Narrative
Please structure your proposal to provide the following information in the order indicated. Provide the narrative in
paragraph form in the text field provided. Please be thorough but strive for brevity.
1. Briefly describe the organization’s history, mission and goals.
We began in 2008 as a ministry of St. Timothy Catholic Church serving food to migrant workers in the fields and
evolved to an independent 501(c)(3) non-profit organization. In 1994 we established our own facility located at
Broadway and Arizona Avenue. Since then we have been serving evening meals and distributing food boxes
daily.
Our mission at Paz de Cristo is to feed, clothe and empower those in need in the Phoenix East Valley. Much of
our resources are devoted to providing healthy and nutritious meals to the poor and under served in the East
Valley. In addition to our food programs we provide showers and haircuts for the homeless, bus tickets for
medical appointments or travel to a job , gift cards for medical prescriptions, vouchers for Arizona ID's and
clothing for men, women and children.
Our goals are to empower our guest to reach their full potential by removing barriers so they may become more
self-sufficient.
2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona
municipalities including past and present support, whether through partnerships, collaborations, in-kind
contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If
the applying organization is a municipality governmental sub-division please enter the text “Not applicable.”
Paz de Cristo has been located in Mesa for the past 29 years and enjoys a close and supportive relationship with
the City of Mesa. The Mayor, City Council representatives and many City employees volunteer at Paz on a
regular basis. The Mayor has spoken at several of our fund raising events. We are on the Mayors “Housing our
Heros” task force for supporting homeless veterans. The City courts hold “Homeless Court” at Paz de Cristo once
each month. We are a part of the East Valley Access to Housing work group that works with other east valley
cities and Maricopa County Continium of Care Coordinated Entry system to review and implement assessment
tools
3. Describe the proposed project and planned goals and activities.
The proposed project is to upgrade the dining room building by installing air condiditoning and enclosing the
building. The reason we want to enclose the building and install air conditioning is to provide an area where
our homeless guests can congregate during the day. We have initiated a program in collaboration with Marc
Community Resources and the City of Mesa to address the needs of those clients experiencing mental health
and behavioral health issues. Marc staff members with experience in assessment and program navigaton will

meet with clients at the Paz dining facility. This facility is currently an open air facility with no air conditioning.
The goal of the program is to coordinate with other community resources to provide housing support and
employment services to individuals who are homeless and suffering from mental health and behavioral health
issues.
4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River
Indian Community.
We believe our request falls in the priority area of Health Care. Statistics show that those individiuals living in
poverty have a higher rate of medical expenses thatn those with higher incomes. Providing housing,
employment as well as eligibility for multiple benefits including AHCCS, food stamps, SSI, SSEI and other
specialized needs will help the mental health issues and behavioral health issues of our population.
5. Identify the needs/problems to be addressed, target population and number of people to be served by the
project.
The needs/problems to be addressed are the large number of guests that come for services at Paz de Cristo that
have mental health and behavioral health issues. We currently average 7,000 visits each month from
approximately 2,100 unique individuals. We believe with this new program and upgraded facility, we can
serve30-50 individuals per month with a combination of housing, employment and eligibility for benefits. Over
the previous yhear, Marc Community Resources served 342 individuals who were homeless or about to lose
their housing withour our intervention. Many of these individuals had either mental illness or substance abuse
disorders and were frequently hospitalized and/or involved with criminal justice for a variety of crimes.
6. Define the project as a new or continuing program.
This is a new program based on existing, successful programs at Marc Community Center.
7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible.

8. Identify other organizations, partners or funders participating in the project and their roles.
Marc Community Resources is providing the two Navigators for the program. The City of Mesa is providing
funding for the Navigators. Paz de Cristo will attempt to find another grant resource to provide funding for
the facility upgrades.
9. Would you be able to implement the proposed plan if your organization received partial funding for this project?
We would need the full $100,000 to complete the facility upgrade. We are currently looking for other
funders/foundations to provide some of the funding.
10. Describe your plan for project financial sustainability beyond the grant period. If this is a program/project
previously funded by the Gila River Indian Community describe efforts made towards the previously described
sustainability plan.
Once the facility is upgraded their will be no further funding needed. We currently have operating funds to
maintain the facility.
11. Describe your plan to document progress and results.
All client contacts are recorded in our software program with all activities documented. Progress for the
facility upgrades will be documented via a project management gantt chart.
12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state
shared revenues for this and any other project for the past five (5) years. If this information is included in a
separate attachment, please indicate that here.
