AMENDMENT TO CONTRACT

I. The above named contract is hereby amended as specified below:
A. Modify the contract term end date to December 31, 2017;
B. Increase the aggregate, not-to-exceed amount by $70,039, from $210,633 to
$280,672 for the contract term of February 1, 2015 thru December 31, 2017.
II. All other terms and conditions of the original contract shall remain in full force and
effect.
