The Maricopa County Department of Public Health (MCDPH) serves the third largest local public health jurisdiction in the U.S., with more than 4.1 million residents according to the 2015 census. Maricopa County encompasses over 9,200 square miles, roughly the size of the state of Massachusetts, composed of a mix of urban, suburban, and rural areas including the whole or parts of five sovereign American Indian Reservations. Home to the major metropolitan cities of Phoenix, Scottsdale, Mesa, Glendale, and Tempe, Maricopa County serves as the state’s major geopolitical and economic center. Our mission is to protect and promote the health and well-being of all of our residents and visitors. This means providing protection from the health threats such as food-borne illnesses, natural and man-made disasters, toxic exposures, and preventable injury. Public Health also works to prevent chronic diseases, such as heart disease, cancer, and diabetes by addressing their risk factors: poor nutrition, inadequate physical activity, and tobacco use. We are increasingly moving towards policy, systems and environmental approaches to improving the health of our community. As a partner of the Arizona Department of Health Services we have a long history of implementing innovative and effective health programs and initiatives within Maricopa County. 2. The purpose of the Healthy Maricopa Through Healthy Living program is to implement and expand self-management programs to populations who have a need for Chronic Disease education with the combined goal of improving health, increasing the self-efficacy of completers, reducing costs associated with healthcare and reducing hospital readmissions within Maricopa County. 3. The Stanford Model of evidence-based self-management programs, also known as Healthy Living workshops, is a proven intervention that help Arizonans lead longer and healthier lives. Unfortunately, many do not know about this opportunity within our community. The Healthy Maricopa Through Healthy Living program will help bridge the gap to access to self-management workshops by empowering community partners to deliver this evidence-based program within Maricopa County. Goal #1: Train 30 lay-leaders to facilitate Healthy Living workshops within the grant year Goal #2: Create partnerships that result in the planning and facilitation of 30 Healthy Living workshops within the grant year (self-management workshops can be provided in both English and Spanish). Goal #3: Provide technical assistance for workshop implementation that includes activities related to workshop logistics and support including workshops books and nutrition support. The workshops are promoted to people who have a chronic disease, are at risk for a chronic disease, or are the caregiver for a person with chronic disease. Implementation of 30 workshops will provide approximately 360 individuals with access to an evidence based program that improves overall health while at the same time reducing healthcare costs and hospital readmissions. 2 4. The Arizona Living Well Institute (AZLWI) is the state endorsed vendor of self-management workshop implementation. They provide lay-leader training and workshop fidelity. MCDPH works closely with this organization to ensure participants have a positive experience while engaging in the program. 5. Work will begin immediately following grant funding with the creation of partnerships and lay- leader training activities to support workshop volume that is projected. Step 1: identify and train 6 lay-leaders in the first quarter and 8 lay-leaders in each of the following 3 quarters in collaboration with community and healthcare partners Step 2: plan and implement 6 Healthy Living Workshops in the first quarter and 8 Healthy Living Workshops in each of the following 3 quarters in collaboration with community and healthcare partners 3 Signature: ____________________________________________ Melody Henderson Date Maricopa County Government Relations 301 W Jefferson, 10th Floor Phoenix, AZ 85003 ATTACHMENT: Healthy Maricopa through Healthy Living Budget Proposed Budget Expense Amount requested Lay-leader Training $7,500 Workshop Books $12,000 Workshop Coordination Stipend $15,000 Nutrition Support (Fresh Produce) $6,000 Total $40,500 4
