1. The proposed program or project, and whether or not it is a new or continuing project.
Arizona YWCA Metropolitan Phoenix requests $25,000 for our Valley West Senior Center that provides
congregate meals, social activities, companionship, and transportation to 1,150 low-income senior and
disabled adults in the Glendale area. YWCA coordinates services among four locations to provide
nutritionally balanced hot meals at noon, educational opportunities, self-improvement activities, health
screenings, health fairs, activities, and outings to engage and support clients. YWCA delivers meals and
companionship to clients who are homebound and provides other transportation as needed. Home meal
delivery also serves as a social visit and wellness check. These visits often loneliness, isolation, and
mental health status checks. The Centers also provide pet food for client animal companions through our
We Feed Four Feet program. All YWCA program staff are trained and undergo background checks.
Program Goal:
1) Seniors and/or disabled adults eat at least one nutritious meal each week day.
Objectives:
1) Provide 33,000 congregate meals to seniors and disabled adults in the Glendale area at one of four
multi-cultural centers.
2) Deliver 60,000 hot, nutritious meals to homes of seniors and disabled adults unable to visit one of our
Centers.
Activities:
1) Serve one hot, nutritious meal each weekday at one of three senior center locations and once a month at
an additional senior center.
2) Registered dietitian approves all meal menus.
3) Provide transportation to and from each senior center location for congregate lunch participants.
4) Provide meal delivery for homebound participants.
5) Schedule Health and Wellness presentations at two centers twice a month.
6) Provide socialization activities, special events, community resources, supplemental food programs,
health fairs, and health screenings to address loneliness and isolation.
The YWCA is proud of our program accomplishments in 2015-16. We served 612 unduplicated seniors
through our congregate meals program, 588 unduplicated seniors through the YWCA Meals on Wheels
program and 70 unduplicated seniors received rides through YWCA assistance to the "Dial-A-Ride"
program. We provided over 10,870 rides to seniors and disabled. Each weekday, nearly 1,200 seniors
receive a nutritious, hot meal through our services. Annually the YWCA serves over 105,000 meals to

seniors and disabled. The YWCA also provides over 18,000 programs and activities to seniors and
disabled.
2. The purpose of the program or project and how it meets one or more of Fort McDowell’s funding
priorities (i.e., education, public safety, health, environment, and economic and community
development)
This program serves the public by providing nutritious hot meals, disease prevention, wellness activities,
recreational activities that promote physical fitness, nutrition in seniors and disabled adults.
3. The need that the program or project aims to fulfill, the population targeted, and the estimated
number of people who will be served by the grant.
Nearly 16% of Arizona’s seniors face the threat of hunger. In Glendale, 5% of seniors live alone, 7% have
an annual income of less than $10k, and 28% earn less than $20k. Food insecure seniors face increased
risk for chronic health conditions including being more likely to experience depression; report a heart
attack; develop asthma; and experience congestive heart failure. It is proven that elderly nutrition
programs prevent or delay placement in nursing homes and help people manage chronic conditions, thus
avoiding costly hospital and care center stays. The target population is low-income seniors and disabled
adults in Glendale AZ and surrounding communities. Key findings include that 57% of participants report
their meal from YWCA as their main meal and 47% report as their only meal of the day.
This program will serve approximately 1,150 low-income disabled adults and seniors in Glendale aged 55
years and older.
4. The partners in the program or project, and other entities providing funding and support (please
indicate whether the organization will be submitting applications to any other tribes).
The YWCA collaborates and shares resource with several agencies in the west Valley with a goal to
eliminate elderly hunger, and the only organization that delivers meals to the elderly in the West Valley.
The Valley West Senior Center coordinates meal service and delivery among three other multi-cultural
centers including Glendale Community Center, Glendale Adult Center, and the Japanese Senior Center.
The City of Glendale provides recreation and socialization activities. St. Mary’s Food Bank Alliance
shares maintenance food and supply items, and stocks our food pantry with take home items. Gleaners
Brown Bag Program supplies free food products out of the Valley West Senior Center weekly and Waste
Not provides excess food products. The YWCA Center also collaborates with the Area Agency on Aging
to develop, coordinate, and deliver services to seniors. For more than 10 years, the YWCA Center also
provides free use of our facility and multi-passenger van during off hours to the DEFAP and Fiesta
Mexicana Dance Company, nonprofit whose mission is to reduce teenage drug abuse. YWCA will not
submit an application to other tribes at this time.
5. Timetable for implementation of your program or project. This is a year round program with a
fiscal year ending on June 30th.
