Change Order No. 10 shall increase the contract amount $252,395.61 for additional roadway design, structural design, and updates. Much of the
changes resulted from the agency management direction and has been agreed to by the Northern Parkway Executive Committee. The attached
Scope of Work and Cost Proposal include additional labor and direct expenses required to complete the remaining tasks. Also, this change
order shall extend the contract performance period an additional 254 calendar days.
By reason of this change 254 days extension of time will be allowed.
The contract completion date is: February 1, 2018.

Dear Mr. Markert:
Enclosed is our proposed change order request for changes to the scope of the project as outlined in the attached Scope of
Services for Contract Modification No. 10 on the Northern Parkway Phase II Design Project.
To address these scope changes, we are requesting a change order addition to our current contract and increasing the
“Not to Exceed” contract amount by $ 252,395.61. Documentation of the derivation of this fee is attached. All
salaries and rates used to establish the increase are in conformance with the contract.
Sub consultants will be used for 22.09% for this contract modification.
DBE Participation in this contract modification is 0%. The contract DBE goal is 4.82%. The updated composite
project DBE participation inclusive of Modifications 1 thru 10 is 5.34% which is conformance with the stipulated
contract DBE goal. The proposed scope changes do not include elements of work impacting the work of our DBE
consultant.

Basis for Contract Modification
This change order package is for additions, revisions, and updates associated with the design
of roadway, structures, and drainage work elements for Northern Parkway — Dysart Road to
111% Avenue. The original design services are specified in the Design Services Project No.
TT347, Northern Parkway: Dysart Road to 111'" Avenue dated May 7, 2014.
General:
Item 1. Project Management and Coordination: Stanley will perform additional project
coordination and project management services for an additional six (6) months resulting from
extension of the design schedule.
Stanley will attend coordination meetings with MCDOT as requested to support resolutions of
R/W or other development coordination of unforeseen items including Marbella drainage,
Premier RV access, and CMEX / Inert landfill access discussions.
Stanley will attend six (6) additional monthly progress meetings resulting from the extended
design schedule and prepare meeting notes per the requirements of the original contract. The
original contract end date was set at January 18, 2015 which was extended to September 15,
2015 by zero dollar Change Order no. 2 —and extended to May 23, 2017 by zero dollar
Change order no. 8. Additional contract time extension will be needed to address completion
of the project.

Item 2. Quantity Take-Offs: Stanley will perform quantity take-offs on all additional scope of
work items in this contract modification per the requirements of the original project contract.
Item 3. QA/QC: Stanley will perform QA/QC on additional scope of work items in this scope
modification per the requirements of the project QA/QC plans.
Roadway:
Item 1. Roadway Modeling:
Update roadway modeling for the addition of Dysart Road retaining walls
Update earthwork calculations and report for adding mainline embankment to full height at
Dysart Road approaches
Item 2. Plan Revisions:
Update roadway typical sections for addition of Dysart Road retaining walls and final grading in
ramp / wall infields.
Update topo base file for partial basin excavation and soil stockpile from El Mirage storm drain
construction.
Item 3. Waterline Extensions:
Prepare waterline extension plans from El Mirage to Dysart to serve landscaping services
north of the Northern Avenue section line with El Mirage water system.
Drainage:
A preliminary drainage memo and box culvert weir analysis was performed to establish the
basis of modifications to the 127th Ave box culvert and Hanson Channel offsite drainage
system. Drainage work hours were estimated for drainage modifications to the Northern
Parkway Project - Dysart Road to 111th Avenue. Proposed modifications include alternative
analysis for realignment and resizing of the 127th Ave box culvert and inclusion of a weir
outfall for the reconfigured 127th Ave box culvert. The new weir outfall will be designed
between Dysart Road and 127th Ave to match the existing roadway weir overflow condition in
existing Northern Parkway. Modifications to the onsite storm drain systems include separating
the onsite and offsite flows with the inclusion of storm drain trunk line(s) that will outfall to the
El Mirage Basin. First Flush for these roadway improvements will be relocated and treated in
the El Mirage Basin.
The following work descriptions detail the required drainage design efforts required for the
proposed modifications to the drainage plans, report and estimate. These work efforts include
updating the Pre-Final Plans for a 95% and 100% submittal of the modifications.

Item 1: Alternatives Analysis - 127th Avenue RCBC and Hanson Channel:
e Preliminary Box Culvert and Weir Analysis (CulvertMaster, Existing Condition Analysis):
Work efforts to develop the existing condition flow patterns during a 100-year return
event using a culvert and weir analysis approach. Effort also include alternative analysis
for downsizing and reconfiguring the 127th Avenue box culvert.
e Drainage Memo and Exhibit for Weir Alternative: Work efforts to develop a Preliminary
Drainage Memo and Exhibit to present the findings of the analysis.
e Alternative Storm Drain Analysis (Trunk Line(s) - Dysart to El Mirage): Work efforts
needed to develop the proposed hydraulic analysis and layout for the storm drain trunk
line system needed to separate onsite and offsite flows.
e First Flush Treatment Alternatives (El Mirage Basin): Efforts needed to develop an
alternative for the First Flush Treatment of onsite flows at the El Mirage Basin.
Item 2: Final Design Analysis - 127th Avenue RCBC and Hanson Channel:
e Final Weir Hydraulic Analysis (HEC-RAS Existing Condition): Efforts needed to develop
the existing condition for 100-year flows crossing Northern Parkway between Dysart
Roan and 127th Avenue.
e Final Weir Hydraulic Analysis (HEC-RAS Proposed Condition): Efforts needed to
develop the proposed condition for 100-year flows crossing Northern Parkway between
Dysart Road and 127th Avenue.
e Storm Drain Hydraulic Analysis (StormCAD, Proposed Condition): Efforts needed for
the hydraulic analysis of the preferred alternative storm drain configuration between
Dysart Road and El Mirage Road.
Item 3: Drainage Modifications - Dysart Road to El Mirage Road:
e Summary of Estimated Drainage Quantities (2 Sheets): Work hours to update the
drainage quantities for changes to the storm drain system, box culvert and proposed
weir between Dysart Road and 127th Avenue.

e Street Transportation Storm Drain Design Summary Sheet (1 Sheet): Work hours
needed to update the storm drain parameters on the design summary sheet.
¢ Storm Drain Profiles (6 Sheets, 30 Pipe Profiles): Work hours estimated to prepare the
updated storm drain profiles between Dysart Road and 127th Avenue.
e Box Culvert (127th Ave) Plan and Profile (1 Sheet): Effort needed to update the box
culvert plan and profile for the realigned and reconfigured box culvert at 127th Avenue.
Results from the alternative analysis will be used to potentially reduce the number of
cells in the box.
e Weir Detail (Dysart Road to 127th Avenue) (1 Sheet): Work hours needed to develop a
concrete and riprap weir detail that will be integral to the south edge of the Dysart Road
Southeast First Flush Basin.
e Drainage Plan (6 Sheets): Work hours estimated to update the drainage plan sheets for
the new storm drain system, box culvert, weir analysis and first flush treatment.
e Update Final Drainage Report: Efforts needed to update the Final Drainage Report for
the modified storm drain system, box culvert, weir analysis and first flush treatment.
Structures:
Item 1 Agua Fria River Bridge Design - Modifications:
Revise the current bridge design and construction plans to construct the bridge with six lanes
(3 + 3 with shoulders) and an open median.
The substructure and foundations shall remain as currently designed for the ultimate bridge
width.
The revised design shall consider the future expansion of the bridge to close the median and
elements needed to facilitate ease of median closure will be included in the updated design.
The future widening will require removal of interior barriers; placement of additional girders,
construction of diaphragms, widened deck and a new median barrier. Lighting will be moved
to the outside of the bridge to eliminate need for future replacement.
item 2 Wall Design - Plans - Dysart:
Add retaining wall plans at the Dysart Road approaches to eliminate/minimize future disruption
of interim traffic flow on the adjacent roadways in following phases of construction.

Wall plans will include General Notes, key plan and quantities, wall location plan, plan sheets,
wall elevations, sections and details, initial concept development for future Dysart Road Bridge
to ensure compatibility with the walls, quantities, estimate and special provisions.
Traffic Signals
Change traffic signals at Dysart Road and El Mirage Road to MCDOT equipment and design
standards and revise plans to MCDOT signal formats. Revise signal specifications to MCDOT
standards.

Contract Modification (CM) No. 2 - Revisions to Final Design Tasks

SCOPE OF SERVICES:
The TT347 project consists of advancing the conceptual plans documented in the August 14, 2013
Final Scoping Plans prepared by Parsons Transportation Group to the final design stage, providing
bid ready sealed documents. The project limits are along Northern Parkway from Dysart Road to
111% Avenue. The work to be performed by PB under this CM is documented in the Work Tasks
described below. PB will be a sub-consultant to Stanley Group (Prime).
On February 10, 2015, Stanley Group (Stanley) requested that Parsons Brinckerhoff (PB) prepare
CM 1 for revisions to the design shown on the Pre-Final submittal (October 2014). These changes
were based on direction provided by MCDOT. The CM 1 was executed, however, MCDOT put the
project on hold.
On February 13, 2017, a design meeting was held with Stanley to discuss the status of the project
and Stanley provided an update on the design modifications to be implemented on the project. This
CM incorporates requested changes that were not included with CM 1.
WORK TASKS:
Task 1A: Revisions to El Mirage Rd
e Revise El Mirage Rd design to assume MCDOT’s El Mirage Rd Project (connecting to the
north leg of El Mirage Rd being constructed with this project) will be completed at the time
this project is under construction.
e Revise the on-site drainage system along El Mirage Rd to connect to the drainage system
that is currently being built by MCDOT. The system that is currently under construction will
be shown as an existing drainage system on PBs plans.
The following shall be incorporated into the design:
Revise typical sections
Revise connection of El Mirage Rd at north leg to match El Mirage Rd project
Revise El Mirage Rd plan sheets
Revise El Mirage Rd curb and gutter profile sheets

Revise driveway entrance to Premier parcel and modify median configuration
Revise drainage sheets as needed to connect to the drain system built by MCDOT

Task 1B: Revisions to Northern Parkway from New Aqua Fria Structure to Existing Roadway
at 111" Ave.
e Revise connection from Agua Fria Bridge to Northern Parkway based on new bridge
configuration (by Stanley)
e There will be a total of 2 deceleration and acceleration lanes along the Northern Parkway with
driveway connections (Stanley to provide exact locations)
The following sheets will be updated to incorporate new design at 115" Ave and at 111" Ave:
=> Design, model and add deceleration and acceleration lanes at above said locations.
=> Update the Typical Section Sheets
= Update Roadway Plan Sheets for new alignment
= Create new driveway plan and profile sheets
= Update Roadway Profile Sheets
= Update Sign Summary Sheets
=> Update drainage as needed for the new alignment
=> 2 new Driveway Plan and Profile Sheets

Task 2: Additional Miscellaneous Revisions
The following additional design items shall be incorporated in the design:
e Update north leg of Dysart Rd to connect to a 4-lane ultimate section.
e Update all Removals Sheets to reflect new conditions and removals that have taken place.
Stanley will provide PB with the necessary changes to be incorporated.
e Update MOT and construction phasing for entire project.
e One additional plan submittal is anticipated that was not previously scoped (Pre-Final, Final
and Sealed to Pre-Final, 95%, 100% and Sealed). Added hours for attendance of meeting,
review comments and incorporation of comments in the bid documents.

Task 3: Project Coordination
Due to delays in the completion of the project (original date was February 2015), it is assumed the
project schedule will be finalized in September 2017. It is assumed that 1 member of PB will attend
4 additional progress meetings at the County and 1 additional meeting at Stanley to discuss design
and progress efforts.
