Narrative: A narrative of no more than two (2) pages describing the following:
1. The proposed program or project, and whether it is a new or continuing project;
HAWP 102 - SCHOOL EMPLOYEE WELLNESS PROJECT: This is a continuing project which provides
training to business sector employers on comprehensive, evidence-based wellness initiatives. The
Project will expand the Healthy Arizona Worksite Program into the school sector. If granted, the
funding will allow for broadening the reach of our on-going Healthy Arizona Worksite Employee
Wellness Program training (HAWP 101) to include school employee wellness training (HAWP 102).
A school employee wellness training curriculum has been developed. The project will fund the
delivery of the trainings and a summit to increase school personnel’s capacity and will focus on
building sustainability into the initiative.
The Health Arizona Worksite Program curriculum uses the Center for Disease Control’s Workplace
Health Model which is a comprehensive approach with interventions that address multiple risk
factors and health conditions at the same time. The model recognizes that the interventions and
strategies chosen influence each employee and the organization as a whole. In addition the HAWP
102 program utilizes the CDC’s School Employee Wellness: A Guide to Protecting the Assets of Our
Nation's Schools as a resource and model for best practice for school staff wellness.
2. The purpose of the program or project and how it meets one or more of Fort McDowell’s funding
priorities (i.e., education, public safety, health, environment, and economic and community
development);
Evaluation data indicated there was a need to design wellness program training specifically for
school employees. An RFP was released and the HAWP 102 training curriculum was designed
recently to meet the specific needs of the school employee wellness programs. The training is two (2)
hours and includes three (3) modules; Making the business case, Assessment/Planning and finally,
Evaluation. The school employee wellness training is aimed at reducing the incidence of chronic
disease, obesity, and tobacco use among school employees in Arizona. In addition, the funding will
support a school employee wellness summit focused on supporting school staff capacity to
implement and sustain School Health Policies and comprehensive, evidence-based school employee
wellness programming. The HAWP 102 School Employee Worksite Wellness Project has a focus on
education, health and economic and community development.
3. The need that the program or project aims to fulfill, the population targeted, and the estimated
number of people who will be served by the grant;
Healthy communities are vital to the sustained growth of organizations, leading to a more
competitive economy. Cultivating a culture of health and wellness within the school community
leads to a vibrant, healthy school workforce where adults model healthy behavior choices to
students. Attracting and retaining talent within schools requires more than high-wage jobs, it is
driven by quality of life and a supportive culture of wellness. HAWP 102 provides the evidence-
based training to make it happen. Communities throughout the country are receiving national
media attention for cultivating wellness programs. The 2017 Fort McDowell Yavapai Nation Prop
202 funding can make an impact toward improving the work environment and wellness culture for
Arizona’s School employees. The HAWP 102 School Employee Wellness Training project aims to
train 125 individual school employee wellness champions on comprehensive evidence-based
2

wellness programming. The champions will be expected to then implement wellness programs within
the schools where they are employed. A School Health Wellness Summit will be provided to 150
school employees and partners to support building school employees’ capacity and sustain School
Health Policy and worksite wellness programming.
Maricopa County Department of Public Health’s (MCDPH) mission is to protect and promote the
health and well-being of Maricopa County residents and visitors. Through program operations,
community health transformation, child and family health promotion, disease control, clinical
services, and public health policy, the Department of Public Health strives to keep Maricopa County
a healthy and safe community. The HAWP School Employee Worksite Wellness Project aligns with
both the MCDPH’s mission and the focus of Fort McDowell Yavapai Nation Prop 202 funding
intention, which is funding “projects that focus on education, public safety, health, environment, and
economic and community development.” This is an exciting collaboration opportunity to leverage
resources, work across sectors to improve the educational system, the economic wellbeing of our
community and health outcomes for our citizens.
4. The partners in the program or project, and other entities providing funding and support (please
indicate whether the organization will be submitting applications to any other tribes);
MCDPH Office of Worksite Health Innovation partners with the MCDPH Office of School Health
and Wellness Initiatives (OSHWI) staff as well as other internal staff, state agencies, and the
Greater Phoenix Chamber of Commerce Foundation to broaden the reach of worksite wellness
initiatives. In kind contributions are made in providing support, expertise, and shared resources
to improve school health and wellness initiatives.
