Fort McDowell Yavapai Nation Total grant amount request 50,000 1 / 1 Because Every Child Deserves to See NARRATIVE--VisionQuest 20/20: Promoting the Health and Educational Success of Arizona Children Through Sustainable Vision Screening 1. The proposed program or project, and whether it is a new or continuing project: Founded in 2003, VisionQuest 20/20 (VQ2020) is an Arizona nonprofit organization dedicated to the use of technology for the early detection of vision disorders and preventable blindness in children to promote educational success, vision health and quality of life. Vision health is the 4th leading public health crisis for children in America today. VQ 20/20 is addressing this crisis by bringing modern, cost effective, user-friendly vision screening technology to thousands of Arizona schoolchildren in grades K- 12 with use of the EyeSpy 20/20 computerized “video game.” Since its inception, VQ 20/20 has vision screened more than 350,000 Arizona children in over 219 Arizona schools and community health programs. In 2016, we vision screened more than 84,000 Arizona children. The proposed project will build upon VQ 20/20’s success for Arizona children, taking our efforts to a new level by: 1. Reaching 1000's more children with vision screening through public events, health fairs, and public libraries. At least 1,000 children will be vision screened in 2017 through this outreach. 2. Continuing to implement the program in schools in need, reaching 20 new schools this year, 10 of which would be supported with the Ft. McDowell Yavapai Nation's funding and receive 5 year licenses. In just one year, 2,500 children would be vision screened- reaching 12,500+ children over 5 years. 3. Closing the Loop with Title 1 schools where EyeSpy 20/20 has been implemented to provide vouchers for professional eye examinations and eyeglasses for children whose families do not have insurance or the means to pay for corrective lenses. At least 100 children will receive this expanded service. Funding of $50,000 from the Fort McDowell Yavapai Nation will be specifically targeted to implement EyeSpy 20/20 in 10 schools in medically underserved Arizona communities and to conduct vision screening at 3-5 public events. Significant partnerships and relationships are already in place to expand and enhance our reach and our impact. (See 4 below) The Ft. McDowell Yavapai Nation would receive recognition at community events as a champion for children's vision as well as would also recognition on every vision report sent home to parents for every child at the 10 schools as the sponsor of the program. 2. The purpose of the program and how it meets one or more of Fort McDowell’s funding priorities: Vision health is fundamental to all aspects of a child’s health, quality of life and educational success. In fact, 80% of learning occurs visually, children unable to see clearly in school often suffer academically. Although less obvious, many children diagnosed with attention related disorders have undiagnosed vision problems, resulting in difficulty concentrating, slow reading, and procrastination – all of which significantly hinder learning. VQ 20/20's vision screening and expansion goals are in alignment with the Fort McDowell Yavapai Nation’s commitment to support the overall health and educational needs of children and lifelong success. 3. The need that the program aims to fulfill, the population targeted, and the estimated number of people who will be served by the grant: Childhood vision screening is directly related to preventive healthcare and a child’s education and academic success. In the United States alone, every 6.5 minutes a child is diagnosed with irreversible vision loss that could have been prevented with early detection and treatment. The Vision Council of America estimates that 25% of children suffer from vision problems that could have been addressed or eliminated if appropriate screening and follow-up had been in place upon entry to school. Additionally, VisionQuest 20/20, 4113 E. Pinchot Ave., Phoenix, AZ 85018 1-888-MY-VQ2020 VisionQuest2020.org EIN 04-3747651 Because Every Child Deserves to See low-income children and children experiencing problems in school are disproportionately affected by vision problems. (Healthier Students Are Better Learners, 2010) Arizona children are at particular risk because Arizona is 1 of 8 states in the nation that does not require vision screening in schools. This leaves a significant health care gap for what is now the 4th leading health crisis afflicting children. VQ 20/20 is the only Arizona nonprofit that is systematically addressing this public health gap for children in Arizona public schools and low income communities. Our target population are children in Kindergarten through 12th grade. Support from the Fort McDowell Yavapai Nation will be utilized throughout Arizona to vision screen underserved children through expansion to 10 new schools and by expanded activities at public libraries, public health fairs and community events. With the Tribes support, in 2017-18, approximately 2,500 new K-12 students will be screened in 10 new schools (approx.250 children per school) and at least 200 children will be vision screened at new community outreach events. 4. The partners in the program or project, and other entities providing funding and support (please indicate whether the organization will be submitting applications to any other tribes): Significant partnerships have been developed over the years, allowing us to take our efforts to the next level. 1. The Phoenix Public Libraries have provided venue space for vision screenings at their libraries. They are eager to take our partnership to a new level, offering even more screenings to children and will publicize this program on their website. 2. Yuma Regional Medical Center - Will partner to provide vision screenings to children participating in the Healthy Kids Program. 3. The Parsons Center for Pediatric Dentistry and Orthodontics – Will partner to provide venue space for vision screenings at the Parsons Center’s Community Health Fairs. 4. Grand Canyon University – Will partner to provide venue space for vision screenings at the Canyon Cares/Arvizu NINOS Back-to-School Back-pack, Health and Safety Fair for underserved children. 5. The Phoenix Chapter of the Delta Gamma Organization will provide dozens of volunteers to perform vision screenings at schools, public libraries and community events. The Delta Gamma's charitable focus has been children's vision for more than 60 years. 6. Global Source Rx (GSRX) will provide eyeglasses at a significantly discounted cost, allowing VQ2020 to provide free eyeglass vouchers to children in need. VQ2020 has submitted a request to the Gila River Indian Community for $75,000 in 2017. Last year, the Gila River Indian Community provided a $75,000 grant to expand into new schools in Maricopa County. 5. Timetable for implementation of the program or project: October 2017–June 2018: Screen underserved children at public events, health fairs and public libraries using EyeSpy 20/20; Implement EyeSpy 20/20 into 10 additional Arizona schools. June-August 2018: Evaluate progress through data analysis and surveys and set goals for the 2018 school year; Continue screening of underserved children at public events, health fairs and in public libraries.
