.com, Laura’s Gourmet Granola, Sprouts Food Stores, Mayesh Florists, among others for business solutions services as they support Gompers by hiring the group-based employment crews. 9. Would you be able to implement the proposed plan if your organization received partial funding for this project? Yes, Gompers is looking for many funding partners for this pilot program and the combined support will allow Gompers the ability to hire a job coach. 10. Describe your plan for project financial sustainability beyond the grant period. If this is a program/project previously funded by the Gila River Indian Community describe efforts made towards the previously described sustainability plan. As stated above, the funds requested from GRIC will start the Individual Employment Program and will be providing the salary and benefits of the Job Coach for one year. Funding to continue the program once started can be provided by the Division of Developmental Disabilities (DDD) with the Arizona Department of Economic Security. DDD has specific deliverables to be achieved during the pilot project period that must be reached before funding will begin. 11. Describe your plan to document progress and results. Gompers Individual Employment Program will survey and interview the members served every 3 months with a 10-part questionnaire, utilizing the data to determine such outcomes as customer overall satisfaction, delivery of services, quality of services and value. The goal is not only to place and support individuals in gainful employment in the community but also stay within their set goals, skill-set, career choice and sustainability of the job. Gila River Indian Community Grant Application Grant Cycle 2018 Focus Groups will enable Gompers to assist individuals in their determination of what type of employment they desire and possess the skill-set, education and motivation to be successful. This process will includes using the O*NET Resource Center assessment tool. The O*NET® is a set of self-directed career exploration/assessment tools to help workers consider/plan career options, preparation, and transitions more effectively. They also are designed for use by students who are exploring the school-to-work transition helping individuals identify their work-related interests, what they consider important on the job, and their abilities in order to explore those occupations that relate most closely to those attributes. Individuals can make a seamless transition from assessing their interests, work values, and abilities to matching their job skills with the requirements of occupations in their local labor market. 12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state shared revenues for this and any other project for the past five (5) years.
