The Maricopa County Department of Public Health (MCDPH) has had an on-going grant from the U.S, Department of Health & Human Services
(HRSA) since 12/05/2001 for the purpose of running the Homeless Clinic located on the Human Services Campus at 220 S. 12th Avenue in Phoenix.
Through this grant MCDPH delivered quality health care to homeless people through street outreach, integrated primary care, mental health
services, substance abuse services, case management, employment assistance, nutrition services and emergency, transitional and permanent
housing services. As of January 1,2018 MCDPH will no longer be eligible to run the clinic and HRSA has identified a new community based
organization (Circle the City) to take over the daily operations and services. However MCDPH has a surplus of revenue funds (approximatley
$2,000,000) that had been previously generated and are required to be spent on providing these same services to the homeless population, It is our
intent to support the new grantee with these funds by providing funds for staff, pharmaceuticals, and other necessary goods and services.
SINGLE SOURCE CATEGORY (Check applicable box, attach documentation or provide explanation below.)
Maricopa County Procurement Code MC1-351 states: "Single source" means an acquisition where, competition is impractical because
of standardization, warranty, or other factors as approved by the Board of Supervisors.
NOTE: The following are unlikely to be sufficient single source justifications:
• Personal or agency preference for a product or vendor
• Agency perception that the vendor is the best qualified (this should be determined through a competitive process)
• Lack of agency planning resulting in limited time to conduct a competitive procurement
• Special Incentive or deal offered (can be assessed In open and competitive solicitation)
• Agency convenience
• (Z, Grant specified (supporting documents attached) c, Software license renewals, additions, or upgrades available
from only one source
(7, Mailing lists, subscriptions or media advertising C" Brand compatibility available from only one source (no other
' distributors)
C Warranty voided if service provided by other contract vendor C, Other proprietary situation
C Other (Provide written explanation below)

THIS PROCURIVIENT IS A SINGLE SOURCE BECAUSE (attach additional page if needed):
HRSA has selected Circle the City as the new grantee for the Healthcare for the Homeless clinic. The
leftover revenue funds that are requesting to be spent are a direct result of the HRSA grant that Circle
the City is taking over. The grant stipulates that these funds must be spent on providing the
homeless population primary medical care and Circle the City will be the only medical provider
located on the Human Services campus where the biggest concentration of our homeless men and
women go for primary medical care. These funds are a direct result of program revenue and will be
spent supporting the new grantee until expended which is anticipated to occur no later than
12/31/2018,
SEARCH
A search was conducted consisting of: (check all that apply) RESULTS
El Market research IIII Public notice given, list n no alternatives were identified
El Other vendors contacted El Other ,,, no alternatives were deemed acceptable
because (explain below);
Describe the search from above in detail:
Due to the original funds being earmarked for primary medical care to the homeless population
when the grant was received, the program revenue that has been generated must go to providing
the same services and to the same population. Because Circle the City is now taking over the primary
medical care function at this location they are the only viable recipients of this leftover revenue.

Describe methodology and results (attach any written supporting data, e.g., survey or market analysis):
The charges will be the usual standard and customary charges currently in place. In addition, Circle
the City will be submitting a contractor's expenditure report (CER) on a monthly basis that MCDPH
