The activities include; • Women participate in 21 (2-hours) classes that reshape thinking and decision making skills • Participant will engage in 3 achievement celebrations of the course of 21 week program • Each participant is connected with support staff that performs intense follow up and social supports • Participants are administered a skills assessment to assist in employment goals; resume development, interview skills development, job searching and interview follow up. • Reflection Survey - interview session with support staff to reflect on 30, 60, 90 day goals achieved. • Post Program - Hope's Crossing has aftercare support for participants that have completed the 21 weeks program. Hope's Crossing celebrates a 92% success rate for women that complete the Fundamentals of Freedom program. Ensuring women return to the community empowered for change, obtaining sustainable employment, establishing healthy relationships with family and children, and becoming a contributing member of their community. The population we serve , women 18 yrs of age and older through the Fundamentals of Freedom program that experience a significant reduction in risk or the risk has been completely eliminated. Gila River Indian Community Grant Application Grant Cycle 2018 5. Identify the needs/problems to be addressed, target population and number of people to be served by the project. Fundamentals of Freedom (FoF) program has a target population of women 18 yrs and older that have been incarcerated or at risk of incarceration, within communities of Maricopa County in Arizona. The total number of people served by this project over the 3 year proposal period will the 195. There has been a long time focus on reducing recidivism, revolving door of the prison system for individuals released from prison and the time period in which they relapse back into criminal behavior. National Institute of Justice (NIJ) produced a report from tracking recidivism. Over 400,000 released inmates showing two thirds (67.8%) of released prisons will be rearrested within three years. For those rearrested, over half of them (56.8%) were rearrested by the end of the first year ([note 1] Durose, Matthew R., Alexia D. Cooper, and Howard N. Snyder, Recidivism of Prisoners Released in 30 States in 2005: Patterns from 2005 to 2010 (pdf, 31 pages), Bureau of Justice Statistics Special Report, April 2014, NCJ 244205.). Upon reentering society, former offenders are likely to struggle with substance abuse, lack of adequate education and job skills, limited housing options, and mental health issues. The Fundamentals of Freedom (FoF) program serves as a reentry program that facilitates the process to cognitive transformation which leads to a mental shift in criminal thinking and criminal behavior. The FoF program has also served beneficial for individuals at risk of jail or prison because of the criminal behavior of substance abuse, domestic violence, , but has not be adjudicated by the justice system. NIJ continues to describe that this mental shift leads to a reduction in recidivism in our comminutes, creating safer communities. 6. Define the project as a new or continuing program. The Fundamentals of Freedom project is a continuing education and training program for the offered to women 18 years and older for the past 7 years. 7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible. Because the Fundamentals of Freedom is an existing program, we have a predefined series of 4 starts per year. Start date - January - Finish date - June Gila River Indian Community Grant Application Grant Cycle 2018 Start date - March - Finish date - August Start date - June - Finish date - October Start date - August - Finish date - Jan A participant can enter the program at either of these start dates, which makes the timelines very flexible and open for women interested in the program throughout the calendar year. 8. Hope's Crossing has partnered with Fresh Start Women's Resource Center since 2016 and their role as a community partner provides meeting space, participant referrals to our program and share access counseling and legal aid service by their organization for the participants in our program. Fresh Start is also a referral agency to Hope's Crossing, for participants they serve that could benefit from Fundamentals of Freedom program. Hope's Crossing has partnered with Dress for Success - Phoenix since 2016 and their role as a community partner provide interview clothing and employment attire to women participating in job seeking activities. 9. If this is a program/project previously funded by the Gila River Indian Community describe efforts made towards the previously described sustainability plan. Current funding streams include Fundraising Campaigns - 50%; Private Donations - 31%; Grant Income - 18%; and Corporate Contribution - 1%. Our plan include leveraging opportunities to increase revenue streams in the following funding areas of Private Donations, Fee For Service income with community agencies; and Fundraising income. Increase in these funding streams will aid in the long term sustainability beyond the grant period. 11. Describe your plan to document progress and results. Gila River Indian Community Grant Application Grant Cycle 2018 The Fundamentals of Freedom programmatic objective is to strengthen participants in four specific area's of life; obtain and sustain employment, safe and adequate housing, emotional health, and family reunification. The plan includes the personal goal, suggested timeline, perceived barriers, resources to overcome barriers and outcome & date. Each plan is signed and dated by both participant and program team member.
