Narrative
Please structure your proposal to provide the following information in the order indicated. Provide the narrative
in paragraph form in the text field provided. Please be thorough but strive for brevity.
1. Briefly describe the organization’s history, mission and goals.
HonorHealth was established in 2014 when Scottsdale Healthcare and John C. Lincoln Health Network, two
community-based nonprofit health systems, merged to form one comprehensive health care network. The
mission of HonorHealth is to improve the health and well-being of those we serve. The system is anchored by
five hospitals serving the greater Scottsdale and North Phoenix area and includes more than 70 primary and
specialty care locations, surgery centers, a cancer center, a research institute, a children’s emergency center,
a breast health and research center, Desert Mission community services, and HonorHealth Military Partnership
and Forensic Nurse Examiners Program
The Maricopa County Attorney’s Office contracts with HonorHealth’s Forensic Nurse Examiners Program to
provide medical forensic examinations for victims of sexual assault, domestic violence, and child and elder
abuse free of charge. Since its inception in 1998, the program has become a national leader in providing
medical forensic exams to victims of sexual assault and domestic violence strangulation. The program was
nominated for Magnet Exemplar Recognition by the American Nurses Credentialing Center during
HonorHealth’s 2016 site visit, one of the highest recognitions in nursing.
HonorHealth’s Forensic Nurses are certified sexual assault nurse examiners (SANE) and are highly trained to
provide comprehensive medical forensic exams to patients. HonorHealth’s Forensic Nurses are unbiased
medical professionals whose primary focus is compassionate care while conducting a thorough medical
forensic exam to identify and coument injuries and collect potential evidence samples. They are experienced
in caring for traumatized patients and identifying and documenting injuries that may not be apparent to health
care providers who have not received this specialized SANE training. The goal of HonorHealth’s Forensic
Nurse Examiners program is to respond to victims of sexual assault and domestic violence strangulation in one
hour, 24/7, anywhere in Maricopa County. The program also provides out-of-contract exams when needed,
including cases from Gila River Indian Community. Patients in need are never turned away.
2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with
Arizona municipalities including past and present support, whether through partnerships, collaborations,
in-kind contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in
this project. If the applying organization is a municipality governmental sub-division please enter the
text “Not applicable.”
The Maricopa County Attorney’s Office has contracted with the HonorHealth Forensic Nurse Examiners
Program to provide medical forensic examinations for sexual assault and domestic violence strangulation
countywide. The contract pays for the cost of exams, but does not pay for rent or equipment. The program is
co-located in five Family Advocacy Centers in Glendale, Goodyear, Phoenix, Scottsdale, and Mesa. Program
space, including exam rooms, is provided by these cities free of charge. Patients often arrive at the Family

Gila River Indian Community Grant Application
Grant Cycle 2018
Advocacy Centers in a traumatized state accompanied by municipal or tribal police. The program works hand-
in-hand with law enforcement at the intersection of both the healthcare and justice systems. However, best
practices for medical forensic exams specify that every effort be made to facilitate treatment and evidence
collection, but patients are not required to participate in the criminal justice system or cooperate with law
enforcement to receive an exam. While the environment at the Family Advocacy Centers is specifically
designed for optimal compassionate care, HonorHealth Forensic Nurse Examiners will also see patients at
hospitals, police stations, and other licensed care facilities in the community. HonorHealth has recently been
awarded a Victims of Crime Assistance (VOCA) grant from the Arizona Department of Public Safety to add a
full-time Victim Advocate position to the Forensic Nurse Examiners team. The Victim Advocate will provide
non-clinical care, linking patients with needed services in the community to promote their continued safety,
health, healing, and overall well-being. HonorHealth staff contribute and serve as chairpersons on committees
such as the Phoenix Police Department Sexual Assault Kit Workgroup, Maricopa County Attorney’s Office
Sexual Assault Protocol, and Maricopa Associations of Government’s Vulnerable Adult Protocol helping write
protocols, set standards of care, and improve overall outcomes for patients who are victims of crime.
HonorHealth values collaborative workgroup partnerships to best serve the community. The Forensic Nurse
Examiners Program works with both local, statewide, and national partners. In 2016 HonorHealth partnered
with Arizona Coalition to End Sexual and Domestic Violence (ACESDV) to offer a 40-hour training for Sexual
Assault Nurse Examiners. This effort was intended to expand the number of nurses in Arizona who are trained
to perform medical forensic examinations. The specialized nurses are in high demand as several areas of the
state are underserved and may not have a SANE (Sexual Assault Nurse Examiner) qualified nurse available at
all.
3. Describe the proposed project and planned goals and activities.
The goal of the HonorHealth Forensic Nurse Examiners Program is to provide a one-hour response time to
those experiencing sexual assault or domestic violence strangulation 24/7 anywhere in Maricopa County.
These patients will experience a continuum of care that begins with a medical forensic exam and continues
with follow-up advocacy services to help them to heal, remain safe, and attain/regain their physical health and
emotional well-being.
Forensic Nurse Examiners provide all patients with a head-to-toe examination. In a sensitive and caring
manner, they document injuries and collect samples potentially used as evidence. Patients identified as
needing further treatment are referred to medical, behavioral health, or substance services. All patients are
prescribed preventive medications, which are offered free of charge to those who cannot afford the cost of
treatment. If patients do not have a healthcare provider, they are offered care through HonorHealth’s affiliated
community health centers called NOAH. Unaccompanied patients and those identified as living in unsafe
situations, without food or shelter are connected to community resources.
As a medical forensic exam can take up to 90 minutes, the patient’s comfort is of paramount importance. For
this reason, we are requesting funding to purchase two new gynecological exam beds as well as replacement

Gila River Indian Community Grant Application
Grant Cycle 2018
covers for four existing exam beds. Also included in this request are funds for six new stand lights to replace
outdated gooseneck lamps which often become too hot, Littman stethoscopes and pulse oximeters which are
used to take vital signs, assess, and monitor patients. These purchases will replace broken, worn and outdated
equipment and will have a useful lifetime of at least five years. All of this equipment will be used to provide
direct medical care to patients.
4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the
Gila River Indian Community.
The proposed project satisfies the interest of Gila River Indian Community in healthcare as the HonorHealth
Forensic Nurse Examiners Program provides direct patient care that assists in improving the health of those in
our community who experience sexual assault and domestic violence strangulation. The project also
addresses the interest of public safety as it works collaboratively with law enforcement to meet the needs of
patients who are victims of crime. Patients will also receive assistance with safety planning from the program’s
Victims Advocate to increase their safety in the future.
5. Identify the needs/problems to be addressed, target population and number of people to be served by
the project.
Sexual violence in Arizona is a greater problem compared to the US as a whole. In 2014, 2,445 cases of rape
and 200 cases of attempted rape were reported in Arizona (Arizona Department of Public Safety. Crime in
Arizona 2014, 2015). According to Arizona Crime Trends, a 2015 report published by the Arizona Criminal
Justice Commission, over the ten year period from 2004-2013, the rate of forcible rape increased in Arizona
(from 33.0 per 100,000 people to 35.4 per 100,000 people) while, during the same timeframe, the rate
decreased nationally (from 32.2 to 25.2).
Intimate partner violence is experienced by one in four women in their lifetime (National Intimate Partner and
Sexual Violence Survey, United States, 2011), and 68% of these women will experience near-fatal
strangulation by their partner (Taliaferro et al (2009) Strangulation in Intimate Partner Violence).
Sexual assault and domestic violence strangulation result in significant physical trauma, anguish, and suffering
of victims. These individuals deserve a competent and compassionate medical forensic examination and
supportive services that are delivered in a culturally sensitive, dignified, and patient-centered manner.
The HonorHealth Forensic Nurse Examiners Program serves patients who are victims of sexual assault over
age 12 and all ages of domestic violence strangulation. In 2017, the program served more than 2,400 patients
– 93% of whom are women. Eighteen percent of patients are under the age of 18 and 82% are adults. The
program has documented an average increase in patients of approximately 8% per year who experience
interpersonal violence. Based on this average rate of growth, the program estimates serving approximately
2,600 patients in 2018.
6. Define the project as a new or continuing program.
The HonorHealth Forensic Nurse Examiners Program is a continuing program that was established in 1998.
7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible.

Gila River Indian Community Grant Application
Grant Cycle 2018
Start date of the program is November 1, 2018 with estimated completion by January 31, 2019. Once funding
is received, equipment can be purchased and in place for use by the program within 90 days. The timeline is
flexible.
8. Identify other organizations, partners or funders participating in the project and their roles.
The HonorHealth Forensic Nursing Examiners Program collaborates with local law enforcement to respond
within one hour to victims of sexual assault and domestic violence strangulation throughout Maricopa County
and rural outlying areas. The program also works with the Maricopa County Crime Lab in the collection of
evidence. The program’s Victims Advocate will work with community-based service providers, including
HonorHealth’s affiliated community health centers, to address the health, safety, and social services needs of
patients.
No other partners will be required for the purchase and installation of the equipment for which funding is
requested from Gila River Indian Community.
9. Would you be able to implement the proposed plan if your organization received partial funding for this
project?
Yes, if partial funding is awarded, HonorHealth will reduce the equipment purchase accordingly.
10. Describe your plan for project financial sustainability beyond the grant period. If this is a
program/project previously funded by the Gila River Indian Community describe efforts made towards
the previously described sustainability plan.
This funding request to Gila River Indian Community will cover one-time costs for purchase of equipment
needed for direct patient care. The equipment has a useful life of at least five years. As additional program
equipment needs may arise in the future, HonorHealth will look to individual donors and other sources of
funding for support.
11. Describe your plan to document progress and results.
HonorHealth can document the purchase of equipment with receipts for purchases. The program is also
happy to send photos of the new equipment once it is placed for use by the program.
12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community
for state shared revenues for this and any other project for the past five (5) years. If this information is
included in a separate attachment, please indicate that here.
Currently, there are no other grant applications pending and/or awards made to HonorHealth by a tribe other
than Gila River Indian Community for this and any other project for the past five years. However, in August,

Gila River Indian Community Grant Application
Grant Cycle 2018
2017, HonorHealth submitted a request for forfeited jackpot funds from Gila River Gaming Enterprises for its
Military Partnership Program, which was declined. Thank you very much for considering this proposal for Gila
River Indian Community’s support to our community.

Project Budget Detail
Please provide a narrative description for each of the project budget items listed on the previous page. Include
the dollar figure and how it was derived.
1. Gynecological exam beds are needed to replace two older exam beds that are broken and cannot be
adequately repaired. $12,294 will be used to purchase two new exam beds at $6,147 each.
2. Exam bed covers are needed as replacements for worn covers on four existing exam beds. $1,776 will
purchase four new exam bed covers at $444 each.
3. Stand lights are needed for adequate lighting to perform exams and will replace existing gooseneck lamps
which are outdated, broken, and often get too hot. $5,400 will pay for six stand lights at at $900 each that will
be placed in each of our six exam rooms.
4. Littman stethoscopes are needed to replace 6 old and worn stethoscopes. Littman stethoscopes are very
high quality and are used to obtain vital signs and listen to the heart, lungs, and bowel sounds. $300 will pay
for six stethescopes at $50 each.
5. New pulse oximeters are needed to replace 6 old and outdated models. Pulse oximeters indirectly monitor
the oxygen level or oxygen saturation of a patient’s blood. They measure how well oxygen is being sent to
parts of the body furthest from the heart – such as arms and legs. These measures are part of obtaining a
complete sed of vital signs. $300 will pay for 6 pulse oximeters at $50 each.
