Fort McDowell Yavapai Nation Total grant amount request 10000 1 / 1 555 West Glendale Avenue Phoenix, Arizona 85021 Mrs. Selena Castaneda, Fort McDowell Yavapai Nation Tribal Council Secretary Prop202@ftmcdowell.org March 15, 2017 Dear Ms. Castaneda: On behalf of Duet: Partners in Health & Aging, thank you for the opportunity to apply for Proposal 202 funding. We are grateful for the work you do for the community. We request $10,000 in funding for Care for the Caregiver, which impacts the lives of hundreds of elders each year. Duet’s mission is to promote health and well-being through a broad range of community-based services for homebound adults, family caregivers, faith communities, and grandparents raising grandchildren. Applicant Information: Name and contact information for government entity/organization applying for grant funds: Maricopa County Board of Supervisors Melody Henderson, Maricopa County Government Relations, 602-506-2798, mhenders@mail.maricopa.gov Signature:_______________________________________________ Melody Henderson Primary Contact: Name, address, e-mail and telephone numbers for the primary point of contact: Duet: Partners In Health & Aging, 555 W Glendale Ave, Phoenix, AZ 85021, Elizabeth Banta, Executive Director, ebanta@duetaz.org 602 275 5022 x 11. 1.The proposed program or project, and whether it is a new or continuing project: The project is an expansion of the existing caregiver services at Duet. A caregiver is anyone who provides unpaid care for an aging loved one. Many family members and friends do not consider such care “caregiving” — they are just doing what comes naturally. But that care may be required for months or years, and can take a spiritual, physical and financial toll. Duet provides emotional and practical help through support groups, respite, workshops, retreats, guidance, and referral information. Duet requests funding from Ft. McDowell to increase awareness of Duet’s caregiver services and inspire more caregivers to ask for help. Our community is facing a crisis of care. Family caregivers are predeceasing their loved ones at a rate of 30%i. Each day in Arizona, approximately 800,000 family members provide unpaid care for loved ones with significant disabling conditions, including Alzheimer’s disease (in 2015, Alzheimer's rose to become the number four leading cause of death in Arizonaii), Parkinson’s disease, and stroke. They represent the backbone of Arizona’s long-term care system, providing care worth more than $9 billion annuallyiii. Twenty percent of caregiving is provided to people 80 years and older by people of the same ageiv. This project provides double impact by affecting the health of both caregiver and aging loved one. Growing services requires growing community awareness. Duet will increase awareness of caregiver services through coordination of outreach, website redesign, broad placement of multiple sets of “I Am a Caregiver” traveling exhibit featuring professionally photographed images of family caregivers, their stories, and resources for help. The exhibit will travel to settings including faith communities, medical centers, local government and Executive Director: Elizabeth Banta Phone: (602) 274-5022 Email: info@duetaz.org Web: www.duetaz.org business offices, and cultural venues. Duet has completed the production of video and photographic support for the exhibit. In addition, your support will help Duet put into wide use our new video series based on the book “Loving Someone Who Has Dementia” by Dr. Pauline Boss. The series will be available at no charge for use in small groups. In 2016 Duet served 1,813 caregivers through support groups, information calls, trainings, our caregiver symposium, and retreats. We provided outreach through 21 presentations on caregiving, launched a caregiver- to-caregiver mentoring program, produced powerful outreach videos featuring local caregivers on their heroic journey, and impactful photos featuring caregivers who benefit from our service, together with the loved ones in their care. Sets of our “I Am a Caregiver” traveling exhibits will travel to a range of settings as ongoing resource to help people self-identify as caregivers, and thus have a greater likelihood of reaching out for help. Duet’s Caregiver Services Program Coordinator has carried out outreach activities at 14 public/community gatherings so far in 2016 to increase the participation rate of ethnically and culturally diverse family caregivers. The part-time public relations coordinator hired in 2016 has already generated 16 media mentions and articles on Duet’s caregiver services. 2. The purpose of the program or project and how it meets one or more of Fort McDowell’s funding priorities: Aligning with the priority of health, the project supports Duet’s strategic plan which calls for an increase by a minimum of 20% annually of Duet’s existing caregiver services. Caregiver Services strategic Goal # 1 is to increase the number of caregivers attending Duet support groups; caregiver personalized information and guidance assistance; attendance at caregiver workshops and educational programs; and the number of unduplicated caregivers served, including an increase in the participation rate of ethnically and culturally diverse family caregivers by a minimum of 25% in three years. The vast majority of older adult caregivers are not accessing support services. Statistics show that more than 90% of family caregivers become more proactive about seeking resources and skills they need after they have self-identified as caregivers. 3.The need that the project aims to fulfill, the population targeted, and the estimated number of people who will be served: Research shows that older caregivers generally face the highest risks to well-being of all caregiver groups. Unsupported, the stress of caregiving quite literally puts their lives at risk. Caregivers age 75 or older are four times more likely to be caring for a spouse or partner, are typically the sole care provider, and usually live with the care recipient. On average, they have provided care for 5 ½ years. These caregivers often report that they had “no choice” in taking on the caregiving role; of these, 53% report high levels of caregiver stress. Older adult caregivers typically average 62 hours per week of care and have a “high burden” of care. The typical caregiver served by Duet is a white female over age 65 caring for her husband who is likely suffering from dementia. According to the Marketplace Morning Report, November 11, 2015, “Latinos are the fastest growing group of people 65 and older living in the U.S. today. And the number of elderly Latinos with dementia is growing too. Already, there aren’t enough bilingual, bicultural services to go around. That means increasingly, Latinos are going to have to leave work and other responsibilities to care for ailing family members.” Duet is targeting Spanish-speaking, LGBTQ, and other minority populations through its outreach efforts. Total unduplicated individuals to be served in the 12-month granting period: 626. 4. The partners in the program or project, and other entities providing funding and support: Program Partners: ASU’s School of Social Work and College of Nursing and Health Innovation, The Alzheimer’s Association Desert Southwest Chapter, Muhammad Ali Parkinson Center, Church of the Beatitudes. Program Funders: Area Agency on Aging, Region One, The Virginia G Piper Charitable Trust, USAA Foundation. A proposal will be submitted to Salt River Pima Maricopa Indian Community for support of the complete range of Duet’s services including help for homebound adults, and grandparents raising grandchildren. 5. The timetable for implementation of the program or project: Upon announcement of the award in August, Ft. McDowell Yavapai Nation funds will be allocated to the costs of outreach and publicity as detailed on the full project budget attached.
