Grant Cycle 2018
Narrative
Please structure your proposal to provide the following information in the order indicated. Provide the narrative in
paragraph form in the text field provided. Please be thorough but strive for brevity.
1. Briefly describe the organization’s history, mission and goals.
Maricopa County is home to one of the largest regional parks systems in the nation with over 120,000 acres of
open space parks that include hundreds of miles of trails, campgrounds, nature centers and the Desert Outdoor
Environmental Learning Center at Lake Pleasant.
Currently, there are 11 regional parks in the system which we're happy to report were visited by over 2.1 million
people in 2013. Whether you're planning on hiking along a barrier-free trail, enjoying the scenic Sonoran Desert
views on horseback, or peddling rigorously up a trail on a mountain bike, the parks offer a variety of
opportunities for all types of users, ages and comfort levels. Best of all, Maricopa County's regional parks are
all within a 45-minute drive from downtown Phoenix!
The department is staffed by a team that is passionate about what they do and dedicated to our mission and
vision which are:
Vision: Our vision is to connect people with nature through regional parks, trails and programs, inspire an
appreciation for the Sonoran desert beauty and natural open spaces, and create life-long positive memories.
Mission: Our mission, through responsible stewardship, is to provide the quality parks, trails, programs,
services and experiences that energize visitors and create life-long users and advocates.
The Mission of the Maricopa County Department of Public Health is: to promote, protect and preserve the
health of individuals and diverse communities in the Valley of the Sun.
MCDPH staff seeks to address health inequities amongst our community’s most vulnerable - those that have
disabilities, less access to health care, lower incomes, and those that are non-white. Persons with disabilities
have higher rates of chronic diseases.
This proposal to provide park infrastructure in a West Valley Park experiencing high growth patterns will be an
investment in health equity that supports the mission of the MCDPH in helping families with special needs find
safe and appropriate places to recreate and achieve health.
2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona
municipalities including past and present support, whether through partnerships, collaborations, in-kind

Gila River Indian Community Grant Application
Grant Cycle 2018
contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If
the applying organization is a municipality governmental sub-division please enter the text “Not applicable.”
Not applicable.
3. Describe the proposed project and planned goals and activities.
The project would implement a total of 14 muilti-funtional outdoor fitness equipment pieces around an
exisiting (but new) paved walkway. Several pieces of exercise equipment are ADA accessible and all of the
fitness areas are within a close proximity to the super playground. Each county parks hosts a variety of
programs where groups can participate in instructer lead activities. If the grant request is successful, the
Department of Public Health and Parks and Recreation would work together to establish additional fitness
programs that are focused on the utilization of the outdoor fitness track and equipment.
4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River
Indian Community.
The project is focused on recreational activity that promotes physical fitness while connecting people with
nature. Connecting people with nature has been shown to improve a person’s overall health and wellbeing.
Both physical and mental health through physical fitness, wellness or recreation activities are represented in
the Healthcare example on third page of the grant.
5. Identify the needs/problems to be addressed, target population and number of people to be served by the
project.
Estrella Mountain Regional Park is currently being revitalized and would greatly benefit from this fitness
equipment. The equipment would offer park visitors a new activity to get healthy while being surround by
nature and the Sonoran Desert. Since Estrella is located southwest of the phoenix metropolitan area it would
predominantly surve residences from South Phoenix, Avondale, Goodyear, Litchfield and Buckeye.
6. Define the project as a new or continuing program.
The remodel and revitalization of Estrella Mountain Regional Park is about a year and a half in, however the
idea of adding in a physical fitness loop is relatively new and has not begun due to budget constraints and
lack of funding. This grant would allow the Parks Department to implement equipment for a fitness loop that
we otherwise would not be unable to afford.

Gila River Indian Community Grant Application
Grant Cycle 2018
7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible.
If the funds are granted for this project we would proceed with ordering the equipment. The equipment can
be fabricated and shipped within two months. While the equipment is being fabricated, the Parks Trades
Crew or a county contractor would lay the concrete slabs for the equipment to be mounted on. Trees would
be purchased and planted the same week the concrete slabs are poured. Drip irrigation would also be
brought to the new trees via the main irrigation line. Once the equipment is received the Installation would
take approximately two to three days. The overall turn around time for this project is relatively short, at
approximately three and a half months from start to finish.
8. Identify other organizations, partners or funders participating in the project and their roles.
Maricopa County Department of Public Health is poised to work with Parks on additional programing
revolving around the incorporation of this outdoor fitness equipment. The Department of Public Health’s
vision is that of “a healthy and safe community” whos mission is to “protect and promote the health and
well-being of all of our residents and visitors.” Both departments are focused on the physical and mental
health of community members. Public Health would like to see Estrella Regional Park include additional
options like this for community members to be more active as well as promote inclusivity through play. The
two departments are working together to see that these ideas come to fruition at Estrella Regional Park.
9. Would you be able to implement the proposed plan if your organization received partial funding for this project?
Unfortunaly, the department does not have the funds to purchase the fitness equipment. The only way we
would be able to move forward with this project would be through a grant funding opportunity such as this.
10. Describe your plan for project financial sustainability beyond the grant period. If this is a program/project
previously funded by the Gila River Indian Community describe efforts made towards the previously described
sustainability plan.
This is a one time funding request. Once the fitness equipment is purchased and installed in the park, the
maintenance of the equipment would be undertaken by the park staff. Parks and Public Health are working
together on implementing group fitness programs at the park which would start when the equipment has
been incorporated.
11. Describe your plan to document progress and results.

Gila River Indian Community Grant Application
Grant Cycle 2018
With all of our projects, we take before, during, and after photos to document the progress of each project.
Additionally, we upload photos to the “projects” tab on the parks corresponding facebook and web page. A
communications plan for this project could also be compossed in order to inform GRIC of project milestones
if so desired.
12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state
shared revenues for this and any other project for the past five (5) years. If this information is included in a
separate attachment, please indicate that here.
This is Maricopa County Parks and Recreation Department’s first time applying for the 12% revenue sharing
funds for this project. If these funds were provided for this project, the Parks Department would like to
recognize the Gila River Indian Community as sponsors of the fitness equipment via the instructional plaques
at each station.
