Narrative
Please structure your proposal to provide the following information in the order indicated. Provide the narrative in
paragraph form in the text field provided. Please be thorough but strive for brevity.
1. Briefly describe the organization’s history, mission and goals.
The mission of the Civitan Foundation is to provide our community an accessible, safe, and affordable environment
while delivering superior life experiences to enhance the quality of life to individuals with developmental disabilities of
all ages. Civitan offers a variety of programs, including Camp Civitan, Recreation Programs, Community Outreach,
Disability Education, Food and Nutrition Education, Day Treatment Programs, and the Transitioning into Employment
Program (TEP).
TEP provides students with a broad spectrum of disabilities (down syndrome, cerebral palsy, autism spectrum disorders,
intellectual disabilities) the skills to secure and maintain gainful employment. The training and work activities take place
in seven separate vocational tracts (Media Design; Fine Arts; Culinary; Retail (Thrift Store); ECommerce; Janitorial; and
Shredding).
2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona
municipalities including past and present support, whether it be through partnerships, collaborations, in-kind
contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If
the applying organization is a municipality governmental sub-division please enter the text “Not applicable.”
Civitan Works closely with the state of Arizona, Maricopa County, and several municipalities through the Department of
Developmental Disabilities and the Department of Education. Civitan receives grant monies to deliver workforce
development, food and nutrition, and various other care programs for individuals with developmental disabilities
throughout the state, all year.
3. Describe the proposed project and planned goals and activities.
Transitioning into Employment is an educational work readiness program for adults with intellectual and developmental
disabilities that desire to be employed. It is specifically designed to educate and train individuals for a variety of work
environments. The program consists of four phases: assessing career aspirations and current skill level; adapted
classroom educational instruction; on the job training; and, job placement.
Program participants are expected to gain and demonstrate job readiness skills and be referred for employment upon
program completion.
1) At least 70% of participants completing twelve (12) months of participation, or their individualized training schedule,
whichever comes first, will be referred for community integrated employment.

Grant Cycle 2017
2) At least 10% of participants participating in this program will be referred for competitive employment without
supports.
4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River
Indian Community.
Civitan Foundation serves people marginalized and underestimated by the majority of society. However, like the Gila
River Indian Community, Civitan believes in working with others, all others, and in the dignity of work. Civitan enhances
the quality of life for individuals with developmental disabilities by providing pathways to meaningful work, and earning
competitive wages.
5. Identify the needs/problems to be addressed, target population and number of people to be served by the
project.
Young adults with development disabilities are often discouraged or discriminated against when it comes to
employment opportunities in the community. The unemployment rate among individuals with developmental
disabilities is 85%. They tend to acquire jobs that are disability typical, and are paid less than minimum wage. Our
students, however, are seeking and acquiring nontraditional employment. An investment of $35,000 from the Gila River
Indian Community will provide the necessary support to ensure one year of programming and related marketing for the
Transitioning into Employment Program (TEP), serving 80 individuals with developmental disabilities, creating
sustainable jobs within the community.
6. Define the project as a new or continuing program.
The TEP is a continuing program.
7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible.
The TEP is an existing, year-round program with no discernable end or beginning. Individual time in the program may
vary, but typically individuals remain in the program for multiple years.
8. Identify other organizations, partners or funders participating in the project and their roles.
Community supporters include: Thunderbirds Charities; ASU's School of Journalism and Mass Media; Arizona
Broadcasters Association; Which Wich sandwich shop; 32 Shea (local charter school lunch contract); Orange Tree Resort;
and Arizona Department of Education.
9. Would you be able to implement the proposed plan if your organization received partial funding for this project?
Civitan Foundation takes its role in the community seriously, as a result it has worked diligently to diversify and
strengthen its revenue sources for programs like the Transitioning into Employment Program. Although, full funding

from the Gila River Indian Community would ensure continued, uninterrupted service and employment to 80 individuals,
even partial funding will ensure service and employment for students. With an investment of $25,000, the Gila River
Indian Community would serve 50 students, and with $10,000; 20
10. Describe your plan for project sustainability beyond the grant period. If this is a program/project previously
funded by the Gila River Indian Community describe efforts made towards the previously described
sustainability plan.
For 49 years, Civitan Foundation has partnered with the greatest champions and advocates of the community, like the
Gila River Indian Community, and served individuals with developmental disabilities. In that time, and through those
partnerships and work, Civitan has created deep, meaningful relationships and fostered trust within the community. In
return, the community has always supported Civitan and its programs with its time, effort, and money. Civitan
Foundation plans to continue to be a reliable and effective partner in serving our community.
11. Describe your plan to document progress and results.
Civitan conducts an initial vocational/career interest inventory with participants that desire to learn the skills for
employment. When participants have mastered the classroom, they are placed in a Civitan center based job in the area
they trained; (culinary, media, cleaning crew, car/van washing, receptionist,
piece work, and SNAP outreach worker). Employees are given a monthly work schedule and they sign in and out daily
and job coaches monitor hour recordings and turn them into the bookkeeper. Civitan's Bookkeeper calculates work
hours and employees are paid on the 10 and 25 of each month.
More than 50% of program participants that are employed, have direct deposit. Every six months, employees go through
a time study to qualify for a pay increase.
Civitan ensures that direct service staff is trained in developing and teaching the employment related skills identified in
the Program Objectives. Staff will complete this by:
* maintaining a copy of each participant's current planning document;
* keep daily progress notes for each participant in the program;
* submit quarterly individualized progress reports and submit to Division of Developmental Disabilities;
* Keep a record of each participant's attendance and number of hours worked;
* Each participant shall successful pass an assessment at the end of each phase; and
* Civitan will maintain data that demonstrates full compliance with all programmatic and contractual requirements of
the Department of Labor and Division of Developmental Disabilities.
12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state
shared revenues for this and any other project for the past five (5) years. If this information is included in a
separate attachment, please indicate that here.
