Narrative
Please structure your proposal to provide the following information in the order indicated. Provide the narrative in
paragraph form in the text field provided. Please be thorough but strive for brevity.
1. Briefly describe the organization’s history, mission and goals.
In 2013, The Van Norman brothers, Kevin and Jeff, founded Opportunity4Kids with the goal to change the lives of
Arizona Youth and promote equal opportunity for children in need during Out-of-School time. Participation is supported
by providing financial assistance, equipment and uniforms. Far too frequently families lack the funds to purchase
sporting equipment and uniforms needed for youth to participate and simply do not have the financial resources to pay
for registration fees required by most out-of-school time activities. Opportunity4Kids is organized to change the lives of
children; to foster out-of-school time and extracurricular participation; to promote healthy and active lifestyles; to
further mental and social development; and allow equal opportunities for children in need. In the past two years alone,
Opportunity4Kids has granted 265 individual scholarships to children at or within 1.5% of the federal poverty line and
provided community programs that impacted over 2,000 youth.
2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona
municipalities including past and present support, whether through partnerships, collaborations, in-kind
contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If
the applying organization is a municipality governmental sub-division please enter the text “Not applicable.”
Opportunity4Kids has been a 501(c)(3) nonprofit since 2013 and an Arizona Qualified Charitable organization
since 2015. We provide financial support to underprivileged youth and their families living throughout
Maricopa County, the primary locale which we serve. Since 2013, Opportunity4Kids has also been included in
the Maricopa County Charitable Contribution Campaign and Scottsdale Charitable Contribution Campaign.
3. Describe the proposed project and planned goals and activities.
To provide resources for children in Maricopa County that fall within 1.5% of the federal poverty line to
engage in Out-of-School time activities. These activities serve not only as childcare but also as developmental
opportunities, furthering individual growth as well as peer-to-peer interaction.
We would facilitate these opportunities by providing individual Youth Out-of-School Time Scholarships to pay
for registration fees required to participate; or by providing equipment and uniforms as necessary for children
engaging in out-of-school time activities. Activities available for Scholarship would include afterschool
programming, extracurriculars and summer camps. Opportunity4Kids does not provide the programming
ourselves but rather works with local organizations such as Boys & Girls Clubs, ICAN, City Programs/Leagues
and more, who offer affordable, quality services located nearby the Scholarship Youth. This ensures
programming is accommodating of family routine and serves a value in both the youth and parent’s lives.

Gila River Indian Community Grant Application
Grant Cycle 2018
Our goal of this specific funding request is to provide 30 individual Youth Out-of-School Time Scholarships to
Maricopa County youth. We expect youth to experience an increase in self-esteem, responsibility and sense of
community, as well as improved communication with their peers.
4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River
Indian Community.
Opportunity4Kids will foster out-of-school time participation that serves many purposes within the Maricopa
County Community:
• Promoting healthy and active lifestyles serving the field of Healthcare- more specifically making
physical fitness and wellness activities accessible during out-of-school time.
• Furthering mental and social development in the field of Education- Out-of-School Time activities
provide countless social and interpersonal developmental benefits thus extending the classroom
outside the traditional school day.
5. Identify the needs/problems to be addressed, target population and number of people to be served by the
project.
Per the US Census Bureau’s 2015 American Community Survey, in Maricopa County there were 1,014,531
children under the age of 18 years old in households and 753,436 enrolled in school. The percentage of
children living in households at or below the poverty line is 24.4% and it was determined that 28.7% of
children are living in households with supplemental security income, cash public assistance income or
receiving food stamp benefits. Opportunity4Kids is a much-needed resource with its main goal to provide
assistance to children ages 5-18 years old, who are students enrolled in the Maricopa County school district.
We will target a minimum of 30 unique youth that would otherwise not be involved in out-of-school time
activities due to their financial circumstances.
6. Define the project as a new or continuing program.
Grant funding will supplement the organization's annual fundraising goals of $100k to provide financial
resources that will change the lives of children in Maricopa county. Since 2013, Opportunity4Kids has provided
out-of-school time activities and resources for children, positively impacting over 2,200 children. Awarded this
additional funding, Opportunity4Kids will dramatically increase youth enrollment in out-of-school time
activities. There is a desperate need for this project in Maricopa County.
7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible.

Gila River Indian Community Grant Application
Grant Cycle 2018
The timeline for this project is somewhat flexible but proposed to begin in November of 2018 and conclude as
late as November 2019. Scholarships will be evaluated and awarded on a case-by-case basis to children whose
parents complete the online application process. Scholarships will fluctuate will school semesters and activity
seasons.
8. Identify other organizations, partners or funders participating in the project and their roles.
Opportunity4Kids has established a supportive network of providers throughout Maricopa County by building
relationships with activity organizations that provide influential out-of-school time programming such as: the
Boys and Girls Clubs of Metropolitan Phoenix, Skate After School, The Valley of the Sun YMCA, City of Phoenix
Recreational Activities, ICAN, ATA Martial Arts, Various Soccer Clubs, Phoenix Center for the Arts, San Marcos
Music Academy, ProCamps, Ballet Arizona, The Hubbard Family Swim School, The Little Gym and many more.
These organizations support Opportunity4Kids’ goals of providing financial resources by discounting or
reducing registration fees required to participate supporting the overall Scholarship of more children.
9. Would you be able to implement the proposed plan if your organization received partial funding for this project?
The proposed Out-of-School Time Project is a supplemental funding request. This is a much-needed project
and we hope to provide substantial assistance to underprivileged children in Maricopa County. We will
increase youth enrollment in out-of-school time activities by at least 125% specifically targeting those who are
at most in need of financial help. Opportunity4Kids will track household location of those who participate to
pinpoint specific areas of higher need for future focus.
10. Describe your plan for project financial sustainability beyond the grant period. If this is a program/project
previously funded by the Gila River Indian Community describe efforts made towards the previously described
sustainability plan.
This is our second time applying for a grant opportunity through the Gila River Indian Community. This is a
much-needed project presented by a unique nonprofit organization; due to increasing costs of out-of-school
time activities and childcare, combined with the expanding population of children living below the poverty
line, coupled by the federal funding cuts to out-of-school time programming, the number of children needing
assistance to access out-of-school time activities has grown.
Providing these opportunities to children at risk and in need will very positively impact their lives and the
community. It has thus been identified that there is a high need for this type of organizational resource serving
underprivileged youth in Maricopa County. We are seeking this as a supplemental funding grant. Our end goal
is to someday provide this type of support to all Maricopa County youth in need. Annually Opportunity4Kids

Gila River Indian Community Grant Application
Grant Cycle 2018
conducts ongoing fundraising efforts through events and donation campaigns. Fundraising goals increase each
year.
11. Describe your plan to document progress and results.
To identify the success of Scholarships provided, Opportunity4Kids requires a narrative from the referring
Parent/Guardian/Case Worker on the behalf of the child’s mental/physical/emotional state. In addition, the
Parent/Guardian/Case Worker will complete a post survey to determine individual progress and development.
Through the Scholarship application process, Opportunity4Kids’ Director of Operations will utilize a tracking
system to record information on children and their families applying for Scholarship. Information gathered will
include- school enrollment, age of participating children, participation in government financial assistance
programs, family financial situation, disabilities and individual stories. The expectation is that 100% of children
provided Scholarships will fall within 1.5% of the federal poverty guidelines and at least 85% also receive
additional government assistance. Furthermore, 90% will show increase in self-esteem and social skills due to
participation in programming.
12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state
shared revenues for this and any other project for the past five (5) years. If this information is included in a
separate attachment, please indicate that here.
For the period of 2015-2016, Opportunity4Kids was a recipient of $35,000 from a 12% Tribal Gaming Grant
through the Salt River Pima Maricopa Indian Community, and then $20,000 in 2017-2018. Maricopa County
Supervisor Steve Chucri supported our application for both awards.
