Provide the narrative in paragraph form in the text field provided. Please be thorough but strive for brevity. 1. Briefly describe the organization’s history, mission and goals. Maricopa County Department of Public Health’s (MCDPH) mission is to protect and promote the health and well-being of the citizens of Maricopa County. MCDPH works to prevent chronic diseases by addressing the associated health concerns including poor nutrition, inadequate physical activity, and tobacco use. We strive to meet the Healthy People 2020 goals including access to health services, decrease in cancer, diabetes, heart disease, and stroke, education in exercise, nutrition, and weight management; food safety; increase in immunizations; maternal infant and child health; accident prevention and safety, decrease in substance abuse, teen and adolescent health, and safe transportation. 2. If the applying organization is non-municipal or a non-profit 501c, please describe your relationship with Arizona municipalities including past and present support, whether it be through partnerships, collaborations, in-kind contributions, grants, tax-based funding or other. Describe the supporting municipality’s role in this project. If the applying organization is a municipality governmental sub- division please enter the text “Not applicable.” Not applicable. 3. Describe the proposed project and planned goals and activities. Upon receiving the funding from the Gila River Indian Community grant, the Office of School Health and Wellness Initiatives will purchase two (2) smoothie bikes to provide nutrition education and food demonstrations to school-aged students and families. These bikes incorporate physical activity by having the user pedal, creating the power that blends the smoothie. They can be used at any special events such as health fairs, and family nights or in place of classroom celebrations that often include less-nutritious foods. Our goals for use include targeting the following school districts that share a border or are in close proximity to the Gila River Indian Reservation: Gila Crossing Community School, Kyrene Elementary School District, Laveen Elementary School District, Tempe Elementary School District, and Tolleson Elementary School District. 4. Describe how the proposed project satisfies one or more of the priority funding areas identified by the Gila River Indian Community. The Smoothie Bike project would satisfy the education portion of the priority areas by assisting school districts local wellness policy demands. Each school is required to maintain a Local Wellness Policy to promote student wellness, while preventing and reducing childhood obesity. Gila River Indian Community Grant Application Grant Cycle 2017 It will also satisfy the area of healthcare including health promotion, wellness activities, recreational activities that promote physical activity, and nutrition. 5. Identify the needs/problems to be addressed, target population and number of people to be served by the project. The needs/problems to be addressed include the increasing obesity epidemic, lack of nutrition knowledge, and inadequate physical activity. According to the Center for Disease Control and Prevention, 12.7 million children and adolescents are affected by obesity within the United States. Due to this rising epidemic, each school is required to meet demands of a local wellness policy for schools that participate in the USDA meal programs, ensuring that each student’s health and wellness is a priority. The smoothie bike will meet the demands of the local wellness policy by providing nutrition education, healthy alternatives to classroom parties, and physical activity opportunities. We are targeting districts that border, or are within close proximinity, to the Gila River Indian Reservation including Gila Crossing Community School, Tolleson Elementary School District, Kyrene Elementary School District, Tempe Elementary School District, Laveen Elementary School District, and Fowler Elementary School District. A total of at least 41,578 students can be served through this project. The article below is an example of a successful smoothie bike nutrition and physical activity program: http://www.grandhaventribune.com/Education/2017/02/16/Students-use-bike-to-make- smoothies.html 6. Define the project as a new or continuing program. Maricopa County’s Department of Public Health, Office of School Health and Wellness Initiatives consists of a team of experts in the field of nutrition and physical activity. Through a partnership with the Arizona Nutrition Network, our staff provides many services and resources to teachers, students, and their parents at over 200 schools within Maricopa County. Our team of Registered Dietitians (RD) work to assist in an integrated school, parent, and community approach to change the health behaviors of the children of our future. The Smoothie Bike would be a new initiative within the work that is currently taking place within this school health, nutrition, and physical activity programs. 7. Provide a brief timeline including start and finish dates. Indicate if the timeline is flexible. Upon receiving funds, we intend to purchase two smoothie bikes immediately. We also have support from Maricopa County Department of Public Health. Employees within the Office of School Health and Wellness Initiatives will track data, use, and maintenance of smoothie bikes. 9. Would you be able to implement the proposed plan if your organization received partial funding for this project? No, all funding would be necessary for the purchase of the smoothie bikes. 10.
