AMENDMENT NO. 1
TO THE SUBRECIPIENT AGREEMENT FOR SERVICES
BETWEEN
MARICOPA COUNTY
ADMINISTERED BY ITS
HUMAN SERVICES DEPARTMENT
AND
COMMUNITY BRIDGES, INC.
Maricopa County (“County”) administered by it Human Services Department and Community Bridges, Inc.
(referred to as “Subrecipient”) entered into a Subrecipient Agreement for services (“Agreement”) on or
about September 14, 2017 for the provision of Rapid Rehousing services for homeless individuals. The
Agreement was retroactive to July 1, 2017 through June 30, 2018. The County and the Contractor may be
referred to individually as the “Party” or collectively as the “Parties.”
The Parties now wish to amend the Agreement as follows:
I. Correct the amount listed on Section III Work Statement, paragraph B BUDGET.
Activity Duration Amount
Rapid Rehousing 12 months $198,000
II. This Agreement is subject to A.R.S. §38-511.
III. The foregoing paragraphs contain all the changes to the Agreement as amended. All other terms
of the original Agreement and Amendments not amended hereby, remain in full force and effect as
approved and amended.
