AMENDMENT NO. 2
TO THE
CONTRACT
BETWEEN
MARICOPA COUNTY
ADMINISTERED BY ITS
HUMAN SERVICES DEPARTMENT
AND
BRIGHT FUTURE STARS
I. Maricopa County by and through the Maricopa County Human Services Department (“County”) and
Bright Future Stars, an Arizona corporation (hereinafter referred to as the “Contractor”) entered into
a financial Contract Agreement (“Agreement”) on or about December 12, 2016, to provide Head Start
program services to eligible preschool children ages 3-5 years old.from November 16, 2016 through
June 30, 2017. The County and Contractor amended the Agreement on or about June 21, 2017 to
extend the term from June 30, 2017 to August 31, 2017. The County and the Contractor may be
referred to individually as “Party” or collectively as “Parties.”
II. The Parties wish to amend the Agreement as follows:
A. Extend the contract termination date from August 31, 2017 to September 30, 2017.
B. Funding for Fiscal Year 2018, extension period July 1, 2017 to September 30, 2017 shall
increase from $58,238 to $79,839.
C. Delete in its entirety Attachment B and substitute the attached Program Calendar in its stead.
III. This Amendment is subject to A.R.S. §38-511. The foregoing paragraphs contain all the changes made
by this Amendment. All other terms and conditions of the original agreement remain the same and in
full force and effect as approved and amended.
