AMENDMENT NO. 2
TO THE CONTRACT FOR SERVICES
BETWEEN
MARICOPA COUNTY
ADMINISTERED BY ITS
HUMAN SERVICES DEPARTMENT
AND
COMMUNITY BRIDGES, INC.
Maricopa County (“County”) administered by it Human Services Department and Community Bridges,
Inc. (referred to as Contractor”) entered into a Contract for Services (“Agreement”) on or about
December 21, 2016, for the provision of Rapid Rehousing for homeless men and women. The
Agreement was retroactive to July 1, 2016 through June 30, 2017. The County and the Contractor may
be referred to individually as “Party” or collectively as “Parties.”
The Agreement was amended on or about August 23, 2017 to extend the term from June 30, 2017 to
June 30, 2018. The Amendment effective date is July 1, 2017 through June 30, 2018. The Parties now
wish to amend the Agreement as follows:
I. Increase the Agreement funding amount by $17,473 from $132,000 to $149,473.
II. This Agreement is subject to A.R.S. §38-511.
III. The foregoing paragraphs contain all the changes to the Agreement as amended. All other
terms of the original Agreement and Amendments not amended hereby, remain in full force
and effect as approved and amended.
