EXHIBIT P – CHILDHOOD LEAD POISONING DATA
1. BACKGROUND & PURPOSE:
Houses and apartments built prior to the 1978 federal ban of lead-based paint have an
increased risk of containing lead. An estimated 24 million housing units in the United States are
contaminated. Lead sources may come from deteriorating paint, dust, and soil. Lead painted
surfaces may not become a hazard if they are kept in good condition. However, deteriorating
paint (paint that is peeling, chipping, chalking, cracking, or damaged), due to age or being on an
impact or friction surface is highly hazardous. Surfaces such as windows and sills, doors and
door frames, stairs, railings, banisters, and porches, have all been identified as areas where dry
scraping, sanding, heating, or heavy use can lead to deteriorating paint and lead dust.
The objective of sharing childhood lead poisoning data is to ensure housing-related lead
exposures are evaluated, reduced and/or eliminated for children identified with elevated blood
lead levels residing in Maricopa County.
Numerous individual Housing Authorities within Maricopa County receive funding from the U.S.
Department of Housing and Urban Development for public housing and rental assistance. These
Housing Authorities aim to prevent childhood lead poisoning by ensuring lead-safe housing per
the Lead-Safe Housing Rule (24 CFR 35, subpart L.). They are required to complete an
environmental investigation and lead-based paint hazard reduction (when applicable) of the
dwelling unit to which this subpart applies, when a child aged six or less has been identified with
elevated blood lead levels.
ADHS is authorized by statute to develop and conduct local programs for the prevention,
detection and treatment of lead-based paint poisoning, including programs to detect incidents of
lead poisoning due to lead-based paint and other sources among community residents.
Pursuant to this agreement, Maricopa County Department of Public Health (MCDPH) has
designated Housing Authorities within Maricopa County, or Housing Authorities that are divisions
of the Maricopa County Government, as their authorized agents to both receive lead poisoning
public health data shared under this MOU, and to conduct all public health interventions deemed
necessary and appropriate. These interventions may include evaluation of lead hazards in the
homes of individuals identified with elevated blood serum lead levels, and actions undertaken to
mitigate lead exposure to occupants of the household. The objective shared by both MCDPH
and their designated agent in using the public health data shared under this MOU is limited to
public health intervention activity to decrease the prevalence of lead-based paint poisoning
among children residing within Maricopa County. Both MCDPH and their authorized agents will
be subject to the limitations and protections of shared public health data owned by ADHS as set
forth in this MOU.
Sharing between ADHS and MCDPH, the addresses for children aged six or less who have
elevated blood lead levels will allow both entities to identify barriers to eliminate blood lead
poisoning such as:
1.1. Lead-based paint hazards in the home.
The shared vision of the AGENCY and ADHS includes:
1.2. Identify the source(s) and locations of lead-based paint exposure in Maricopa County
dwellings receiving assistance from the Housing Authority
1.3. Eliminate the lead source(s) when possible

child’s dwelling is being subsidized by available housing assistance programs.
2.1. ADHS will:
2.1.1. Provide MCDPH with information from ADHS’ blood lead reporting system
regarding children aged six or less who reside in Maricopa County and for
whom an elevated blood lead level of 5 µg/dL or greater has been reported.
The information shall consist of the child’s age and address. Reports will be
provided on a quarterly basis.
2.2. Maricopa County will:
2.2.1. Enter into data sharing agreements with authorized Housing Authorities
within Maricopa County for the purposes of determining 1) whether children
identified with elevated blood lead levels are residing in housing units
subsidized by the Section 8 Rental Assistance Program and if so, 2) the
status of any environmental lead testing and remediation activities.
2.2.2. Provide the designated authorized Housing Authority with information
received from ADHS’ blood lead reporting system regarding children aged six
or less who reside in their jurisdiction and for whom an elevated blood lead
level of 5 µg/dL or greater has been reported. The information shall consist of
the child’s age and address. Reports will be provided on a quarterly basis.
2.2.3. Compile data received from the Housing Authorities regarding environmental
lead testing and remediation activities on all units within their jurisdiction
identified as housing children aged six or less for whom an elevated blood
lead level of 5 µg/dL was identified.
2.2.4. Provide quarterly reports to ADHS identifying whether any of the children
identified with elevated blood lead levels reside in units subsidized by the
Section 8 Rental Assistance Program and if so, the status of any
environmental lead testing and remediation activities.
3. PAYMENT AND AMOUNT:
There are no payments associated with this Agreement.
4. AMENDMENTS:
4.1. All requests for additional services shall be in writing and signed by both parties.

PUBLIC USE DATA AGREEMENT
Background and Purpose:
Houses and apartments built prior to the 1978 federal ban of lead-based paint have an increased risk of
containing lead. An estimated 24 million housing units in the United States are contaminated. Lead
sources may come from deteriorating paint, dust, and soil. Lead paint that is peeling, chipping,
chalking, cracking, or damaged due to age or high impact is highly hazardous.
The United States Department of Housing and Urban Development (HUD) addressed the issue of
environmental lead in federally owned and subsidized housing with the Lead Safe Housing Rule (24
CFR 35, subpart L.), which requires Housing Authorities to complete an environmental investigation
and lead-based paint hazard reduction (when applicable) of the dwelling unit to which this subpart
applies, when a child aged six or less has been identified with an elevated blood lead level of 5 µg/dL or
greater.
Lead poisoning can cause serious developmental damage to children that can last a lifetime, yet it is
completely preventable if lead hazards are removed. The purpose of this project is to reduce the
incidence of childhood lead poisoning due to environmental causes in the home by monitoring lead
poisoning reports and implementing environmental remediation when needed.
Process:
In the proposed project, childhood blood lead level data reported in Maricopa County will be used to
determine locations for environmental testing on housing authority property within Maricopa County. If
environmental treatment and/or remediation is necessary, it will be completed by the responsible
housing authority. All results will be reported back to the Maricopa County by and through its
Department of Public Health (MCDPH) and Arizona Department of Health Services (ADHS).
Housing Authority will:
1. Provide MCDPH with quarterly list of current addresses of the Housing Authority dwelling units
to which the Lead Safe Housing Rule applies.
2. Provide MCDPH with monthly reports, on a date agreed upon by both parties, regarding any
action taken to conduct environmental testing and/or remediation on a Housing Authority
dwelling unit identified by MCDPH as the residence of a child under age 6 with a positive blood
lead level of 5 µg/dL or greater.
MCDPH will:
1. Determine if any child under six that tests positive for an elevated blood lead level of 5 µg/dL or
greater resides in an applicable Housing Authority dwelling unit. MCDPH will provide the
addresses of any positive cases to the appropriate Housing Authority on a monthly basis.
2. Provide ADHS with data received from the Housing Authority regarding actions taken to conduct
environmental testing and/or remediation.
