BACKGROUND 1.1. The Arizona Department of Health Services (ADHS) is the designated Agency which houses the surveillance programs established under Arizona Revised Statute §36-133. The Office of Health Registries (OHR) contains both the Arizona Cancer Registry (ACR) and the Arizona Birth Defects Monitoring Program (ABDMP). The programs are responsible for the data collection of cancer and birth defects. In addition, the Office of Health Registries provides technical assistance and support to the Bureau of Women’s and Children’s Health (BWCH) in the Pregnancy Risk Assessment Monitoring System (PRAMS) and also receives support from the BWCH. 1.4 PRAMS 1.4.1 PRAMS is a surveillance project of the CDS and State Health Departments. Developed in 1987, PRAMS collects State-specific, population-based data on maternal attitudes and experiences before, during and shortly after pregnancy. PRAMS provides data that is not available from other sources. This data will be used to identify groups of women and infants at high risk for health problems to monitor changes in health status and to measure progress towards goals in improving the health of mothers and infants. 1.4.2 Each year in Arizona, there are hundreds of infants born with serious health problems. Many of these infants die. To better understand this, PRAMS was launched in Arizona in April, 2017. The purpose of PRAMS is to provide information to help find out why some infants are born healthy and others are not. Each month PRAMS will ask 1 in 30 new mothers in Arizona to share their experiences before, during and after pregnancy. This surveillance project will recruit over 2,400 new mothers each year in the State. The effort requires intensive recruitment, data collection, data-entry and follow-up on all participating mothers. All of these activities, including data analysis and reporting, occur inside ADHS as outlined in the PRAMS implementation manuals set by the CDC. 1.4.3 Due to the increasing need for surveillance of new mothers in Arizona, ADHS was awarded a CDC grant to establish and sustain PRAMS. 1.4.4 Due to the limited amount of program resources, PRAMS will require support from outside sources to achieve the overall program requirements and deliverables. In order to successfully launch, implement and maintain surveillance of new mothers in Arizona, Maricopa County Department of Public Health (MCDPH) shall work with ADHS / PRAMS to assist in fulfillment of the grant deliverables regarding public health surveillance activities of new mothers in Arizona. 1.4.5 The functions of PRAMS include, but are not limited to: 1.4.5.1 Identify groups of women and infants at high risk for health problems; 1.4.5.2 Monitor changes in maternal and infant health status; 1.4.5.3 Capture maternal and infant health data that is not currently available from other routine sources; and 1.4.5.4 Perform epidemiological studies and to provide biostatistics and epidemiologic information to the public 2 ARIZONA DEPARTMENT OF HEALTH SERVICES INTERGOVERNMENTAL AGREEMENT (IGA) 150 N 18th Ave., Ste.
