Provide the narrative in paragraph form in the text field provided. Please be thorough but strive for brevity. 1. Briefly describe the organization’s history, mission and goals. Maricopa County is the nation’s fourth most populous county with an estimated 4.1 million people and is home to 60% of the state’s residents and the city of Phoenix, now the nation’s fifth largest city. Spread out over 9,200 square miles, the county is a mix of urban and rural areas and includes the whole or parts of five Indian Reservations. One of the fastest growing areas of the country during the 1990s and early 2000s, Maricopa County is the state’s major center of political and economic activity. Maricopa County Department of Public Health (MCDPH), created in June 13, 1957, is the public health agency of Maricopa County government. Its mission is to protect and promote the health and well-being of Maricopa County residents and visitors. MCDPH is organized into offices that are housed in five Divisions: Community Health Action, Community Transformation, Disease Control, Clinical Services, Program Operations and Administration. Additionally, the Office of Community Engagement, Office of Performance Improvement, and Public Health Policy Consultant report independently to the director. MCDPH provides an extensive array of clinical and community health services. Major programs include maternal and child health home visiting and community education, parenting education, childhood immunizations, HIV/AIDS counseling and education, STD prevention and treatment, oral health, tobacco use prevention, health promotion and education, WIC and other nutrition and education services, and health care for the homeless. The department has over 700 employees and a current annual (FY 2017) budget of $62.2 million. 2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona municipalities including past and present support, whether it be through partnerships, collaborations, in-kind contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If the applying organization is a municipality governmental sub- division please enter the text “Not applicable.” Not applicable. 3. Describe the proposed project and planned goals and activities. Upon receiving the funding from the Gila River Indian Community grant, the Office of School Health and Wellness Initiatives will develop, implement and evaluate community presentations delivered to the schools and their community. Using Student and Parent feedback, presentations will be developed to address community health concerns with the goal of improving health and well-being of youth and their families. 4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River Indian Community. The Helping Teen Make Critical Choices project would satisfy the education portion of the priority areas by assisting the schools and their families by enhancing the general knowledge of Gila River Indian Community Grant Application Grant Cycle 2017 our participants through additional workshops that focus on teen health, i.e. drug/alcohol use and abuse. The project will also satisfy the area of healthcare including health promotion and wellness activities that the community has requested via health surveys parents complete. 5. Identify the needs/problems to be addressed, target population and number of people to be served by the project. The needs/problems to be addressed include surveys/feedback from parent workshops indicating suggested topics that MCDPH health educators could discuss in future meetings. Unfortunately, current grant requirements do not allow us to address these topics as they arise in the classroom. Therefore, our Helping Teen Make Critical Choices project would allow us to develop and implement the topics identified from community evaluations to meet the needs of the school and community with the intent of promoting life-long learning. We intend to provide the presentations to 650 teens ages 12-19 , their parents and members of the community on a yearly basis. 6. Define the project as a new or continuing program. Maricopa County’s Department of Public Health, Office of School Health and Wellness Initiatives consists of a team of experts that work in school-based programs and assist teens with identifying healthy choices. Through partnerships with our current schools, our staff provides many services and resources to teachers, students, and their parents at many schools within Maricopa County. Our team of health educators works to assist in an integrated school, parent, and community approach to change the health behaviors of the teens of our future. Helping Teens Make Critical Choices project is a new program that will support existing teen health initiatives. 7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible. Upon receiving funds, we will develop surveys to be used with our parent and teen classes to solicit their feedback and create presentations for their schools and community. Our timeline is flexible as we will be working within the school’s calendar year. 8. Identify other organizations, partners or funders participating in the project and their roles. Upon receiving funds, we will develop surveys to be used with our parent and teen classes to solicit their feedback and create presentations for their schools and community. Our timeline is flexible as we will be working within the school’s calendar year. 9. Would you be able to implement the proposed plan if your organization received partial funding for this project? Gila River Indian Community Grant Application Grant Cycle 2017 No, we would not. 10. Describe your plan for project sustainability beyond the grant period. If this is a program/project previously funded by the Gila River Indian Community describe efforts made towards the previously described sustainability plan. Once the surveys and presentations have been created and implemented with the community over the 3 years we will be able to train current school and community based organization staff to continue to present the topics and work with schools to provide training for them to continue with these efforts. 11. Describe your plan to document progress and results. Currently, we implement curricula in schools to help teens develop decision making skills. We would like to work with parents and teens to address additional presentations via surveys in which we would ask for topics that are of concern to them and their communities. We would document the results of the surveys as well as the number of our participants. We would also request feedback on the delivery methods used during our presentations. 12. Indicate any application to and/or awards made by a tribe other than the Gila River Indian Community for state shared revenues for this and any other project for the past five (5) years. If this information is included in a separate attachment, please indicate that here. 1.
