Dear Sir or Madam:
Maricopa County (County) Department of Public Health Women, Infants, and
Children (WIC) Program has been informed that their WIC clinic located on HTA-
Thunderbird Medical, LLC’s (HTA) property at 5422 W. Thunderbird Road in
Glendale will not be funded in the coming fiscal year. Therefore, the County must
terminate the Lease Agreement by and between HTA as Lessor and Maricopa
County as Lessee; executed on January 21, 2005, and amended on December 15,
2010 and September 21, 2016 respectively (Lease Agreement).
Pursuant to Section 21 of the Lease Agreement, this letter serves as written
notification that earlier today, the Maricopa County Board of Supervisors approved
Agenda Item C-86-05-566-4-05 authorizing early termination of the subject Lease
Agreement effective September 30, 2017 due to non-appropriation of funds. The
County will contact you prior to September 30 to schedule a walk-through and return
of the leased premises.
