Narrative
Please structure your proposal to provide the following information in the order indicated. Provide the narrative in
paragraph form in the text field provided. Please be thorough but strive for brevity.
1. Briefly describe the organization’s history, mission and goals.
For more than 60 years United Cerebral Palsy of Central Arizona, Inc. (UCP) has served the developmental disability
community in Arizona. Our programs touch the lives of more than 5,000 children and adults each year and include an early
learning center, early intervention, afterschool and summer programs for children and teens, physical, occupational, speech
and feeding therapy, a day treatment program for adults and home and community-based services.
UCP serves infants, children and adults with a wide array of disabilities including autism, cerebral palsy, sensory processing
disorder, genetic disorders, orthopedic disabilities, developmental delay and more. Program services are delivered at the
Laura Dozer Center in north Phoenix and UCP Downtown, 7th Street at Roosevelt. Our mission is to deliver comprehensive
services to individuals with disabilities and their families by providing physical and developmental support, as well as,
educational growth which is the foundation for independent living and a “Life without Limits”.
2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona
municipalities including past and present support, whether through partnerships, collaborations, in-kind
contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If
the applying organization is a municipality governmental sub-division please enter the text “Not applicable.”
Maricopa County provides the local government entity to submit application to the State-Shared Revenue
Program. UCP does not receive in-kind contributions, tax-based funding or any other funds from Maricopa
County.
3. Describe the proposed project and planned goals and activities.
Gila River Indian Community funding is the catalyst to ensuring that children with disabilities have the resources and
education they need and deserve. The proposed project affords the opportunity for children with disabilities ages 3-17
and young adults still in high school to continue to participant in afterschool and summer programs. The four main
components of the program include: physical activity, socialization, communication and activities of daily living. Training
and education includes personal hygiene, decision making, meal planning/cooking, socialization, money management
and computer skills. Activities and classes include music, dance, social outing, community events and more. The goals of
the program include:

Grant Cycle 2018
1. To continue to provide quality afterschool and summer programs that meet the recreational, social and
educational needs of children and young adults with disabilities.
2. To further expand services in the UCP afterschool and summer program to serve 120 each year.
3. To increase opportunities by providing children and young adults with meaningful educational opportunities,
community outings and social experiences.
4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River
Indian Community.
The proposed project aligns with the Gila River Indian Community priority funding areas to assist students in
achieving success in the classroom, providing hands-on learning and enhancing the general knowledge of
chidren and young adults with disabilities. In addition, UCP afterschool and summer programs align with the
Gila River Indian Community’s priority to promote wellness activities, recreational activities, physical fitness
nutrition. We accomplish these goals by working with family members to develop individual support plans
and by ensuring the highest quality of staff in the delivery of service and education.
5. Identify the needs/problems to be addressed, target population and number of people to be served by the
project.
Research conducted by the ASU Morrison Institute of Public Policy, 2014, stated that children with disabilities
are often excluded from afterschool and summer programs as families/caregivers fear bullying, cite lack of
transportation and anxiety regarding fear of failure as societal barriers to success for the child in community
settings, school and employment. The UCP Afterschool and Summer Program affords children with
disabilities the opportunity to experience and achieve success by participating in enriching and healthy
lifestyle activities, that otherwise they would be not be able to experience.
6. Define the project as a new or continuing program.
The project is continuing as UCP has provided afterschool and summer programs for over 25 years.

7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible.
UCP plans to implement the project as follows: Year 1: November 2018 to October 2019 and Year 2:
November 2019 to October 2019. The timeframe is flexible and determined by the needs of the children and
primarily based on school calendars for start and finish dates for the afterschool and summer programs.
8. Identify other organizations, partners or funders participating in the project and their roles.
UCP is committed to collaborating with the community to ensure that children with disabilities have the
opportunity to fully participate in educational, social and community activities. UCP partners with 10 school
districts in Maricopa County to provide transportation for children with disabilities to afterschool and
summer programs, these include: the Deer Valley United School District, Washington Elementray School
District Paradise Valley School District, Peoria School District, Glendale Union High School District and various
charter schools that include the Foundation for Blind Children. UCP operates under funding from the Arizona
Department of Economic Security, Division of Developmental Disabilities, however funding provided on a
reimbursement basis are not enough to sufficiently cover the full costs necessary to staff the program, nor to
support enrichment activites, specialized equipment, and community outings. Additional funding derived
from individual donors through fundraising efforts.
9. Would you be able to implement the proposed plan if your organization received partial funding for this project?
Yes
10. Describe your plan for project financial sustainability beyond the grant period. If this is a program/project
previously funded by the Gila River Indian Community describe efforts made towards the previously described
sustainability plan.
Government funding is the important base that contributes to the sustainability of the program, additional fundraising
efforts promote the continuation of services.
11. Describe your plan to document progress and results.
Programs and results are determined both individually and as a program. For the individual, an individual
service plan and monthly progress report documents progress and results. On the program level, quarterly

survey to parents and guardians help to determine the program’s success. Data collection and collecting
stories of those we serve further document the success of the program.
12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state
shared revenues for this and any other project for the past five (5) years. If this information is included in a
separate attachment, please indicate that here.
In 2014, UCP received $25,000 fro the Tohono O’Odham Nation to provide scholarships for children with
disabilities who attend the UCP Early Learning Pre-school. The Early Learning Center provides services for
children birth to 5 years of age and the program is not associated with the Afterschool and Summer Program
for which UCP requests funding through the Gila River Indian Community.
