Indian School Road, Suite 203 Scottsdale, AZ 85251 480-397-3550 www.Opportunity4Kids.org Primary Contact: Alexandra Munter, Director of Operations Alexandra@Opportunity4Kids.org 480-340-7017 Proposed Program: Out-Of-School Time Community Outreach Opportunity4Kids' goal is to elevate children's health and overall wellness to the highest-level possible by addressing the underprivileged youth in Maricopa County through an Out-of-School Time Community Outreach Program. This proposed program will target in-need, at-risk children ages 5 to 18 years old, who are enrolled students in the Maricopa County school system. We are seeking supplemental financial support to expand our outreach through this much-needed program. Last year, we served over 1,000 Arizona youth by providing resources to increase participation in out-of-school time activities; a dramatic increase in positive community impact from 2015-2016. Purpose of Program The purpose of the Out-of-School Time Community Outreach Program is to encourage the healthy behavior of children residing in Maricopa County by providing financial support through Youth Recreation Scholarships in the areas of Health and Education; more specifically by making recreational activities, that promote physical fitness and wellness, accessible during out-of-school time. Need and Target Population Per the US Census Bureau’s 2015 American Community Survey, in Maricopa County there were 1,014,531 children under the age of 18 years old in households and 753,436 enrolled in school. The percentage of children living in households at or below the poverty line is 24.4% and it was determined that 28.7% of children are living in households with supplemental security income, cash public assistance income or receiving food stamp benefits. Opportunity4Kids will provide financial assistance to children ages 5-18 years old, who are students enrolled in the Maricopa County school system and fall within 1.5% of the federal poverty guidelines. With this program, we will serve a minimum of 65 unique youth that would otherwise not be involved in out-of-school time activities due to their financial circumstances. Collaborations Opportunity4Kids has established a supportive network of partners throughout Maricopa County by building relationships with recreational organizations that provide influential out-of-school time activities such as: the Boys and Girls Clubs of Metropolitan Phoenix, Skate After School, The Valley of the Sun YMCA, ATA Martial Arts, Phoenix Center for the Arts, Scottsdale Soccer, San Marcos Music Academy, ProCamps, Ballet Arizona, The Hubbard Family Swim School, The Little Gym and many more. These organizations support Opportunity4Kids’ goals of providing financial resources by discounting or reducing registration fees required to participate, facilitating the Scholarship of more children. We are seeking this award as a supplemental funding grant. Our goal is to someday provide this type of assistance to all Maricopa County youth in need. To attain this goal, Opportunity4Kids conducts local business outreach and ongoing fundraising efforts; culminating with our annual VIP Dinner, most recently raising $30,000. Respectfully, we will be submitting to other tribes for Tribal Gaming Grant opportunities. Timeline Beginning September 1, 2017 through August 31, 2018, the Out-of-School Time Community Outreach Program will be implemented by Opportunity4Kids’ Director of Operations, with the oversight of a volunteer Board of Directors. The Director of Operations is the only paid personnel. This expense is completely funded by other sources. We are not requesting funds from this grant to pay for personnel. Opportunity4Kids has established specific standards to be implemented in the proposed Program. It is through word-of-mouth that Opportunity4Kids will solicit underprivileged children to provide out-of-school time Youth Recreation Scholarships. The Parent/Guardian/Case Worker of qualified children will be required to complete an online application, including a mandatory narrative from the referring Parent/Guardian/Case Worker on the behalf of the child’s mental/physical/emotional state. The application will be vetted by the Director of Operations and then reviewed and processed by a designated Scholarship Approval Committee. At minimum, 65 Youth Recreation Scholarships will be issued on a case-by-case timeline throughout the grant period of 2017-2018. The application process is utilized to identify the true need of youth applicants by requesting financial and health information to determine that those awarded are the most in need. Opportunity4Kids will track household location of youth who receive Scholarship to pinpoint areas of higher need for future targeting purposes. In conclusion, the Parent/Guardian/Case Worker will complete a post survey to determine individual progress and perceived development. Throughout the Scholarship process, Opportunity4Kids’ Director of Operations will utilize a tracking system to record information on children and their families applying for Scholarship. Information gathered will include: school enrollment, age of participating children, participation in government financial assistance programs, family financial situation, disabilities and individual stories. The expectation is that 100% of children provided Scholarship will fall within 1.5% of the federal poverty line and at least 85% of those will also receive additional government assistance. Furthermore, 90% of those who receive Scholarship will show an increase in self-esteem and social skills due to their participation in out-of-school time programming. A final report will be produced by the Director of Operations on the success of the Out- of-School Time Community Outreach Program. We look forward to recognizing the dramatic results that will be generated by this program! March 20, 2017 Mrs. Selena Castaneda Tribal Secretary Fort McDowell Yavapai Nation Tribal Council P.O.
