AMENDMENT NO. 1 TO FACILITIES USE AGREEMENT
Between
HELPING FAMILIES IN NEED
And
MARICOPA COUNTY
This Amendment No. 1 to Facilities Use Agreement (this "Amendment") is made and
entered into by and between the Helping Families in Need, an Arizona non-profit corporation
(“Contractor”) and Maricopa County, a political subdivision of the State of Arizona, (“County”).
Contractor and County are sometimes collectively referred to herein as the “Parties” or
individually as a “Party.”
RECITALS:
A. County and Contractor are parties to that certain Facilities Use Agreement No. P-50277
approved by the Maricopa County Board of Supervisors on October 7, 2015 (the
“Agreement”). The Agreement is for use of premises at 635 E. Broadway Road, Mesa, AZ
85204 and 1850 North 95th Avenue, Suite 184, Phoenix, AZ 85307 (collectively, the
“Premises”).
B. The term of the Agreement expires on October 6, 2017.
C. County and Contractors now mutually desire to amend the Agreement to extend the term.
AGREEMENT
NOW, THEREFORE, in consideration of the foregoing and other valuable
consideration, the receipt and sufficiency of which are hereby acknowledged, County and
Contractors agree to amend the Agreement as follows:
1. The term of the Agreement is hereby extended approximately two (2) years, to commence
October 7, 2017, and expire October 31, 2019. This Amendment is subject to termination
pursuant to the provisions of A.R.S. § 38-511. Either party may terminate the Agreement
upon 30-day written notice to the other.
2. The effective date of this Amendment is the date of execution by the Chairman of the
Maricopa County Board of Supervisors.

3. The foregoing paragraphs contain all the changes made by this Amendment. All other
terms and conditions of the original Agreement shall remain the same and in full force and
effect.
